    Pandemic influenza (H1N1) 2009 - Update 3 August 2009, 17:00 hours CEST


    Main developments in past 24 hours

    • 864 new cases reported from the EU and EFTA countries, reaching a total of 27 337 cases;
    • 2 445 new cases including 12 fatal cases, were reported from non-EU and EFTA countries, reaching a cumulative total of 160 802 cases.


    This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 3 August 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

    Disclaimer:

    The number of confirmed cases reported is based on laboratory test results, except for the US. Depending on the national laboratory testing policies, the actual number of cases by country may therefore be higher. Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


    Epidemiologic update

    Out of the 31 EU and EFTA countries, 30 countries have reported a total of 27 337 confirmed cases of influenza A(H1N1)v infection, including 40 deaths. In the past 24 hours, 864 new cases were reported from the EU and EFTA countries.

    Outside of the EU and EFTA countries, a total of 2 445 new cases have been reported within the last 24 hours. In total, 160 802 cases including 1 225 fatal cases have been reported from non-EU and EFTA countries. Pakistan reported the first case. 12 new fatal cases were reported today; South Africa reported the country?s first fatal case and other fatal cases were reported from Australia (4), New Zealand (1), Costa Rica (1), Dominican Republic (2), Jamaica (1), Israel (1), Malaysia (1) and Singapore(1)

    Globally, the total number of confirmed and reported influenza A(H1N1)v cases to date is 188 139, including 1 265 deaths.

      Globally, the total number of confirmed and reported influenza A(H1N1)v cases to date is 193 574, including 1 362 deaths.

        Outside of the EU and EFTA countries, a total of 4 499 new cases have been reported within the last 24 hours. In total, 170 126 cases including 1 402 fatal cases have been reported from non-EU and EFTA countries. 82 fatal cases were reported from Australia (4), Costa Rica (3), El Salvador (3), Spain (1), Saudi Arabia (2), Bolivia (2), Brazil (40), Ecuador (1), Peru (3), Indonesia (2), Malaysia (3) and Thailand (16).

        Globally, the total number of confirmed and reported influenza A(H1N1)v cases to date is 199 034, including 1 444 deaths.


        Public Health Developments

        Pandemic influenza A(H1N1) 2009 in pregnancy places women at higher risk of adverse outcome - published analytic study from the United States

        Earlier this year the US Centers for Disease Control and Prevention (CDC) reported cases of influenza A(H1N1)v virus infection among pregnant women in the US. A number of these infections resulted in complications or even death. Working with American state authorities and clinicians, CDC undertook an analytic study of the first month of transmission in the USA (15 April ? 18 May) which has now been published. The analytic study compared the experience of three groups of infected people: the 34 pregnant women, non-pregnant women of reproductive age (15?44 years old), and the general population. The estimated rate of admission for influenza A(H1N1)v virus infection in pregnant women during the first month of the outbreak was higher both than for non-pregnant infected women and for the general population. These findings indicate that pregnant women deserve particular care during a pandemic and some countries have already issued guidance for maternity services and clinicians.

          Main developments in past 24 hours

          • 1 421 new cases reported from the EU and EFTA countries, reaching a total of 30 310 cases;
          • 1 730 new cases including 107 fatal cases, were reported from non-EU and EFTA countries, reaching a cumulative total of 171 856 cases;
          • WHO issues briefing notes on the safety and manufacturing process of the pandemic vaccine.


          This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 6 August 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

          Disclaimer:

          Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


          Epidemiologic update

          Out of the 31 EU and EFTA countries, 31 countries have reported a total of 30 310 confirmed cases of influenza A(H1N1)v infection, including 42 deaths. In the past 24 hours, 1 421 new cases were reported from the EU and EFTA countries. The first five cases in Liechtenstein were recorded.

          Outside of the EU and EFTA countries, a total of 1 730 new cases have been reported within the last 24 hours. 107 fatal cases were reported from Australia (3), Jamaica (1), Argentina (94), Bolivia (1), Paraguay (3), Peru (2) and Malaysia (3).

          In total, 171 856 cases including 1 509 fatal cases have been reported from non-EU and EFTA countries.

          Globally, the total number of confirmed and reported influenza A(H1N1)v cases to date is 202 151, including 1 550 deaths.


          Other updates

          The World Health Organization (WHO) issued a briefing note on the safety of pandemic vaccines and the vaccine manufacturing process.

            EU sentinel surveillance update

            The weekly influenza surveillance bulletin from EISN has been published on the ECDC website. All of the 84 subtyped influenza viruses detected by sentinel networks in week 31/2009 were A(H1N1)v, and the UK (Northern Ireland) reported medium activity for the first time. Ireland and the UK (England, Northern Ireland) are the only countries that have reported influenza activity above their national baseline levels since the pandemic was declared.

            In this week's edition of Eurosurveillance an article from Australia presents influenza trends observed during the first 11 weeks of sentinel surveillance, indicating that from the eighth week of surveillance (calendar week 25) more than 90% of detected influenza viruses were confirmed as pandemic influenza (H1N1). The median age of patients with pandemic influenza was 21 years (range 2-63 years) compared with the median age of patients with seasonal (H1N1) influenza in 2007 or 2008 of 23 years (range 1-75 years).             A report from Germany describes the early stage of the pandemic (H1N1) 2009 with 198 cases confirmed until 16 June, almost half of these imported, mainly from Mexico and the United States. About two thirds of domestic cases were outbreak-related (including two large school outbreaks). The number of cases has increased rapidly since mid-July (about 8,000 confirmed cases notified until 5 August) but the overall picture has not changed considerably since the recent increase is mainly due to travellers (79% imported cases). http://www.eurosurveillance.org/View...rticleId=19295

            An article analysing the pandemic (H1N1) 2009 in Thailand gives estimates of the basic reproduction ratio (2.07 and 1.78 for two different generation intervals) and case fatality ratio (0.58%). These estimates are comparable with those reported in the literature on the initial outbreak in Mexico but the CFR is higher than that recently calculated for developed countries. http://www.eurosurveillance.org/View...rticleId=19292

            Two papers share experiences of recent mass gathering events in Serbia and Belgium where cases of pandemic influenza (H1N1) were detected. Both articles stress the importance of timely communication which made it possible to identify cases. It is likely that the mass gathering events in both countries had an impact on the local epidemiological situation, however, as the pandemic unfolds, community transmission seems unavoidable and the mitigation approach most appropriate.
            Preparedness planning for two large mass gatherings events were considered in Serbia in the context of pandemic influenza (H1N1) 2009. Planning included approaches to prevention, detection and response in order to mitigate the situation at this early stage of the epidemic in Serbia. Cases of influenza A(H1N1)v were identified nationally immediately prior to the mass gatherings but also identified in association with both events, as expected in the context of the pandemic situation. This article describes the experiences of planning and the epidemiological situation during the period of the mass gathering events.

            On 6 July 2009 the Belgian enhanced surveillance system for influenza-like illness among travellers returning from influenza A(H1N1)v affected areas detected a case linked to a rock festival which took place on 2-5 July. The health authorities implemented communication and control measures leading to the detection of aditional cases. This paper describes the outbreak and its impact on the management of the influenza pandemic in Belgium.



            Other updates

            Changes to situation report next week

            ECDC will refresh the format of the Situation Update on Monday, August 10, to better accommodate current information needs and prepare for anticipated needs as the pandemic evolves within Europe. The name of this report will change into "Daily update". We will discontinue reporting number of cases outside Europe, since many countries have already stopped updating their totals, and the figures are becoming increasingly less informative. This will help focus ECDC's epidemic intelligence and surveillance into a more analytical overview of the European data and the evolving seriousness of the pandemic. Worldwide monitoring is nevertheless provided by international organisations on regular basis. A new table to report on the number of hospitalized cases and cases admitted for intensive care in EU member states, based on publicly available reports from national official sites will be included. The other tables, graphs and maps will change accordingly. We also will begin including summaries of the "Scientific Advances", "Public Health Developments", and the sentinel surveillance reports on a weekly basis on given days. In addition, we are developing a short report to weekly monitor in-depth the evolution of the pandemic in selected countries outside the EU.

              Main developments in past 24 hours

              • 261 new cases reported from the EU and EFTA countries, reaching a total of 32 636 cases;
              • Ireland reported the first fatal case;
              • 943 new cases including 103 fatal cases, were reported from non-EU and EFTA countries,


              This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 7 August 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

              Disclaimer:

              Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


              Epidemiologic update

              Out of the 31 EU and EFTA countries, 31 countries have reported a total of 32 636 confirmed cases of influenza A(H1N1)v infection, including 43 deaths. In the past 24 hours, 261 new cases were reported from the EU and EFTA countries; Ireland reported the first fatal case.

              Outside of the EU and EFTA countries, a total of 943 new cases have been reported within the last 24 hours. 103 fatal cases were reported from Costa Rica (2), Ecuador (2), Ghana (1), Malaysia (4), Occupied Palestinian Territory (1), Paraguay (5), Peru (4), Samoa (1), USA (83).

              In total, 174 351 cases including 1 630 fatal cases have been reported from non-EU and EFTA countries.

              Globally, the total number of confirmed and reported influenza A(H1N1)v cases to date is 206 987, including 1 673 deaths.

                This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 7 August 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

                Disclaimer:

                Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


                Epidemiologic update

                All 31 EU and EFTA countries have reported cases, with a total of 32 690 confirmed cases of influenza A(H1N1)v infection, including 43 deaths. In the past 24 hours, 54 new cases were reported from the EU and EFTA countries.

                Outside of the EU and EFTA countries, a total of 1 017 new cases have been reported within the last 24 hours.

                Fifteen (15) fatal cases were reported from Argentina (1), Iraq (1), Israel (3), Malaysia (8), Singapore (1), South Africa (1); Iraq reported the first fatal case.

                In total, 175 368 cases including 1 645 fatal cases have been reported from non-EU and EFTA countries. Globally, the total number of confirmed and reported influenza A(H1N1)v cases to date is 208 058, including 1 688 deaths.

                          UK publishes the pandemic vaccination priority groups

                          UK announced the priority groups for pandemic vaccination, which are in the following groups in this order (numbers given are approximate and are for England only):

                          1. People aged over six months and under 65 years in current seasonal flu vaccine clinical at-risk groups (about 5 million people). Vaccination of frontline health and social care workers (approximately 2 million people) will begin at the same time.
                          2. All pregnant women, subject to licensing conditions on trimesters (about 0.5 million people).
                          3. Household contacts of people with compromised immune systems e.g. people in regular close contact with patients on treatment for cancer (about 0.5 million people).
                          4. People aged 65 and over in the current seasonal flu vaccine clinical at-risk groups (about 3.5 million people). This does not include otherwise healthy over 65s, since they appear to have some natural immunity to the virus.

                          Overall, more than 11 million people fall into these priority groups.

                          Switzerland publishes vaccination priority groups

                          According to a media report, Switzerland has published the national priority groups for pandemic influenza vaccination. The priority groups are (in the following order):
                          1. Health care workers and carers of infants under 6 years of age;
                          2. Pregnant women, mothers and children as well as adults under 65 years of age with chronic heart, lung or metabolic diseases;
                          3. Family members of patients with chronic diseases and family members of infants;
                          4. Adults 65 years or older with chronic diseases

                          Overall, 1.2 to 2 million people fall into these categories according to the Federal Office of Public Health.


                          Finnish media discusses additional requirements on a pandemic vaccine clinical trial

                          Media in Finland discusses additional requirements placed by the National Agency of Medicines on a clinical trial on pandemic vaccine efficacy and safety in children. According to the media reports, children over 6 years of age will be required to get their own information sheet, written consent will be required from both parents and all girls over 12 years of age will be required to undergo pregnancy tests.

                            Main developments in past 24 hours

                            • 346 new cases reported from the EU and EFTA countries, reaching a total of 37 999 cases.
                            • Spain reported 1 new death;
                            • 1 327 new cases including 68 fatal cases, were reported from non-EU and EFTA countries, reaching a cumulative total of 189 563 cases;


                            This report is based on official information provided by the national public health websites, or through other official communication channels. An update on the number of confirmed cases as of 15 August 2009, 16:00 hours CEST, is presented in Table 1 and Table 2.

                            Disclaimer:

                            Several countries have now announced recommendations on treatment on clinical signs only and focused laboratory testing only for specific population groups or in outbreaks. For these countries, the reported numbers of cases presented in this report will severely underestimate the true incidence in the country and will not be comparable to countries still recommending laboratory tests of all suspected influenza cases.


                            Epidemiologic update

                            Out of the 27 EU and 4 EFTA countries, all countries have reported a total of 37 999 confirmed cases of influenza A(H1N1)v infection, including 60 deaths (Table 1). In the past 24 hours, 346 new cases, including one fatal case from Spain, were reported from the EU and EFTA countries. Currently, the majority of the cases are being reported from the UK, Netherlands, Greece and Germany. During this week, we have identified reports of currently hospitalised cases from 6 countries. The majority of the hospitalized cases have been reported from UK, specifically from England.

                            Outside of the EU and EFTA countries, a total of 1 327 new cases have been reported within the last 24 hours. 68 fatal cases were also reported (Table 3). In total, 189 563 cases including 2 013 fatal cases have been reported from non-EU and EFTA countries. Media reports discuss increasing numbers of cases being reported in countries on the northern and eastern side of the Indian Ocean. Many Central and South American countries have passed the peak incidence period, according to media reports and supported by the official reports.

                            Globally, the total number of confirmed and reported influenza A (H1N1)v cases to date is 227 562 including 2 073 deaths.


                            ECDC weekly influenza surveillance bulletin

                            The weekly influenza surveillance bulletin has been published on the ECDC website.

                            All of the 81 subtyped influenza A viruses detected by sentinel networks in week 32/2009 were A(H1N1)v. Ireland and the UK (England, Northern Ireland) are the only countries that have reported influenza activity above their national baseline levels since the pandemic was declared.

                            Other developments

                            Extension of the pandemic

                            The pandemic continues to affect an increasing number of countries. Over this past week, Cameroon, Madagascar, East Timor and Zambia reported their first confirmed cases of influenza A (H1N1)v. During the same period Japan, South Korea, Nicaragua, Mauritius, Iraq, Samoa, Ghana, Occupied Palestine Territory and Ireland all confirmed their first death due to this virus.


                            Changing the format of the daily report

                            As the influenza pandemic evolves, new types of data become increasingly important, whereas other may become less interesting. ECDC has refreshed the format of the Situation Update to better accommodate current and anticipated information needs. The name of this report changed to "Daily Update" and some changes have been made in the data reported.

                            Table 1 ? reported cases by the EU/EFTA countries remains as before. In addition, we are publishing a new Table 2 on the number of current and cumulative hospitalisations and admission to intensive care in EU/EFTA Member States. The source of the information is official statements or surveillance reports published by national health authorities. It should be noted that indications and thresholds for hospitalisations vary by country, so the figures quoted may not be directly comparable between countries.

                            Assuming no changes in the national reporting system, trends of hospitalisations over time should give an accurate representation of the evolution of the pandemic. This might change if countries are faced with capacity issues within the healthcare system at a later stage of this evolving pandemic.

                            Table 3 lists only reported deaths for countries outside the EU/EFTA area; we have discontinued reporting the case numbers for these countries. The main reason for this is that more and more countries have stopped the reporting of new cases, and these figures no longer give a representative picture of the situation.

                            The two graphs now show number of reported deaths by week of report ? one graph for EU/EFTA, one for the entire world. The two maps depict cumulative deaths by country, one for EU/EFTA, one for the world.
