Is BRD College Hiding Encephalitis Cases by Creating New Illness Category?

While the number of AES/JE patients has gone down, acute febrile illness patients have seen an alarming rise.
      Tarique Anwar 17 Nov 2019

New Delhi: Is the Uttar Pradesh government’s claim that the number of patients suffering from Acute Encephalitis Syndrome (AES) and Japanese Encephalitis (JE) at Baba Raghav Das (BRD) Medical College in Gorakhpur has witnessed a sharp decline in the past two years a reality or an eyewash?

Chief Minister Adityanath in August this year claimed to have reduced the number of AES/JE patients by 35% and deaths caused by the disease by 65%. In Gorakhpur division, officials claimed that AES/JE cases and deaths had decreased by more than 60% in the past two years.

However, doctors in the college allege that the BRD administration is manipulating data, resulting in unrecorded acute encephalitis AES/JE patients. Even the unofficial data accessed by this reporter suggests so. A total of 331 patients — as per the data — have been found JE positive from January to November this year.
      ...
Most of the children admitted under AFI category exhibit symptoms similar to encephalitis, yet are denied the AES number (which is assigned to AES patients to maintain a record), which shows a decrease in the number of encephalitis cases.

There is, therefore, a growing fear that the actual number of encephalitis patients may be hidden by denying them an AES number and registering them under AFI category.

More than 13% of JE cases have been found in AFI patients at BRD Medical College. This number is more than the number of patients found in AES. The finding of JE cases in AFI patients suggests that the actual number of encephalitis patients is being hidden.
      ...
Documents accessed from BRD Medical College show that from January this year till date, of 492 cases of AES, 116 were found JE positive. Apart from this, a total of 1,563 AFI cases have been found in the same period in which 215 patients have been found JE positive.
      ...
BRD Medical College Principal, Ganesh Kumar, has rejected any such data and reiterated the chief minister’s claim. “Around 87 patients of AES/JE had been admitted to the hospital in 2019, out of which 19 children died. In 2017, this number was recorded at 2,248, of which 512 had died while in 2018 the total number of footfall of AES/JE patients was at 1,047, with 166 deaths. In 2005, the worst in recent years, the disease had claimed the lives of more than 1,500 children.”.
      ...
But sources in the hospital termed this as an eyewash. A doctor in the paediatric department said: “JE is generally accomplished by testing of CSF to detect virus-specific IgM antibodies. Categorising under the AFI category, instead of AES, the patients are denied proper diagnostic examination and medicine protocol. Several patients here have died here because of the JE, but records suggest they were suffering from AFI,” he said on condition of anonymity.
      ...

Comment: Once again allegations that the official encephalitis numbers from Uttar Pradesh are being manipulated for political reasons.

      • Did BRD Medical College hide the actual number of encephalitis patients?
November 9, 2019 by Manoj Kumar Singh 1 min read

Gorakhpur The reduction in the number of AES / JE (Acute Encephalitis Syndrome / Japanese Encephalitis) patients at BRD Medical College Gorakhpur has come from efforts by the government or due to data alteration. This question has been arising prominently for the last two years. Recently, after receiving data related to AFI (Acute Fibrillary Illness) patients at BRD Medical College, there is a growing fear that the actual number of encephalitis patients is being hidden.

More than 13 percent of Japanese encephalitis cases have been found in AFI patients at BRD Medical College. This number is more than the number of patients found in AES.

The finding of JE cases in AFI patients is evidence that the actual number of encephalitis patients is being hidden and on this basis claims are being made for the end of encephalitis.

A few months ago, the Yogi government claimed to have reduced the number of AES / JE patients by 35 percent and deaths by 65 percent. In Gorakhpur division, the officials claimed that there was a reduction of more than 60 percent in AES / JE cases and deaths in two years.

These claims were always viewed with suspicion but no firm evidence was available. It was also learned that since the year 2018, patients are being admitted under AES / JE as well as another disease AFI (Acute Fibrail Illness). Most of the children are in it and their symptoms are similar to encephalitis, yet they are being admitted to AFI by not giving the AES number. This shows a decrease in the number of encephalitis cases.

It is also necessary to mention here that the name of acute febrile illness was not heard in BRD Medical College before 2018. Physicians or responsible people here never used this medical term.

Ever since BRD Medical College started categorizing patients with AES as well as AFI, dramatic cases of encephalitis have started to appear.

In AES / JE, the patient exhibits fever as well as signs of fainting, tremors, dementia, whereas in AFI the patient has a fever of more than 37.5 ? C. The fever may last for 2–14 days. Along with this, the patient may complain of headache, dizziness, chills, muscle and joint pain and weakness. Malaria, scrub typhus, dengue, chikungunya are the main factors in AFI. AFI patients do not have symptoms of tremors, fainting, or insanity that distinguish them from AES / JE.

Documents found from BRD Medical College show that from January this year till date, there were 492 case reports of AES in which JE positives were 116. Apart from this, there were 1563 case reports of AFI in the same period in which 215 positive JE have been found.

Regional Medical Research Center (RMRC) located in the BRD Medical College campus tested the samples of 29 AFI patients on seven and nine months of October and found five JE positives. Among these, 11 scrub typhus patients were also found. Similarly, in August 2019, a sample of 114 AFI patients tested 21 JE positive and 56 scrub typhus. Of these 143 patients, 92 i.e. 64 percent are children below 15 years of age.

According to information received from reliable sources of BRD Medical College, 215 (13.7%) patients of JE have been found in 1563 cases of AFI in 11 months this year. In addition, more than 350 (22.37%) cases of scrub typhus, 50 (3.14%) of dengue and 100 (6.39%) of chikungunya have been found.

Research conducted on 1564 AFI patients in five states - Asom, Bihar, Chhattisgarh, Maharashtra, Andhra Pradesh, Tamil Nadu in 2011-12 - showed that 17 percent malaria, 16 percent dengue, 10 percent scrub typhus and 6 percent chikungunya Belonged to

The research conducted so far found very few JE positives in AFI patients tested, but finding more than 13% JE positive in AFI patients in BRD Medical College is raising serious questions.

In more than half of encephalitis patients, the cause is 'unknown', doctors sweat to solve the problem
Publish Date: Mon, 18 Nov 2019 07:38 AM (IST)

Lucknow, JNN. There was undoubtedly a havoc in the state of encephalitis. The system of treatment has also improved. Pediatric Intensive Care Units (PICUs) have been made up to the CHC level, but accurate screening of the disease is pushing the situation back. The situation is that more than half of the patients have not been found to be the cause of encephalitis. Therefore, the column is being completed by mentioning 'unknown' in the report.

In fact, the central government first declared 20 districts of UP affected by encephalitis. At the same time, in the communicable diseases department of the state government, 38 districts in the year 2017 and eight more districts in the year 2018 were covered under encephalitis. In such a situation, treatment arrangements were improved in these districts. PICUs were opened at CHC to provide locally to acute patients with acute encephalitis syndrome (AES) and Japanese encephalitis (JE). Pediatricians here were trained at KGMU, PGI and Dufferin Hospital. Its meaningful results were also achieved, but in many patients in the lab, doctors are still unable to find the true cause of the disease.

How will the disease be eradicated: Encephalitis was investigated in other microbiology labs of the state including KGMU. Samples of 1,795 AES patients have been received since January. Among them, 699 confirmed the causes of encephalitis. In the remaining patients, the doctors have written the reason unknown.

          Most scrubtifus

A total of 1795 AES patients were reported by Saturday. Scrubtiferous bacteria were found in 285 of these patients. Japanese encephalitis was detected in 199, chikungunya in 39, dengue in 65, typhoid bacteria in 17. Apart from this, herpes was found in three patients, leptospira in one, measles in three patients and more than one cause of disease in 87 patients. In the remaining patients, the cause of the disease was written unknown.
