    Mystery fever mimicking dengue confounds doctors

    AHMEDABAD: The sudden spurt in cases of a viral fever that is resistant to treatment has flummoxed doctors in the city. Children are its easy prey. Constant high fever, body ache and -in some cases - declining blood platelet count, have made it difficult for doctors to diagnose the causative virus. The common adage, "four days of rest for viral and then back to work", is now a concept of yesteryears.

    Physicians across the city are encountering 30 to 40 such cases a day. Even municipal hospitals, like the VS Hospital in Paldi, are reporting 60 such cases every day...

    We've seen similar stories from around India in the past few years;


    Mimic fever cases reported in Chandigarh
    Kolkata: Unknown fever grips city, virus not identified
    Unidentified dengue-like fever grips parts of Kolkata
    Unidentified fevers in Punjab & Chandigarh

    In one outbreak in Rajasthan, I think the mystery fever turned out to be scrub typhus. In general though, given the overall lack of a comprehensive surveillance system and huge underreporting of dengue & chikungunya in India, there is a very good chance that dengue and/or chikungunya are involved here. Chandigarh has officially reported 2 cases of Dengue and 0 cases of Chikungunya this year.

      ProMED comment on the first article;

      The etiology of these cases has not been determined. Dengue is mentioned in the headline, but it is not clear if it has been ruled out by laboratory tests, nor if have any other of the many pathogens associated with febrile disease. The limited information provided does not permit reasonable speculation concerning which agents might be involved in these cases. An earlier series of cases of undiagnosed febrile disease were reported in Tamil Nadu state this year (2013) and in December 2012. ProMED-mail would appreciate receiving more details, and especially any additional laboratory results for the current outbreak, as they become available.

        Thanks, Ronan. I'm glad ProMED is asking for more information. Maybe it's not the season and encephalopathy isn't mentioned in the Times article, but there was a follow-up to an article tetano posted about atypical H1N1 in India that mimicked dengue:

        Pandemic H1N1 influenza with atypical presentation: Encephalopathy and severe thrombocytopenia
        Our case had atypical presentation of H1N1 influenza with encephalopathy and severe thrombocytopenia. Because of such variable presentations of H1N1 virus, patients with viral fever, with or without flu-like symptoms, should be suspected and screened for H1N1 virus infection, especially in the presence of thrombocytopenia that could mimic dengue fever.

          Indian Institute of Management, Ahmedabad (IIMA) has been stung by dengue again as eight students and two staff members were diagnosed with the disease till Monday.

          A team of Ahmedabad Municipal Corporation (AMC) health officials visited the IIMA campus on Sunday to inspect the breeding of dengue-carrying Aedes Aegypti. Every year, IIMA faces the maximum threat of being a breeding ground for this particular variety Manoj Bhatt, chief administrative officer of IIMA, confirmed that eight students, mostly of first year Post Graduate Diploma Program in Management, and two staff members have been diagnosed with the disease.

            AHMEDABAD: Doctors in the city are flooded with patients freaked out with high fever and excruciatingly painful joints all over the body. While the common refrain is that they have become victims of chikungunya, experts say that this might be a virus from the Arbovirus family which mimics chikungunya symptoms.

            "We are seeing a number of patients who have symptoms similar to chikungunya, mainly high fever, severe joint and body pain but they test negative for chikungunya," says infectious diseases specialist Dr Atul Patel.
