Alert on monkeypox infection in Spain and other non-endemic countries

Current situation in Spain.
In Spain, according to data from the National Epidemiological Surveillance Network (RENAVE), as of November 22,
a total of 7,404 confirmed cases of MPX have been reported.
The reported cases come from 17 Autonomous Communities, see Figure 1.
Of the cases reported in SiViES, a total of 7,242 are men and 162 are women. The age ranges from 7
months and 88 years, with a median age of 37 years (interquartile range: 31-44 years).

The date of onset of symptoms is available in 7,103 cases. Of these, the first reported case started symptoms
on April 26 and the last on November 13 (Figure 2). There is a delay between the date of onset of symptoms and
the date of diagnostic confirmation, with a median of 7 days (IQR 4-10 days). On the other hand, there is also
a delay in reporting cases to the SiViEs platform. For these reasons, the interpretation of the last
days of the epidemic curve should be done prudently.

A total of 508 cases of the 5,830 with information (8.7%) presented complications throughout the process
clinical. The most frequent were mouth ulcers and secondary bacterial infections.
Of the 6,640 cases with available information, 244 cases were hospitalized (3.7%). Two cases for
meningoencephalitis, a case with disseminated infection by monkeypox in the context of immunosuppression
severe and fourth case with concomitant monkeypox infection who died of other causes.
5,628 of 5,879 cases with available information were men who have sex with men (95.7%). Nope
information is available on 1,525 cases.
Regarding the most probable transmission mechanism, of the 4,994 cases with available information, in the
93.0% was due to close contact in the context of a sexual relationship and 7.0% due to close contact not
sexual. Two cases are associated with occupational exposure in the health field. In the 2,410 cases
remaining, the information is pending, not specified or not available.
In relation to attendance at mass events, of the 5,855 cases with information on this variable, 1,005
attended an event on the dates prior to the onset of symptoms.
