      Ukraine: A Rush To Buy Flu Supplies

      While the true scope of the flu outbreak in the Ukraine is anything but clear, the news coverage of the past few days has been sufficient to drive many people to local pharmacies to stock up on masks and other flu supplies.


      Consequently, pharmacies have reportedly raised prices, and some shelves are now empty.

      "I've been to so many pharmacies and can't find medicines anywhere. And flu is on its way," said pensioner Yelena Mikhailova, covering her face with a paper tissue.

      As of this writing, there are reports of as many as 48 deaths attributed to this `pneumonia’, and as many as 150,000 influenza cases.



      Admittedly not a huge number of deaths given the number of cases, but deaths are generally under-counted and are often a `trailing indicator’ in an outbreak. Additionally, there have been media reports of a few cases involving a `hemorrhagic pneumonia’, which has also raised concerns.



      Whether this outbreak constitutes anything out of the ordinary is impossible to tell at this juncture, but it has aroused enough interest that the WHO's Global Outbreak Alert and Response Network (GOARN) team will travel to the Ukraine next week.

        I'm not sure what they mean by conflicting information as to which type of the virus - perhaps that is a reference to whether or not it had mutated? Or perhaps as to whether all the deaths were H1N1 as many were untested?

        MOSCOW — The Ukrainian government is taking some of the sternest measures in the world against the spread of the swine flu virus, ordering schools nationwide to close for three weeks, banning public gatherings and imposing restrictions on travel.

        Prime Minister Yulia V. Tymoshenko announced the measures on Friday in response to rising fears about swine flu, especially in western Ukraine. Federal health officials said 33 people had died from the flu across the country, although there was conflicting information about which type of the virus was to blame.

        The situation in Ukraine “has reached the epidemic threshold,” Ms. Tymoshenko said. The ban on public gatherings, she said, would apply to “all large-scale events, concerts, movie showings and any other gatherings of people for the next three weeks.”

        The World Health Organization said it would send a team to Ukraine to assist the authorities there.

        News reports from western part of the country said there were long lines at pharmacies as people sought medication and masks.

        With all rallies canceled, the anti-flu measures were expected to have an immediate impact on the campaign for Ukraine’s presidency. The election is on Jan. 17, and Ms. Tymoshenko formally registered as a candidate on Saturday. She said she did not expect that the voting would have to be postponed.

        The virus is spreading across Eastern Europe, but it was not clear why Ms. Tymoshenko chose to undertake stronger moves, like closing schools nationwide, than her counterparts in Russia and Poland.

        There were indications, however, that the government’s response was being influenced by electoral politics. Ms. Tymoshenko, one of the leading candidates, and her bitter rival, President Viktor A. Yushchenko, who is far behind in opinion polls in his bid for re-election, both sought to make clear that they were aggressively addressing the outbreak.

        On Friday, Mr. Yushchenko criticized Ms. Tymoshenko, saying that he had ordered an inquiry into why the country was not, in his opinion, prepared.

        “We will have an assessment of the issues that arose — why this has turned out to be so acute,” he said.

          Virological update, week 43/2009

          Of the 15 countries testing 20 or more sentinel specimens this week, influenza positive rates ranged from 0% (Albania, Azerbaijan, Croatia, Georgia) to 81% (United Kingdom-Northern Ireland). The total number of respiratory specimens collected by sentinel physicians in the Region in week 43/2009 was 2595, of which 971 (37%) were positive for influenza virus. This is a much higher proportion than is typical for week 43 (less than 2% during 2004-2008). Of the 971 influenza virus detections, 970 were type A (897 pandemic A(H1), 73 not subtyped) and 1 was type B. In addition, 4004 non-sentinel source specimens were reported positive for influenza virus: 3991 type A (3151 pandemic A(H1), 16 A(H3), 50 seasonal A(H1), 774 not subtyped, and 13 type B. The seasonal influenza A subtype detections were reported by Kazakhstan and the Russian Federation.

              Brief of Minister of Health of Ukraine

              The Minister of Health of Ukraine, Vasyl Knyazevych, has briefed representatives of mass media and the public at the Ministry of Health of Ukraine (MOH) regarding new confirmed cases of influenza А/Н1N1 California.

              The briefing was held based on the results of the travel to the western regions of Ukraine. “Ukraine has joined the influenza pandemic, so called California flu”, - Minister reported.
              He indicated that as of October 29, the L.V.Gromashevskyy Institute of Epidemiology and Infectious Diseases of the Academy of Medical Sciences had analyzed 22 specimens taken from the sick persons in Ternopil. Influenza type A had been confirmed in 11 cases, including 7 cases of А/Н1N1 California.

              Besides, experts of the virology and AIDS laboratory of the Central sanitary-epidemiological station of the MOH of Ukraine examined specimens taken from 8 more persons in Lviv diagnosed with acute respiratory infections (ARI) for А/Н1N1 flu virus. Four out of them were positive for А/Н1N1 California flu (one of the specimens examined was taken from a deceased person).

              Vasyl Knyazevych informed that today a summit of the Cabinet of Ministers of Ukraine and the National Security and Defence Council will be held to discuss problems of response to the influenza pandemic, including introduction of quarantine measures.

              The MOH of Ukraine addressed international organizations for consultancy assistance and requested to grant a sufficient number of А/Н1N1 California flu vaccines.

              Vasyl Knyazevych announced that Ukraine had sufficient stock of antiviral drugs. He added that he had visited hospitals in Ternopil and Chernivtsi regions, and had met physicians, local administrations and medical students.

              The Minister of Health thanked the medical community who currently perform their professional duties conscientiously and are in the epicentre of the epidemic providing competent care for sick persons.

              Vasyl Knyazevych urged against panic, and advised to follow recommendations as to prophylaxis of acute respiratory infections as much as possible and seek medical assistance in time.

                    I believe that many Russian language sources call Pandemic H1N1 "California flu" because the vaccine target from April had "California" in the name. So they call it "California" for the same reason we call the seasonal H3N2 "Brisbane".

                    Or are you implying that this is a different strain of flu?

                      Before the fanfare on the naming of the viruses, an epidemic wave was named according the location of the first virus isolation: in this case the pandemic strain was isolated in California early this year, in april.

                      Perhaps, Eastern Europe is less prone to our compromises.

                      In fact, the A/California/2008 is the name of the reference vaccine strain.

                      No fear, the pandemic is all around the world.

                      No time for blame game.

                        Two points here: Less than 1% of patients with "California" (Pandemic H1N1) flu die, not 1% as indicated in this article. Secondly, the fact that on 14 out of 187,000 patients have tested positive does not mean the others don't have it - Ukraine simply cannot test most cases.

                            As the world enters the [influenza] season, another possible virus
                            emerges as 30 people in the Eastern European country of Ukraine have
                            died from this latest flu [virus infection].

                            Each country is dealing with their own cases of the pandemic (H1N1)
                            2009 virus infection and many have even [under]taken vaccination, but
                            is H1N1 the only flu out there? A viral infection in Ukraine has taken
                            the lives of 30 people and at 1st it seemed like an ordinary flu but
                            after a week the symptoms worsened.

                            Radio Netherlands Worldwide reports 40 000 Ukrainians have contracted
                            the disease and at least 100 are in the hospital. Tests are currently
                            being conducted and all that is known is that it is not the pandemic
                            (H1N1) 2009 strain of influenza virus [but see comment below].

                            A large number of schools and childcare facilities are being shut
                            down, especially in the city of Lvov [Lviv in western Ukraine -
                            Mod.CP]. Also, government agencies are handing out surgical masks and
                            gloves to people in the western part of Ukraine.

                            However, Russia Today is reporting that it is A/H1N1 or Californian
                            Flu [i.e., the A/California/7/2009 strain from which the current
                            pandemic vaccines are derived - Mod.CP] and the total number of deaths
                            is closer to 40.


                            Prime Minister Timoshenko [of Ukraine] stated, "Express-tests cannot
                            provide a hundred percent verification of the virus; they give only 50
                            percent accuracy. That's why blood [samples from] those who died were
                            sent to special laboratories for further testing, and only this
                            morning it was confirmed that at least 11 deaths were caused by the
                            A/H1N1 [pandemic 2009] virus."

                            The Ukrainian government is calling for international aid to help
                            fight their epidemic and President Yushchenko believes the country
                            needs more than USD 6 million to battle the epidemic.

                              Or are you implying that this is a different strain of flu?
                              The reason I say this is because it appears they had quite an increase in the number and proportion of SARI over total number of hospitalizations in the 3-6 age group starting in February.

                                This is the article from which the previous ProMed post is derived. This has a decidedly different flavor than the ProMed post, blaming the outbreak on a new flu virus.

                                I agree with ProMed - these local media reports are very sensationalist and contradictory.

                                40,000 contract serious virus in Ukraine, kills 30

                                As the world enters the H1N1 season, another possible virus emerges as 30 people in the Eastern European country of Ukraine have died from this latest flu.
                                Each country is dealing with their own cases of the H1N1 swine flu and many have even taken the vaccination but is H1N1 the only flu out there? A viral infection in Ukraine has taken the lives of 30 people and at first it seemed like an ordinary flu but after a week the symptoms worsened.
                                Radio Netherlands Worldwide reports 40,000 Ukrainians have contracted the disease and at least 100 are in the hospital. Tests are currently being conducted and all is known is that it is not the H1N1 Swine Flu.
                                A large number of schools and childcare facilities are being shut down, especially in the city of Lvov. Also, government agencies are handing out surgical masks and gloves to people in the western part of Ukraine.
                                However, Russia Today is reporting that it is A/H1N1 or Californian Flu and the total number of deaths is closer to 40.
                                Prime Minister Timoshenko stated, “Express-tests cannot provide a hundred percent verification of the virus, they give only 50&#37; accuracy. That’s why blood probes of those who died were sent to special laboratories for further testing. And only this morning it was confirmed that at least 11 deaths were caused by the A/H1N1 virus.”
                                The Ukrainian government is calling for international aid to help fight their epidemic and President Yushchenko believes the country needs more than $6 million to battle the epidemic(s).
                                Wolne Media have also reported that the symptoms have been diagnosed as pneumonia. Furthermore, four patients in the hospital in Lviv Oblast have been confirmed to have “very severe pneumonia” after contracting the influenza.
