UNICEF-WHO Philippines: Polio Outbreak Situation Report #12 (20 December 2019)
Report from World Health Organization, UN Children's Fund
    Published on 20 Dec 2019

    Highlights

Two healthy contacts were confirmed with cVDPV2 in the past week - Currently 9 human cases confirmed with cVDPV2; one case with cVDPV1; one case with VDPV1; and one case with immunodeficiencyrelated VDPV type 2 (iVDPV2).

One case of cVDPV1 from Malaysia was confirmed as genetically linked to the Basilan case.

Synchronized polio vaccination campaign conducted on 25 November-13 December (including 7 days of extension) vaccinated 1,404,517 (110%) children under 5 with bivalent Oral Polio Vaccine (bOPV) against poliovirus type 1 in National Capital Region (NCR), and 2,937,327 (95%) with monovalent OPV against poliovirus type 2 (mOPV2) in Mindanao.

Department of Health (DOH) planning to conduct outbreak immunization with bOPV targeting 710,296 children under 10 in the Sulu Archipelago, Zamboanga City and Lambayong, Sultan Kudarat on 6-12 January 2020.

Additional 4 rounds of vaccination planned for February-March 2020 in NCR with mOPV2 and in Mindanao with bOPV.

Current polio outbreak resulting from persistently low routine immunization coverage, and poor sanitation and hygiene.

Philippines is affected by both cVDPV1 and cVDPV2. cVDPV is considered a public health emergency of international concern (PHEIC)

Circulating Vaccine Derived Polio Virus type 1 (cVDPV1)

A cVDPV1 case in Sabah State, Malaysia was confirmed to be genetically linked to the Basilan case by the Victorian Infectious Diseases Reference Laboratory (VIDRL) in Australia. Since the two viruses are genetically linked they are both classified as circulating.

All 13 cVDPV1 environmental samples found in Manila are all genetically linked.

DOH agreed to conduct 2 additional rounds of immunization against poliovirus type 1 in Mindanao in February-March 2020, using bOPV.

Circulating Vaccine Derived Polio Virus type 2 (cVDPV2)

All 9 human cases and 17 environmental samples confirmed with cVDPV2 are genetically linked. All human cases were reported from Mindanao (BARMM and Region XII), whereas environmental samples were found in NCR and Davao...

Polio cases in the Philippines now 14, say WHO, UNICEF
        January 15, 2020 Admin5 News

Manila – Six more cases of polio have been detected in the Philippines, bringing to 14 the total number of patients with the highly contagious disease, the World Health Organisation (WHO) and UN Children’s Fund (UNICEF) said on Wednesday.

The patients, children under five, were from provinces in the southern region of Mindanao, according to a situation report by the two international bodies.

“The risk for further polio transmission continues to be assessed as high at the national level, because of chronically sub-optimal immunisation coverage and poor sanitation and hygiene conditions,’’ the report said.

The WHO and UNICEF said they had dispatched nearly 40 consultants to work with Philippines health officials and support an immunisation campaign in Mindanao and metro Manila...

Polio cases rise to 16 since outbreak declared in September
          Gaea Katreena Cabico (Philstar.com) - January 16, 2020 - 4:20pm

MANILA, Philippines — The number of people who contracted polio has risen to 16 nearly four months since an outbreak was declared in the country last September.

The Department of Health said Thursday there were four more polio cases in the country: a two-year-old boy and a three-year-old boy from Maguindanao, a two-year-old boy from Sultan Kudarat, and a three-year-old boy from Quezon City.

All of them experienced fever, diarrhea, muscle pain, asymmetric ascending paralysis and weakness of extremities, the DOH said...

QC polio case 'immunocompromised', says DOH
            January 17, 2020

THE Department of Health (DOH) on Friday, January 17, disclosed that the three-year-old boy afflicted with the poliovirus in Quezon City has been found to have an impaired immune system.

In an interview, DOH Undersecretary Eric Domingo said they have received a report that the first patient from Metro Manila is immunocompromised, which makes him vulnerable to diseases despite getting a complete dose of oral polio vaccines (OPV).

"The child is immunocompromised. He has several illnesses. Probably, this is why he was easily afflicted with the virus. He was not able to mount the adequate immune response during the vaccination," said Domingo.

"The child is not healthy completely. If one is immunocompromised and the resistance is low, even if you get a vaccine, your body has no capability to mount an immune response versus the disease," he added.

This, despite the child having received complete OPV doses since 2016...

DOH CONFIRMS NEW POLIO CASES IN THE COUNTRY

              Press Release/16 January 2020

The Department of Health (DOH) today confirmed four more polio cases in the country, bringing the total number of polio cases to 16 since the outbreak was declared in September 2019.

The Research Institute of Tropical Medicine (RITM) reported two additional polio cases from Maguindanao, both males, ages two- and three-years old; one polio case from Sultan Kudarat, a two-year old male; and one polio case from Quezon City, a three-year old male. They manifested with fever, diarrhea, muscle pain, asymmetric ascending paralysis and weakness of extremities.

DOH extended its Sabayang Patak Kontra Polio (SPKP) campaign to ensure that no child is left unvaccinated and to boost protection for those already vaccinated. The succeeding rounds are scheduled on January 20 – February 2 in all regions of Mindanao, and on January 27 – February 7 in the National Capital Region.

“I urge all parents and caregivers of children under five years old to take part in the coming SPKP campaign rounds scheduled in your respective areas. Have your children, including those with private physicians or pediatricians, vaccinated with oral polio vaccine by health workers and bakunators. Additional polio doses can provide additional protection to your children. There is no overdose with the oral polio vaccine,” the Health Secretary Francisco T. Duque III said.

DOH and the Local Government Units aim to accomplish 95% coverage in all identified areas for every SPKP round.

DOH also urges all health facilities to strengthen their Acute Flaccid Paralysis (AFP) surveillance. All health facilities must promptly report every case of AFP in any child under 15 years of age and gather sufficient stool specimen samples.

“The outbreak must be put to a halt, and we can only do this if all our health facilities are achieving the targets for all AFP surveillance indicators, and if every SPKP round, ALL of the target population are reached and vaccinated. The DOH, its partners, and the LGUs will continue to work hand-in-hand to ensure that no child is left behind in our fight against polio,” the Health Secretary concluded.

"Safety and security don't just happen, they are the result of collective consensus and public investment. We owe our children, the most vulnerable citizens in our society, a life free of violence and fear."
