Ongeneeslijke variant van tuberculose in India
We hadden al MDR-TBC (resistente TB) en XDR-TBC (zeer resistente TB), nu hebben we ook TDR-TBC: totaal resistente TB. Niets helpt meer.
De eerste gevallen werden in 2006 in Iran gevonden. Nu worden er uit Mumbai, India 12 patienten gemeld in de laatste 3 maanden van 2011.
TDR-TB: no choice left
As if Multi-drug resistant (MDR-TB), extensively-drug resistant (XDR-TB) were not scary enough, now TB has a deadlier edge known as Totally Drug-Resistant TB (TDR-TB). As the name says it all, the physicians are literally left with no choice to treat, as all known TB combination drugs do not work on the patient, owing to the TB bacilli?s ability to mutate and get resistant to the drugs. The clinicians have little to offer these patients with TDR-TB except for drastic surgeries and some relief medications.
TDR-TB cases were first reported in Iran in 2009 and India stands as the second country to report this scary form of TB and 12 TDR-TB cases have been reported in the last 3 months at Hinduja Hospitals in Mumbai.
Of the 12 cases reported in Mumbai 83.3% of them were from the Mumbai city. Moreover, it is worrisome to note that the average age of the 12 patients is just about 32 years.
The TDR-TB strains from the reported 12 patients were found to be resistant to all the first line drugs namely, streptomycin, isoniazid, rifampicin, ethambutol and pyrazinamide and second line drugs namely, ofloxacin, moxifloxacin, kanamycin, amikacin, capreomycin, para-aminosalicylic acid and ethionamide.
In the report published in the December 2011 issue of Clinical Infectious Diseases journal, the authors declare that the emergence of TDR-TB is the failure of overall health system.
Only 1% of patients with drug resistant TB have access to second-line drugs provided by the government sector and the private sector has clinicians who do not follow appropriate guidelines for management of TB patients.
According to an earlier study on the prescription pattern of clinicians in a particular area, less than 5% of clinicians prescribed the right drugs for drug-resistant patients.

Tuberculose is een infectieziekte die veroorzaakt wordt door de tbc-bacterie (Mycobacterium tuberculosis).
Deze bacterie veroorzaakt in het lichaam ontstekingen. Er worden als het ware knobbeltjes van bacteri?n gevormd in het lichaam (tuberculose betekent eigenlijk knobbeltjesziekte). Het kan vele jaren duren voordat iemand uiteindelijk de ziekte tuberculose ontwikkelt.
De meest voorkomende vorm van tuberculose is longtuberculose. In 2007 was er bij 50 procent van de pati?nten in Nederland sprake van longtuberculose en bij 10 procent van een gecombineerde vorm van longtuberculose ?n tuberculose elders in het lichaam.
Behalve in de longen kan tuberculose namelijk ook voorkomen in andere delen van het lichaam, zoals gewrichten, botten, de hersenen of in de lymfeklieren.
Is tuberculose goed te genezen?
Alle vormen van tuberculose kunnen met medicijnen goed worden behandeld. De behandeling begint zodra de diagnose is gesteld. Soms besluit de arts zelfs om te behandelen als alleen al het vermoeden van tuberculose bestaat. Over het algemeen duurt de behandeling minimaal 6 maanden.
Als er sprake is van ongevoeligheid (resistentie) tegen bepaalde medicijnen wordt de behandeling aanzienlijk gecompliceerder en langer. In Nederland komt dit nog maar weinig voor. Maar in andere delen van de wereld zijn de problemen rondom resistientie veel groter, waardoor de kans op toename van de problematiek in Nederland ook zeker aanwezig is.
Resistentie bij tuberculose: een groeiend probleem
In grote delen van de wereld is de groei van multiresistente tuberculose (MDR-tbc) een serieuze bedreiging voor de volksgezondheid. Jaarlijks ontwikkelen wereldwijd bijna een half miljoen mensen multiresistente tuberculose.
Bij MDR-tbc is de ziekteverwekker ongevoelig voor de twee krachtigste medicijnen tegen tbc. Wie een besmettelijke vorm van de ziekte heeft kan deze resistente tuberculose op anderen overdragen.
MDR-tbc ontstaat vooral in landen waar de gezondheidsdiensten zich niet houden aan de afgesproken internationale richtlijnen voor de aanpak van tuberculose.
Een slecht functionerend tbc-bestrijdingsprogramma is de belangrijkste oorzaak van MDR-tbc, bijvoorbeeld als er sprake is van:
-een onvolledige of onzorgvuldige behandeling van tuberculose;
-het gebruik van slechte kwaliteit medicijnen;
-onderbreking van de medicijnentoevoer;
-ontoereikende laboratoriumcapaciteit
-slecht toezicht op het innemen van de juiste combinatie van medicijnen.
De komst van XDR-tbc
De jongste bedreiging is de opkomst van een ?extensief-resistente? tuberculose, ook wel XDR-tbc genoemd. XDR-tbc is een uiterst gevaarlijke variant van tuberculose.
De XDR-tbc-bacterie is resistent tegen tenminste twee van de meest effectieve medicijnen die bij de behandeling van MDR-tbc worden gebruikt.
XDR-tbc is het gevolg van ondeskundig en onverantwoord medicijngebruik en gebrekkig toezicht daarop.
In alle continenten zijn gevallen van XDR-tbc gerapporteerd.
