Working to control the spread of bird flu

The Food and Agriculture Organization?s chief veterinary officer has indicated that, from a veterinary perspective, the world is facing an unprecedented crisis in the form of avian influenza (AI). The situation in Indonesia is particularly dire because of deaths in North Sumatra and the likely possibility of human-to-human transmission of the most virulent form of AI, the H5N1 virus.

In response to this emergency, USAID launched the Community-Based AI Control Activities (CBAIC). CBAIC focuses on three critical, interrelated elements:

- Strengthening AI planning and pandemic preparedness at the national, provincial, and local government levels in coordination with the National Committee for AI;

- Improving management and coordination of village- and community-level activities for animal and human surveillance and response; and

- Fine-tuning and expanding behavior change communication programs for AI target populations, the private sector, and government officials.

In Indonesia, a community-level approach to AI disease control must overcome many challenges. For example, rural communities have limited knowledge of Highly Pathogenic Avian Influenza. Moreover, because of the free-ranging nature of smallholder poultry farming, it is difficult to involve many of these poultry producers in disease control programs. Often they are unable to tell the difference between AI, Newcastle, or other less fatal poultry diseases; they have low levels of bio-security; and they generally know little about AI prevention and surveillance techniques. In addition, because many poultry farmers do not know how to identify AI symptoms or understand the need for laboratory tests to confirm the presence of AI, outbreaks have not always been reported quickly enough. There also are economic disincentives to reporting outbreaks in a timely fashion to local authorities. Finally, even in cases where the knowledge and willingness to report AI outbreaks exist in rural Indonesia, the resources to change husbandry practices (for example, bio-security measures) are scarce or unavailable.

DAI is working with USAID, the Government of Indonesia, and other donors to refine and expand behavior change communication programs to educate poultry producers and community members about AI and to show them how best to respond to outbreaks. At the same time, we are strengthening the planning and coordination activities of the national AI Prevention and Preparedness Committee to measurably reduce the incidence of AI outbreaks and quickly limit the scale and scope of outbreaks that do occur. DAI is developing a clear and targeted public awareness and information campaign that covers a wide range of issues, such as:

- AI surveillance;

- Identification and reporting;

- Outbreak response, including culling and control of animal movements; and

- Post-outbreak actions, including possible vaccination and post-vaccination monitoring and restocking.

DAI provides grants to local nongovernmental organizations involved in training and communication, especially for the backyard poultry sector, in support of improved bio-security, vaccination, destocking, compensation, surveillance/reporting, and public awareness.
