The mosquito-borne Japanese encephalitis patients between the extermination claims are on the rise. BRD Medical College campus recently sent to the National Institute of Virology in the investigation of the forty Sempulon JE confirmed. A. So far this year, 72 patients have been exposed to disease. Comment: A report yesterday put the number of cases at BRD at 920 with 224 fatalities, so these figures are possibly out of date. While only 8% of AES cases (and 10% of fatal cases) have tested positive for JE, that does not mean that the others are not JE. BRD has historically been understaffed, underfunded and not all cases are tested, nor are the results always tallied back to the correct patient. In at least one case this year a discharge slip was given to the parents of a child that had already died in the encephalitis ward. In another case, no doctors were to be found during an overnight emergency resulting in the death of a child. A report from earlier this year suggests that half the ventilator units are either not working or there is insufficient staff to operate them. There are 100 beds in the encephalitis ward and 105 encephalitis patients. Since the rest of the hospital is even more overcrowded, many non-encephalitis patients share beds with encephalitis patients. Outbreaks of AES with high fatality occur frequently in Gorakhpur division, India. Except for a small contribution (<10%) from Japanese Encephalitis, the etiology of AES has largely remained unknown. Investigations during the 2015 outbreak showed that a high proportion (62.7%) of patients had IgM antibodies against O. tsutsugamushi, indicating a possible role for scrub typhus in the etiology of AES.

Each year as monsoon-bearing clouds arrive over eastern Uttar Pradesh, so does a pestilence. Hundreds, mostly children, fall ill with fever, convulsions, and disorientation. So familiar are people with these symptoms that at the first signs they rush their children to Baba Raghav Das Medical College Hospital, Gorakhpur. Few other hospitals in the eastern Uttar Pradesh region either have the doctors or the equipment to handle this affliction. And so, the journey of children from nearly 15 districts around Gorakhpur ends at the biggest hospital in these parts.

For the hospital, it is an annual, inevitable, slow descent into chaos.

By September, around 50 patients arrive every day, and the medical college’s infrastructure stretches to breaking point. There aren’t enough beds: unconscious children, sometimes two or three, lie on a single bed. There aren’t enough doctors, so diploma students in child health from around Gorakhpur are drafted in. There aren’t enough mechanical ventilators, even though patients with severe encephalitis often need them.
