Officials of a medical college in India’s western state of Rajasthan have revealed that an octogenarian lady was tested positive for Zika virus infection, but was sent home after symptoms improved. There has been no confirmation, though, on whether other suspected cases are being monitored.

In a fresh case of Zika infection, an 85-year-old woman in the Indian state of Rajasthan, which is a popular tourist destination, has tested positive for the mosquito-borne virus. The hospital administration has alerted the government on the spotting of the virus and steps are being taken to contain its spread.

The resurfacing of the virus in India was reported by Indian news agency PTI on Sunday, quoting the Sawai Man Singh (SMS) hospital administration in Jaipur, the capital of Rajasthan. Officials of the medical college have not given any further details on the possibility of any more cases of the virus infection.

A 17-year-old boy of Shastri Nagar area in Jaipur district has tested positive for Zika virus, said Dr VK Mathur, director of the public health department. This is the 2nd positive case of Zika virus in the area in less than a fortnight. The 1st case was reported on [21 Sep 2018].

"We sent samples of suspected cases to the National Institute of Virology [NIV], Pune, and one of them has tested positive for Zika," Dr Mathur said. The confirmation from the NIV came on [Mon 1 Oct 2018] evening, he added.

Sawai Man Singh Medical College principal Dr US Agarwal said 8 more samples of suspected cases have been sent to the NIV Pune, and the results are awaited.

Six Zika cases have been confirmed in India to date by the Union health ministry surveillance network: 3 from Gujarat in 2017, one from Tamil Nadu in February 2018, and 2 from Rajasthan in September this year [2018].

Zika surveillance was set up in 2016, when the World Health Organization declared Zika an outbreak.

"There is a high-level team of experts on the ground monitoring the situation closely. These cases were mild infections and recovered on their own. There has been no case of microcephaly reported due to Zika so far anywhere from the country," said a senior health ministry official, requesting anonymity.

[Given another case detection, physicians in the affected district need to consider Zika virus in patients with compatible symptoms, particularly in pregnant women who are at risk of Zika associated complications including preterm birth, miscarriage and microcephaly in newborns. Adults and children are also at increased risk for Zika-associated neurological complications, including Guillain-Barre syndrome, neuropathy and myelitis (http://www.who.int/news-room/fact-sh...ail/zika-virus).

According to the latest WHO Zika virus classification, India falls into Category 2, which is an area either with evidence of virus circulation before 2015 or area with ongoing transmission that is no longer in the new or re-introduction phase but where there is no evidence of interruption (http://apps.who.int/iris/bitstream/h...B05?sequence=1). 50 pregnant women in Jaipur test positive for Zika – but India lacks protocol to monitor them
Zika infections in pregnant women can lead to birth defects in babies. India’s guidelines on Zika are silent on how to tackle this.
                          ...
Since September 23, 140 people have tested positive for Zika in the capital of Rajasthan. Fifty of them are pregnant women like Farhana. It is unclear how many of them know about their infection and the associated risks.

The World Health Organisation recommends ultrasound scans at around 18-20 weeks of pregnancy for Zika positive pregnant women to detect microcephaly in the foetus. It also recommends screening the babies born to them for microcephaly and other birth defects, and tracking their health for two years to identify developmental delays and other neurological abnormalities.

While India’s Union health ministry released national guidelines in February 2016, on surveillance, response and risk communication in case of a Zika outbreak, the document is silent on how pregnant women should be monitored and counselled.

Centre sends health experts to Madhya Pradesh to verify reports of Zika infection
Zika outbreaks have been reported in Rajasthan and Gujarat in recent weeks.

The Union Health Ministry has sent a team of experts to Madhya Pradesh to verify reports of Zika virus cases in the state, PTI reported on Thursday. A team from the National Centre for Disease Control and the Emergency Medical Response was dispatched after local authorities said three people had tested positive for the virus, an unidentified official said.

The development comes on the heels of the virus being detected in Rajasthan and Gujarat. According to PTI, 153 people in Rajasthan are infected with Zika. Fifty of them are pregnant women. Zika infection can lead to birth abnormalities in babies born to pregnant women.

In India, the first outbreak of Zika virus was reported in Ahmedabad in January 2017 and the second in Tamil Nadu’s Krishnagiri district in July that year.
After Jaipur and Ahmedabad, the Union Health Ministry has now sent a team of experts to Madhya Pradesh to confirm and verify reports of Zika virus cases in the state.

A central team comprising of experts from the National Centre for Disease Control (NCDC) and the Emergency Medical Response (EMR) have gone to the state following information about three persons being detected with Zika virus, a Health Ministry official said.

“The team will verify the facts and if the information is true, it will assist the Madhya Pradesh government to replicate the measures and action plan, implemented in Jaipur and Ahmedabad, there, to prevent any possible outbreak of Zika,” he said.

Zika Virus strain that causes microcephaly not found in Rajasthan: ICMR
By Tehelka Web Desk - November 3, 201813

Indian Council of Medical Research (ICMR) under the Department of Health Research has sequenced 5 Zika virus strains collected at different time points of the Jaipur outbreak. Advanced molecular studies of Zika virus strains suggest that the known mutations linked to fetal microcephaly and high transmissibility of Zika virus in Aedes mosquitoes are not present in the current Zika virus strain that has affected Rajasthan.

However, the Government is maintaining a high vigil of the possibility of adverse pregnancy outcomes in women exposed to Zika virus as the strain may mutate in future or some other unknown factors may play a role in microcephaly or other birth defects.

The Health Ministry is reviewing the situation on a daily basis. Around 2000 samples were tested for Zika virus, of which 159 positive cases have been confirmed.

The three cases were confirmed in Bhopal, Vidisha and Sehore districts in Madhya Pradesh, within two months of an 85-year-old woman testing positive on September 21 in Jaipur, where the virus infected 160 people. A joint team of experts from the Union health ministry’s emergency medical response wing and Delhi-based National Centre for Disease Control is investigating if the disease spread to Madhya Pradesh from Rajasthan or whether there are other sources of infection.
