Bird Flu A Public Health Risk

    JUNE 11, 2012

Avian influenza is one of Indonesia's urgent public health risks, in addition to natural disasters. Therefore, the Government implemented the National Avian Influenza Prevention and Control Program. In addition, because of potentially zoonotic disease pandemic, the Government of Indonesia has established a National Committee on Zoonoses sectoral based on Presidential Decree.

This was conveyed by Plt. Minister of Health, Prof. dr. Ghufron Mukti Ali, MSc., PhD., At the opening activities of Influenza Symposium conjunction with IIF 4th Anniversary from Prevention to Treatment: Past, Present and Future Change in London (09.06.12).

Presented, over the past two decades, the world community face the challenges associated with the spread of emerging infectious diseases the causes of microorganisms (EID), one of which is avian influenza (AI). In Indonesia, mortality and morbidity due to FB is relatively low compared with other infectious diseases such as tuberculosis and malaria. However, FB threats to human security is greater than any other infectious disease, because there is the possibility of mutation and initiate a pandemic influenza. When pandemic influenza occurs, the impact will affect all countries in the world. It is estimated there will be deaths in large numbers, drugs, inadequate social and economic disruption. Therefore, each state should prevent it happening and always anticipate. "We remember the pandemic is severe acute respiratory syndrome (SARS) in 2002-2003 and Pandemic Influenza H1N1 in 2009. The second pandemic has an impact not only health but also on economic and social aspects ", said Prof. Ghufron.

According to Prof. Ghufron, SARS pandemic is also behind the revision of regulations and implementation of International Health Regulations (IHR) 2005 at the World Health Assembly (WHA) to-58. Key points of the IHR 2005, among others, is developing the core capacities of member countries for the detection of Public Health Risk of International Concern (PHRIC) and the response to the Public Health Emergency of International Concern (PHEIC). It also gives special attention to the possible spread of infectious diseases with pandemic potential, in addition to the threat of chemical and radioactive pollution.

"Today, most of Indonesia, including medical professionals, believe that the Influenza in Indonesia is a mild disease is self-limiting disease. To date, Indonesia has not had a specific influenza immunization program. Vaccination for seasonal influenza are given only for the people of Indonesia who are traveling to Saudi Arabia to perform Hajj ", explained Prof. Ghufron.

Extract data surveillance of Influenza Like Illness (ILI) in 24 health centers and the Severe Acute Respiratory Infection (SARI) in 30 hospitals Indonesia reported by the National Institutes of Health Research and Development (2012), showed an increase of influenza cases from October 2011 until February 2012.

Prof. Ghufron added, in essence, the world community had been three times as much as Pandemic Influenza, the Spanish Flu (1918), Asian Flu (1957) and Hong Kong Flu (1968). Some research data on pandemic influenza in Indonesia said, around 1980, Australian historian, Collin Brown published a paper titled The Influenza Pandemic of 1918 in Indonesia. The paper provides an overview of events during this period that some 1.5 million people died in Indonesia. Furthermore, Han Gan Koen, et al. states that from April to August 1957, outbreaks of influenza have occurred in Jakarta. They studied the epidemic and found that the so-called Asian Flu is classified as having mild disease.

Cases of Influenza H5N1 found in poultry in 33 provinces in Indonesia. Since first reported in July 2005, the cumulative number of cases reached 189 with FB 157 deaths. Meanwhile, during January-May 2012, the number of reported cases of FB as much as six cases with six deaths. This year, the province of Bengkulu and NTB reported cases of AI in humans.

At the end of his speech Prof. Ghufron stated, the Government of Indonesia has taken several steps influenza pandemic preparedness and response, among other 100 Strengthening Avian Influenza referral hospitals; strengthening of epidemiology and virology laboratories ; Application of sentinel and routine supervision; Vaccine Development: Implementation of the ecology and transmission, clinical spectrum, management and disease molecular genetics ; Training: Build coordination, and setting and distribution logistics.

"Indonesia and other developing countries have succeeded in realizing the fair, transparent, and equitable framework in the WHO system, "said Prof. Ghufron.
Framework for the fair, transparent, and equitable framework that established the World Health Assembly on the 64th WHA in 2011 in resolution WHA64.5 on Pandemic Influenza Preparedness: Sharing of virus and sharing of vaccines and other benefits. This framework guarantees the right of all countries developed to obtain equal access to affordable vaccines, antiviral, and diagnostic ki t during an influenza pandemic as well as assisted in their vaccine production capacity.

On occasion, Prof Ghufron also convey the importance of involving all parties, both at the central, local and private sector in pandemic influenza preparedness efforts.
