Eight suspected monkeypox cases detected in Spain
Spanish health authorities have warned that eight suspected cases of a virus similar to the eradicated smallpox have been detected in Spain in recent days, a day after the WHO alerted of more cases of this disease in the UK and Portugal.

Spain’s Health Ministry has warned regional health authorities of the appearance of eight suspected monkeypox cases, referred to as viruela de mono in Spanish.

Madrid’s General Directorate of Public Health sounded the alarm initially after identifying these eight possible cases that “are currently under study in coordination with Spain’s National Centre of Microbiology, which have the necessary techniques to confirm or rule out the diagnosis”.

“It’s unlikely that there will be a lot of transmission, but we can’t rule it out,” Spain’s health emergencies director Fernando Simón told journalists on Wednesday...

The Community of Madrid detects 23 possible cases of monkeypox infection in the region

The Ministry of Health of the Community of Madrid has detected 23 possible cases of monkeypox infection in the region, which are currently being studied in coordination with the National Center for Microbiology, which has the necessary technique to confirm or rule out this first diagnosis.

    May 18, 2022

Also known as monkeypox , it is a very rare disease that generally presents with fever, myalgia, lymphadenopathy (swollen glands) and a rash on the hands and face, similar to chickenpox. In general, its transmission occurs through the respiratory route, but due to the characteristics of the 23 suspected cases of infection, it indicates that it has been through contact with mucous membranes during sexual intercourse.

The people under study evolve positively and are isolated in their homes, although close surveillance must be maintained since they may require hospital admission. The General Directorate of Public Health is awaiting the results of the laboratory tests and assessing the role of the smallpox vaccine in controlling transmission, as well as the use of antivirals if necessary. It is also carrying out an epidemiological investigation of those affected and identifying possible contacts.

On May 15, the United Kingdom launched a health alert to the World Health Organization, in accordance with international health regulations, after detecting the first four cases in Europe. Through this alert, the technicians of the General Directorate of Public Health informed the Madrid health system describing the symptoms, with which it was possible to identify the eight possible first affected who were reported yesterday to the Ministry of Health.

The Ministry of Health and the autonomous communities have launched a health alert after detecting several dozen suspected cases of monkeypox . The patients are from Madrid and are pending confirmation through genetic tests carried out by the National Center for Microbiology. As Minister Carolina Darias herself explained yesterday in El Objective, they have already confirmed seven positives.

At this time and according to the latest data provided to laSexta by the Madrid Ministry of Health, 24 suspected cases are added to those seven confirmed positives . The Minister of Health of the Community of Madrid, Enrique Ruiz Escudero, pointed out at a press conference that all the cases have been due to close contact and that they do not rule out that more positives will be confirmed in the coming hours.

According to the General Directorate of Public Health in Madrid, the cases detected are progressing well and none have so far had to be admitted. All of them remain isolated in their homes, closely watched in case their situation changes.

Generally the transmission is by respiratory route, but due to the characteristics, the transmission points to being contact by fluids.
      ...
Antonio Zapatero, Deputy Minister of Health Care and Public Health of the Community of Madrid, called for calm today in an interview on Onda Cero. He has explained that the contagion in most cases has been caused by contact with fluids and "by touching material from the wounds" . "It is unlikely by air, 10%," he pointed out. "The course is benign, in two weeks it is resolved and the evolution is usually favorable", he added.

A suspicious case in Gran Canaria

The Ministry of Health of the Government of the Canary Islands has detected today a possible case of monkeypox infection that is being treated at the Dr. Negrín University Hospital of Gran Canaria, without requiring hospital admission and that is being studied in coordination with the National Microbiology Center that will confirm or rule out the case in question.

This is a young male, who, after the epidemiological survey carried out, meets the notification criteria and case profile that the Ministry of Health has registered as an alert for monkeypox infection, also known as Monkeypox. Given his favorable evolution, he must continue his process at home with the corresponding follow-up.

Fernando Simón: "It is not likely that it will generate an important transmission"
The director of the Center for the Coordination of Health Alerts and Emergencies, Fernando Simón, pointed out this Wednesday that "it is not probable that monkeypox will generate an important transmission, but it cannot be ruled out."

For this reason, he insisted that great care must be taken and, in this sense, he stressed that they are working with all possible hypotheses . According to the expert, monkeypox is a disease that, until now, was confined to Africa and was a disease similar to human smallpox , which was eradicated at the end of the 1970s, and was transmitted from monkeys to humans. and that, in general, among humans, it had very little transmission, very low.

In fact, the outbreaks until a few years ago "had no more than a second generation of transmission , but in recent years up to the third and fourth generation have been detected, which are very small, self-limited outbreaks.

In this case, it is associated with very specific population groups and the cause is being investigated and, above all, what is most interesting, the origin, which should be a case from Africa.

Madrid confirms seven cases of monkeypox and Gran Canaria one suspect

      THE INFORMATION NEWS
      05.19.2022 - 4:27 p.m.

There are already seven confirmed cases by the Community of Madrid of monkeypox and it has another 24 suspects pending the test that confirms this disease, sources from the Ministry of Health have indicated to Efe. The cases detected in the Madrid region - none of them are serious - are mainly associated with two foci, the transmission route is by very close contact, and all those diagnosed and suspected are men, as explained by the Minister of Health, Enrique Ruiz Escudero, in statements to the media.

Convinced that more cases will appear, the counselor has indicated that what is surprising about the appearance of this disease is the wide geographical distribution (cases have been registered in the United Kingdom, Canada, the United States, Portugal and France) when it was " practically eradicated" , after some cases occurred in the 70s and an outbreak in 2003. Ruiz Escudero congratulated the General Directorate of Public Health because since the alert was launched by the Ministry of Health in just 24 hours they have detected all these cases and the epidemiological alert has been made.
      ...
Now, he added, epidemiological surveillance will be prioritized, with surveys of all patients who have been infected, control of isolations and monitoring of the clinical evolution of patients. The symptoms are fever, chills and inguinal lymphadenopathy, but what most complicates the disease are the skin lesions it produces on the face and hands, similar to that caused by chickenpox. Ruiz Escudero has pointed out that the decision to administer the smallpox vaccine to prevent the appearance of the disease, if possible, must be adopted by the Spanish Medicines Agency in coordination with the autonomous communities.

"We will have to start wondering why such an extensive geographical distribution worldwide", he said. "The degree of concern is not like in other situations. Since the transmission route is very close contact, it cannot have the dimensions of Covid because Covid has an even easier transmission than it is by air," he stressed.

The Canarian Ministry of Health has detected this Thursday a possible case of monkeypox infection that is being treated at the Doctor Negrín University Hospital of Gran Canaria, without requiring hospital admission, and that is being studied in coordination with the National Center for Microbiology, which will confirm or rule it out.

This is a young man who, after the epidemiological survey carried out, meets the notification criteria and case profile that the Ministry of Health has registered as an alert for monkeypox infection, also known as Monkeypox. Given his favorable evolution, he must continue his process at home with the corresponding follow-up, the Canarian Executive department specifies in a statement.

It can be transmitted by the respiratory route. However, current suspected cases of infection respond to contact with mucous membranes during sexual intercourse . The case under investigation will be notified this Thursday to the Ministry of Health and the European Center for Disease Prevention and Control (ECDC), as indicated by the protocols.

Enrique Ruiz Escudero, ha explicado que no revisten gravedad y están asociados a dos focos, la vía de transmisión es por contacto muy estrecho, y todos los diagnosticados y las sospechas son hombres.

A massive party in Gran Canaria, the second major focus of the monkeypox outbreak in Spain

The authorities are investigating whether three Italians, several of those diagnosed in Madrid and at least one inhabitant of the islands were infected at the celebration, which took place in Maspalomas between May 5 and 15.

              ORIOL GUELL
              Barcelona-20 MAY 2022 - 23:30 EDT

A massive party in the town of Maspalomas, on the island of Gran Canaria, held between May 5 and 15 and attended by nearly 80,000 people, has become the second focus of the monkeypox outbreak in Spain , have confirmed health sources. The meeting was attended by several of the positive cases diagnosed in recent days in Madrid, two or three young Italian men whose infection has been confirmed in Rome and at least one case investigated on the island itself, according to these sources. The public health services of the Government of the Canary Islands are now investigating the relationship between these cases and whether more infections occurred during the 10 days of celebrations, including that of a suspected case detected in Tenerife.

This is the second major focus of the monkeypox outbreak located in Spain in recent days. The first is a place called Sauna Paraíso, located in the Malasaña neighborhood of Madrid, which was closed on Friday by the Ministry of Health. The regional counselor, Enrique Ruiz Escudero, linked the establishment with "the majority" of the cases registered in the Community.

According to data published on Friday by the Ministry of Health, 30 samples analyzed by the National Microbiology Center corresponding to patients in Madrid have tested positive in the PCR tests that detect the presence of a non-human smallpox virus. This analysis is later completed with a genetic sequencing that must confirm that it is monkeypox, but as there is currently no other Orthopoxvirus circulating , confirmation by PCR safely advances the final results.

A spokesman for the Government of the Canary Islands declined to report on the places where those affected have been, and appealed "to the ongoing epidemiological surveys", although he confirmed that one of the Italians has been in the archipelago and denied knowing that the one from Tenerife had been in Gran Canaria, informs Guillermo Vega.

In addition to the confirmed cases, until eight o'clock on Friday afternoon it had transpired that Madrid has another 15 under investigation with symptoms compatible with the disease, the Canary Islands, two, and other communities such as Andalusia, Galicia, Catalonia, the Basque Country and Extremadura, one. each. In this last community it is a woman, according to the information offered by the Board.

Maspalomas Gay Pride is one of the highlights in the gay community calendar, and thousands of people from all over Spain and many European countries attend it every year —except during the forced break caused by the coronavirus pandemic. According to health sources, "among the thirty or so diagnosed in Madrid, there are several who attended the event, although it is not yet possible to know if one of them is patient zero of this outbreak or if they all got infected there." Investigations have revealed that the virus began to circulate in the capital during the second half of April , although most of the infections known so far occurred around the weekend of May 7 and 8.

The Maspalomas festival is also behind two of the first three confirmed monkeypox diagnoses in Italy. The first of them corresponds to a young man who initially went to a health center in Gran Canaria when he began to develop the symptoms of the disease —high fever, intense pain, a striking skin rash...—, but who finally He returned to Rome by plane.

All three are admitted to the Spallanzani Institute of Infectious Diseases in Rome. The director of the center, Francesco Vaia, explained at a press conference on Friday that "they are hospitalized in Spallanzani with confirmed infection with the monkeypox virus." "These are three young men who say they have not had contact with each other, although two of them do explain that they have recently traveled to the Canary Islands," he added.

Vaia explained that the three are in "quite good condition", that only one of them has had a fever, although they all suffer from "an enlargement of the lymph nodes, which are painful, and have developed some skin pustules". Andrea Antinori, director of the Viral Immunodeficiency Unit at the Spallanzani Institute, recalled that monkeypox "is not a sexually transmitted disease because contagion occurs through close contact, such as those that occur during sexual intercourse, but not only in them”. The most common route of infection is through thick drops of saliva or by contact with fluids such as those of the pustules caused by the disease.

The evolution of monkeypox is favorable in almost all cases suffered by healthy people. The incubation of the virus usually lasts between 6 and 13 days, although it can last up to 21. The symptoms begin to manifest with fever, headache and muscle pain, fatigue and swollen glands. One to three days later, the skin rash appears with small blisters that grow, fill with pus, and then scab over. When these dry up and fall off, which usually takes one to three weeks, the person is no longer contagious.

In the Canary Islands, health services have detected two suspected cases, both young men. One of them lives in Gran Canaria and has links with the Maspalomas party, while the public health services are now investigating the one in Tenerife.

Las autoridades investigan si tres italianos, varios de los diagnosticados en Madrid y al menos un habitante de las islas se infectaron en la celebración, que tuvo lugar en Maspalomas entre el 5 y el 15 de mayo

Some confusing counts from Madrid itself. Might be an issue of the city itself versus the surrounding province. We had a report above a couple days ago with 39 confirmed and 40 suspect cases in Madrid already. Have some of the suspects been excluded?

I give Spain credit for announcing the number of suspect cases under investigation as well; England and Portugal are not doing that.

Health confirms a case of monkeypox in Gran Canaria and reports five new ones under study
The result of the samples from the second reported case is still pending. Today, the samples of five people will be sent to the National Center for Microbiology, of which three have been classified as probable because they meet clinical and epidemiological criteria and two as suspected because they meet only clinical criteria. All of them with a favorable evolution

The Ministry of Health of the Government of the Canary Islands, after receiving today the result of the study carried out on the first case notified to the Center for the Coordination of Alerts and Emergencies of the Ministry of Health, reports the first confirmed case of monkeypox in the Canary Islands. The second case reported last Friday is still waiting to know the result of the sample that is being analyzed in coordination with the National Center for Microbiology.

The first two cases notified last week continue with a favorable evolution, carrying out the corresponding isolation at home.

The protocol of the Ministry of Health classifies cases between probable and suspected depending on whether the patient presents only compatible symptoms (clinical criteria) or if he also meets epidemiological and laboratory criteria.

Probable cases: clinical and epidemiological criteria

In addition, the Canary Islands will notify three other probable cases today, all in Gran Canaria that correspond to three young men whose evolution is also favorable, following their process at home, except for one of them who has been admitted for another pathology.
According to the protocol, one that meets clinical and epidemiological criteria has been established as a probable case, but is awaiting the results of the analysis of the laboratory samples.

                    Suspected cases: clinical criteria

On the other hand, the General Directorate of Public Health of the SCS has also notified two cases as suspicious for presenting only compatible clinical criteria, but not epidemiological or laboratory criteria, as stated in the protocol established by the Ministry of Health.
All these cases, both confirmed and suspected and probable, have been notified to the Ministry of Health today, Monday, as indicated by the protocols.

                    monkey pox

Monkeypox infection is a rare disease until now that causes fever, headache, swollen glands and rashes on the hands and face, similar to that caused by Chickenpox.

The virus has a low capacity for human-to-human transmission and requires close, intimate contact. The incubation period ranges from 5 to 13 days, although it can sometimes be as long as 21.

On May 15, the United Kingdom launched a health alert to the WHO, in accordance with international health regulations, after detecting the first four cases in Europe. This alert has activated the protocol in all health centers of the Canary Health Service with the aim of early detection of possible cases that could be detected.

Health reports that one of the suspected cases of monkeypox detected today is ruled out
After the confirmation of the first case declared on Thursday, the result of the samples of the second that had been notified last week is still awaited. Today, the samples of four people will be sent to the National Center for Microbiology, of which three have been classified as probable, for meeting clinical and epidemiological criteria, and one as suspicious for meeting only clinical criteria. All of them with a favorable evolution

The Ministry of Health of the Government of the Canary Islands informs of the update of registered data regarding the cases reported today related to monkeypox diagnosis. Of the two suspected cases, pending notification this afternoon to the Coordinating Center for Alerts and Emergencies of the Ministry of Health, one of them has been ruled out after analyzing the Epidemiological Survey and its clinical criteria by the General Directorate of Public Health . Therefore, the cases reported in the Canary Islands, all males and with favorable evolution, are as follows:

                    – A case from Gran Canaria confirmed this morning that corresponds to the first reported last Thursday.
                    – A case from Tenerife pending confirmation after the sample had been sent to the National Center for Microbiology last Friday.
                    – Three probable cases, all in Gran Canaria, which will be reported today corresponding to three young men whose evolution is also favorable, following their process at home, except for one of them who has been admitted for another pathology.
                    – A suspicious case, corresponding to a British tourist, registered in Fuerteventura.

The protocol of the Ministry of Health classifies cases between probable and suspected depending on whether the patient meets clinical criteria and epidemiological criteria (probable cases) or if it only presents clinical criteria and in such a case it is considered a suspected case.

Health raises to 51 cases of monkeypox in Spain after confirming 15 new positives | | Society THE COUNTRY (elpais.com)

(I suspect the difference between this and the above post refers to the autonomous regions. Notably, 35 of the suspected cases have been discarded after testing negative for monkeypox and positive for something else, such as herpes, chickenpox, and other diseases, suggesting they are casting a very wide net to ensure they are not missing cases. - alert)

The article also mentions a case in Valencia, imported from Burundi, unrelated to the larger outbreak. I wonder if, given that Burundi is not in West Africa, if that case will turn out to be the other clade.
