MDA prepares for bird flu in humans
<hr size="1"><table border="0" cellpadding="0" cellspacing="0" width="100&#37;"><tbody><tr><td>Judy Siegel-Itzkovich , THE JERUSALEM POST </td><td align="right">Jan. 3, 2008</td></tr></tbody></table><hr size="1">Eighteen chickens have been found dead from bird flu in a Binyamina kindergarten's animal corner, while two others were unaffected. The Health Ministry, which is investigating but has stressed there is no reason to panic, said on Thursday that the animal corner had been completely closed, and that no one - including the children - had had any direct contact with the fowl except for the keeper.

However, Channel 1 reported Thursday night that a four-year-old girl who attended the kindergarten had been hospitalized with flu-like symptoms.
Haifa district health office workers in the area arrived at Gan Batya's animal corner early on Thursday to conduct an epidemiological examination and determine whether to recommend a medication called Tamiflu as a prophylactic measure. There is no vaccine against bird flu.

The kindergarten teacher alerted the district health office when she discovered the chickens had died - but not the ducks or pigeons - and the authorities closed the kindergarten for a day. Magen David Adom announced that it would prepare for the possibility of human infection. MDA teams across the country were instructed to refresh procedures and ensure the presence of protective measures and sterilization substances in their ambulances.

Teams in the Binyamina area received briefings on disease symptoms and cautionary steps. In addition, MDA has prepared special enclosed beds, resembling large incubators, to allow safe transfer for people suspected of infection.

The Health Ministry, meanwhile, reiterated basic guidelines to prevent bird flu infection. Consumers should buy food products only in recognized, licensed stores under veterinary supervision, hands should be washed thoroughly before and after handling food and raw foods, including eggs, should be fully cooked.

Symptoms of bird flu (H5N1 virus) include an unexplained acute lower-respiratory infection. People who have been in contact with other poultry - not those in the Binyamina kindergarten, but within three kilometers of it - should be alert to any symptoms of fever within a week of contact with the fowl. If a fever of over 38 degrees Celsius develops, one should go to a hospital emergency room and tell staff about the possibility of bird flu.

Ministry officials said it had no other reports of fowl deaths in the area. The ministry said it had not changed the level of alertness to bird flu, which is not a human epidemic. Only if the virus results in human-to-human infection will the authorities be worried, and this has not happened.

The process, which is expected to be completed before Shabbat enters, will be carried out at a chicken coop belonging to the Muchersky family, the only one located within a two-mile radius of the kindergarten.

On Thursday some 200 chickens were culled in Binyamina.

The Agriculture Ministry said lab tests of samples taken from chicken coops in the area were negative, but it will go ahead with the culling as a safety measure against the spreading of the bird flu virus. The chicken coop owners will be compensated, the ministry added.

The chickens are killed by electrocution in less than a second, in accordance with local animal rights regulations. Parrots and birds will be beheaded or receive a lethal injection.

Once destroyed, the culled poultry will be buried in a pit dug especially for that purpose.

"The chickens are being electrocuted; there are a lot of people outside, but I don?t have the mental strength to watch ? it's very difficult," Nava Muchersky said. "We have no idea what the future holds for us; this is our livelihood."
A four-year-old girl who attended the kindergarten was admitted to the Hillel Jaffe Medical Center in Hadera Thursday with flu-like symptoms. Lab results showed that she had not contracted the bird-flu virus but was rather suffering from the Respiratory Syncytial Virus, the most common cause of bronchiolitis and pneumonia among young children.

Jerusalem, January 4 (Xinhua) Israeli Civil Administration handed over the Palestinian Authority 65 thousand injection to immunize thousands of poultry and animals in the areas of the West Bank and fear of the spread of the bird flu virus in the West Bank areas and then he moved to inside Israel.

Was detected on the first infected bird flu on Thursday in the Binyamina area kindergarten, central Israel, where the girl was infected by the afternoon high temperature and shortness of the respiratory system.

The cull orders came following the outbreak of the human-lethal H5N1 strain of the avian flu in the petting zoo of a Binyamina kindergarten.
All kindergarten and veterinary staff were given preventative treatment on Thursday, even though authorities were keen to emphasize that there was no reason to seriously fear for their health.
At one point a 4-year old girl was hospitalized with flue like symptoms, only to be diagnosed as suffering from unrelated pnumonia. The kindergarten itself was opened Friday afternoon, after disinfection and cleaning procedures were completed. Eighteen chickens have been found dead from bird flu in a Binyamina kindergarten's animal corner on Thursday.
The special incubator-beds prepared by MDA to treat potential avian flu victims.

<!-- display the rest of the paragraphs -->
Symptoms of(bird flu H5N1 virus) include an unexplained acute lower-respiratory infection. Only if the virus results in human-to-human infection will the authorities be worried, and this has not happened. http://www.jpost.com/servlet/Satellite?c=JPArticle&cid=1198517286735&pagename=J Post%2FJPArticle%2FShowFull

A teacher at a nearby kindergarten noticed the dead birds and alerted the veterinary services.
Professor Samuel Rishpon, chairman of the Haifa district health department told reporters on the same day "Our investigation indicates that none of the children, parents or the staff in the kindergarten came in direct contact with the birds, yet we've instructed the hospitals in the Haifa area to be on alert for children showing symptoms common of chicken flu."
Staff at the kindergarten were given preventive treatment and are under observation.
The Israeli veterinary services immediately began culling all birds within a 3km radius, in accordance with international health regulations.
Quarantine orders were issued to poultry farms within a 10km radius. Instructions to the general public on how to deal with avian flu were published on all news sites and television channels.
Israeli authorities are constantly updating their methods of dealing with avian flu after the March 2006 avian flu outbreak which was successfully contained. At the time 1.25 million birds were culled in 13 different locations. No humans were infected, according to the health authorities..
Dafna Yurista, a spokesperson for the Israeli Ministry of Agriculture said that "None of the samples from the culled birds showed positive results and the ministry will compensate chicken farm owners whose poultry has been culled."
Eynav Shimron-greenbaum, a spokesperson for the Health Ministry issued a statement on Thursday stressing that the infected birds were contained in one chicken enclosure and, therefore, the danger of contamination was minimal. The ministry is monitoring the situation, added Shimron-greenbaum.
The ministry has also said that in order to contain the disease from spreading into the Occupied Palestinian Territories (oPt), it will cooperate with the Palestinian Authority's veterinary services if needed, including analyzing samples from oPt in Israeli veterinary service laboratories.

                          1. Immediate notification (3 Jan 2008)
                          2. Follow-up report no. 1 (7 Jan 2008)

Outbreaks: There are no new outbreaks in this report

                          Epidemiology
Source of infection: unknown or inconclusive

                          Epidemiological comments: A 3-km (2 mi)-radius protection zone and a
10-km (6 mi)-radius surveillance zone were set up.

The epidemiological investigation did not reveal evidence of
introduction of new birds into the cage during the 40 days prior to
the outbreak. Apparently, people who were taking care of the pet
birds had no contact with other birds during the 40 days prior to the
outbreak. The birds' feed containers remained closed and there was no
apparent possibility of entrance of wild birds into the feed
containers (which were situated outside the cage). Sparrows were
abundant in the cage during the feeding of the pet birds.

Increase of monitoring activities of sick and dead wild birds in the zone.

                          Control measures
                          Measures already applied
                          - stamping out
                          - quarantine
                          - movement control inside the country

[The timely and efficient manner by which this outbreak at Binyamina
(map included in the report) was eradicated, has become possible by
the immediate application of pre-prepared contingency plans; a
significant contribution for its success was the behaviour of the
owner, who called upon a vet immediately when 3 chickens died, and the
response of the vet who involved the laboratory, even though avian
                          influenza was initially not suspected.

The main puzzling issue in this outbreak the source of infection.

                          ProMED-mail's posting 20080104.0045 included a moderator's comment
regarding the point in time when 3 hens were introduced from a
broiler breeding flock in Kefar Menachem
                          (south-east Israel; see map
                          at
                          <http://www.fallingrain.com/world/IS/1/Kefar_Menahem.html>).

It seems that the media report cited, according to which the transfer
took place a week before the discovery of the 1st 3 dead chickens,
was inaccurate, since the follow-up report above says: "The
epidemiological investigation did not reveal evidence of introduction
of new birds into the cage during the 40 days prior to the outbreak
                          ".
This is in line with the epidemiological investigation report of 3
Dec 2007, kindly forwarded to us by the Israeli Veterinary Services,
which indicates that the exact date of the transfer cited above (3
hens) was 20 Nov 2007. Reportedly, the breeding flock of origin was
(routinely) slaughtered during the beginning of December (2007), and
in any case prior to 9 Dec 2007, without observable morbidity or
                          exceptional mortality.

The epidemiological investigation (carried out commonly by the
Ministries of Agriculture and Health) concludes as follows:

"It is plausible to conclude that the disease was not introduced by
the animals brought from Kefar Menachem. This, in view of the long
time lapse and the fact that no exceptional event was seen in Kefar
                          Menachem.

No other suspected contacts could be traced, either by birds or by
humans. Since the cage's roof (cover) was impervious, it is not
plausible that droppings from birds flying over the cage could have
entered it. On the other hand, the free entrance of sparrows, in
large numbers, makes them [a] suspected source."

Hopefully, this will remain a solitary event. Though the source of
infection remains enigmatic, sequencing results and their comparison
with other isolates from the region (such as, Egypt, Saudi Arabia)
may help in elucidating the route of virus spread. - Mod.AS]
