    The first case came in January,

    Diarrhea
    Bihar has been battling acute encephalitis syndrome, namely chronic fever, that now a new problem in the form of diarrhea has stood in front of people. More than 85 people have become sick due to Diarrhea in Jodhpura village of Rajgir block in Nalanda district. Children, women and men are all included in the grip of diarrhea. People suffering from diarrhea have been admitted to the local hospital for treatment.

      Doctors and health officials have different views on the factors behind AES epidemic and the cause of the deaths. This confusion has further complicated the process to combat or control the seasonal outbreak of AES, which has been recurring every year.

      Bihar Chief Secretary Deepak Kumar has publicly admitted that even the government is unclear about what exactly was causing the AES outbreak, which has been recorded regularly in Muzaffarpur since 1995.

      "We are still not aware if the disease is caused due to some virus, bacteria, toxin effect due to consumption of litchi, malnourishment or due to environmental conditions such as high temperature and humidity," he said.

      "Several researches have been conducted, including by a team of experts from the Centers for Disease Control and Prevention, Atlanta (US), but the finding is inconclusive".

      SKMCH Chief Medical Officer S.P. Singh said the reason behind the outbreak of the disease was yet to be confirmed. "We don't know it and it was being ascertained medically", Singh said.

      Union Health Minister Harsh Vardhan, himself a doctor, who had visited Muzaffarpur three days ago and examined several children in SKMCH, said the deaths could have something to do with electrolyte imbalance and the metabolic system.

      He also did not rule out the possibility of AES due to viral infection or toxin effect, which could possibly be caused by the consumption of litchi, as well as high temperature and humidity.

          Patna / Muzaffarpur [JnN]. In Bihar, the death of AES (acute encephalitis syndrome) or encephalopathy is not visible. AES in Muzaffarpur broke the record of death of 120 children in 2012 Five more children died in SKMCH Wednesday morning. So far, 125 children have died in the hospitals of Muzaffarpur. In Bihar, the figure has so far reached 145 this year. So far 473 children have been admitted in various hospitals. 336 were admitted in the year 2012.

          Here, the time for the victims' arrival was not stopped. More than 22 new patients were admitted in SKMCH on Wednesday, while eight children were brought to the hospital. Earlier on Tuesday four children were killed, while 39 victims were admitted. Due to this havoc of the disease, litigation has been initiated on Central and State Governments. Public interest litigation was filed in the Supreme Court on Wednesday to increase facilities in hospitals.


          SKMCH Superintendent of Muzaffarpur Sunil Kumar Shahi said that now 68 children are recruited in ICU and 65 children are in the ward and today the figure of children's death is less than the previous days. Today only five children have died. Families of children have become aware, due to which the health of the children is recovering quickly now.

              MUZAFFARPUR: Untrained doctors who don?t have the skill to handle critical equipment in intensive care units, and the lack of an awareness drive because of the Lok Sabha elections in April-May, could be behind the sudden spike in the deaths of children due to suspected Acute Encephalitis Syndrome (AES) in this district.

              A seven-member team of experts from the All India Institute of Medical Sciences (AIIMS) at Delhi and Patna, which visited the Shri Krishna Medical College Hospital (SKMCH) here to study the high rate of deaths, blamed the hospital?s doctors in its report for not being able to operate equipment such as CT scan machines, ventilators and blood gas analysers.

                In view of the situation arising due to the AES/JE cases in Muzaffarpur and other adjoining districts in Bihar, Dr. Harsh Vardhan, Union Minister of Health and Family Welfare has instructed for deployment of eight advanced life support ambulances (ALS) in the affected districts. Accordingly, eight additional advanced life support ambulances (ALS) have been pressed into service for transportation of critical patients under National Health Mission (NHM). The central teams of 10 pediatricians and five para-medics sent yesterday, have been deployed in the field and have started working in coordination with the state government. Dr. Harsh Vardhan further added that sixteen senior district officials and medical personnel have been sent to the vulnerable blocks for monitoring and early identification of cases and ensuring daily reporting. Their headquarters have also been shifted to these blocks also, he added Dr. Harsh Vardhan said that the District Collector (DC) has now got involved and is personally taking responsibility. He has deputed the Senior Deputy Collectors (SDC)/District level officers to lead various block teams. These teams will be assisted by a district level Medical Officer and Medical Officer in charge (MOIC) to do the overall work at block level. He further said that the State has been given instructions to switch to daily surveillance and monitoring for detecting early warning signs of the disease. ?The State government has started a social audit along with active case finding campaign through house to house survey and moving the prospective patients to the nearest PHC for early intervention. Surprise checks are also being conducted at the PHCs by the Senior Deputy Collectors (SDC). Additionally, the early morning inspection of Anganwadi Centres is also being undertaken by SDCs and Child development Project Officers (CDPOs). They are also distributing of ORS to each house in the affected villages,? he stated. The ANMs and ASHAs have been directed to visit every house for raising awareness about AES/JE by distributing pamphlets and brochures. Awareness is also being created through other measures such as miking and inter-personal communication. Also, an ICMR team of experts has been deployed at the Sri Krishna Medical College and Hospital (SKMCH) hospital for making the virology lab operational at the earliest, Dr. Harsh Vardhan stated. The Union Health Minister further stated that the multi-disciplinary team deployed earlier is reviewing all the case records of AES patients, admitted and treated in 2019. ?The team is reviewing the case records of discharged and deceased AES patients at SKMCH using a standardized tool to know the reasons for mortality. We expect to complete the activity in next two to three days. A similar exercise will be undertaken in the Kejriwal Hospital,? Dr Harsh Vardhan said. The multi-disciplinary team will systematically collect clinical, nutritional and epidemiological information from AES patients hospitalized since 18 May 2019. Blood, urine and CSF samples will be collected from these patients to rule out infectious aetiology. The team will also conduct case-control study (by taking household/village controls) to reconfirm the role of missing meals and litchi consumption (including the quantity). Orientation training of all pediatricians of SKMCH will also be conducted in batches over the next few days on clinical case management and treatment protocol of AES and brief the new team about case assessment, investigations and treatment protocols already in place and request them to continue the same. Shri Manoj Jhalani, AS & MD (NHM) will visit Muzaffarpur tomorrow and review the overall ongoing operations.

                        Startling reports about the public healthcare system in Bihar has come to light after it was revealed that a primary health centre in Laxmipur village, Motihari district which is being used as a cow shelter.

                        Patna: Startling medical apathy is coming to light from the state of Bihar where as many as 172 children have reportedly died due to encephalitis, a majority of them at the Kejriwal Hospital and Sri Krishna Medical College and Hospital in Muzaffarpur district of the state. The state's public healthcare system has come under severe scrutiny. In damage control mode, to rein in the medical crisis and partly also to save themselves from the backlash from media and people, the central and state government have joined forces to tackle the public health issue.

                        As a result, sections of the media came across a primary health centre in Laxmipur village of Motihari district in Bihar which was being used as a cow shelter. Upon stepping inside the shelter, one can see cows and bulls tied up at each corner and the primary health centre filled to the brim with the odour of cow dung and stale fodder. Even more surprising is that top officials of the state government are urging the people of Bihar to visit their nearest primary health centres in order to fend for themselves.

                        Khushi?s journey to the hospital
                        There was no chart for Khushi, who had died within two hours of being admitted at the overcrowded SKMCH on Friday.

                        From the time that they had first seen her convulsing at five in the morning, it had taken her desperately poor parents five hours to get her to the hospital from Kankatti, the village where they were living temporarily while working on a construction job.

                        Her father Amarnath Manjhi, a labourer, rushed her to someone he described as a ?private doctor?, who gave her an injection. This, however, did not help her.

                        For the next hour, the 25-year-old went scrounging for the Rs 100 that he would have to pay for transporting her in a shared ?tempo? over 60 kilometres. He eventually borrowed it from his maalik, the contractor who hired him for the construction job.

                        To get a tempo, Amarnath had to cycle four kilometres to the main road, with his wife sitting behind him, holding their convulsing daughter in her arms.

                        After the first tempo dropped them off at a place called the ?zero mile? point, six kilometres from SKMCH, it took another shared tempo, five rupees and 20 minutes before they reached the hospital.

                        Why didn?t Manjhi dial 102 to call an ambulance to Kankatti village?

                          NEW DELHI, June 21 (Xinhua) -- At least 161 children have died due to encephalitis in India's eastern state of Bihar, the state-run broadcaster All India Radio (AIR) said Friday.

                          "In Bihar, the death toll due to Acute Encephalitis Syndrome (AES) in Muzaffarpur district and its adjoining areas has touched 161," the broadcaster said.

                          The Indian authorities are rushing special teams of doctors to the affected areas in Bihar to contain the disease and unravel its cause.

                          "A team of National Cooperative Development Corporation (NCDC) Pune and All India Institute of Medical Sciences (AIIMS) will go to concerned villages to know the reason behind deaths due to AES," an official said.

                          Brain fever in Bihar has now claimed as many as 142 lives across the state. The new toll was reported after the number of deaths spiked to 104 in Muzaffarpur's Sri Krishna Medical College and Hospital.

                          Brain fever has afflicted more than 600 children across 16 districts of Bihar of which 142 have lost their lives since the beginning of this month.

                          According to statistics released by the department, the total number of Acute Encephalitis Syndrome (AES) cases registered across the state since June 1 was 626.

                          Muzaffarpur district has bore the maximum brunt, accounting for 117 deaths so far. Besides, deaths have been reported from districts like Bhagalpur, East Champaran, Vaishali, Sitamarhi and Samastipur.

                          The department also said that among the various complications that are covered under the umbrella term AES, hypoglycemia or very low blood sugar level was the most prevalent as it has afflicted 520 children and accounted for 110 deaths.

                              The Acute Encephalitis Syndrome (AES) has claimed the lives of 158 children in Bihar, plunging the state in a grim crisis. The deadly outbreak of the disease has gripped the children of the state since the beginning of this month. The worst-hit district is Muzaffarpur with the viral disease taking the lives of 122 children, followed by Vaishali where 17 children have lost the battle with AES. Another six children have lost their lives in Begusarai district, five in Samastipur, two each in Motihari, Patna and Bettiah and one each in Bhagalpur and Gopalganj.

                                According to the official data of Additional Directorate Health and Medical Gorakhpur Division, between May and June 19 in Gorakhpur, 130 patients of JE, AES have been found.
