Discovery of two cases of hemorrhagic fever in Mauritania

THE MORNING February 14, 2022 at 11:31 am

The Mauritanian Ministry of Health announced on Sunday that it had registered two cases of Crimean-Congo hemorrhagic fever at the beginning of February.

In a circular, the ministry calls for vigilance and the strengthening of measures throughout the country, warning against negligence in the face of contagious diseases.

The Ministry of Health has instructed Regional Health Directors and Chief Medical Officers inside the country to increase epidemiological monitoring measures in addition to taking other measures to prevent the infiltration of any cases of infectious diseases. dangerous.

Crimean Congo fever is one of the most dangerous haemorrhagic fevers among known fevers and according to the World Health Organization, this disease is transmitted to humans by ticks and by cattle, and from person to person. another by contact with his blood.

According to the WHO, although it has appeared for a long time, there is no vaccine against this disease either for people or for animals. Its treatment is limited to dealing with the symptoms that appear on the patient in addition to a permanent follow-up of this one.

From time to time, cases of this disease appear in Mauritania and the last case recorded was at the end of October 2020.

After the appearance of two cases of hemorrhagic fever, the Director of Health reassures the populations

    Nouakchott, 02/13/2022

The Director General of Public Health, Mr. Mohamed Mahmoud Ould Ely Mahmoud, reassured, Sunday in Nouakchott, the populations about the haemorrhagic fever of which two cases were discovered recently.

He added, in a statement to the AMI, that joint missions from the Ministry of Health and the Ministry of Livestock were dispatched to the localities where these cases appeared with the aim of monitoring the situation, isolating suspect animals and to search for other possible cases in order to take the appropriate measures in time.

He affirmed that two cases of this fever were actually discovered, reviewing the appropriate measures taken by the ministry as soon as these cases appeared, specifying that the department immediately issued a circular to the centers and health points calling on them to be vigilant, monitor the health situation and take the necessary preventive measures. These arrangements, he said, are in line with the ministry's strategy to keep people informed of all developments relating to public health.Last modification: 02/13/2022 20:21:14

The Ministry of Health confirms 4 cases of contamination with Congo-Crimean hemorrhagic fever

      Nouakchott, 02/19/2022

After carrying out 31 tests, the Ministry of Health confirmed on Saturday the existence of 4 cases of contamination with Congo-Crimean hemorrhagic fever.

According to information received by the AMI, a case of death has been recorded since the appearance of Congo-Crimean fever, when another case of recovery was noted.

Two cases of contamination are in quarantine, one of which is critical, while the number of community contacts has reached 31 cases.

In a previous joint press release, the Ministries of Health and Livestock indicated that the consumption of pasteurized milk, milk after boiling and cooked meat protects against contamination with Rift fever, cases of which have been observed in the country.

The two ministries urged citizens to promptly inform the health and veterinary services in the event of hemorrhage in humans or abortion in animals.

The statement also invites breeders to limit the movement of herds between pasture areas and to cooperate with veterinary services to prevent the spread of the epidemic.

The ministries advise to follow the following precautionary measures:

      - Boil milk before consumption,

      - Cook meat well,

      - Isolate animals from human habitats,

      - Wash hands with soap after any contact with animals,

      - Incinerate and bury dead animals,

      - Sleep under mosquito nets,

      - Inform the health services in the event of the appearance of haemorrhage in humans,

      - Inform the veterinary services of any abortion in animals.

        WEEKLY BULLETIN ON OUTBREAKS
        AND OTHER EMERGENCIES

        EVENT DESCRIPTION

Five cases of Crimean-Congo haemorrhagic fever (CCHF) have
been reported from Mauritania recently. The first case reported
in week 4 (ending 30 January 2022) had originally consulted at
a local health facility on 27 January 2022, after presenting with
haemorrhagic symptoms: vomiting blood and nose bleeding.
The patient was then referred to a hospital in the capital city
of Nouakchott when symptoms persisted after two days and
then further referred to a national isolation centre at Friendship
Hospital a few days later. The patient is an animal herder who
had several events of contact with animals some of which are
known to have ticks. The case was confirmed positive for CCHF
by the National Institute of Research in Public Health (INRSP) on
        4 February 2022.

Four other confirmed cases were reported in weeks 5, 6, and 7,
including one at the Kiffa Regional Hospital Centre and two others
at the Nouakchott National Hospital Centre. All cases are males
with ages ranging from 25-52 years. Furthermore, all cases had a
known history of contact with animals due to their nature of work:
one of the cases is a butcher and the other three are shepherds.
All cases were confirmed to have CCHF by laboratory analysis.

Though profiles are similar for the cases, there has been no
epidemiological linkage between them. Each case comes from
a different area (Moughataas) of the country representing three
different wilayas: Trarza, Nouakchott-Sud, and Hodh el Gharbi.
The latter wilaya borders Mali. As of 15 February 2022, a total of
31 contacts have been listed from the cases and nine tests have
        been conducted.

        PUBLIC HEALTH ACTIONS

Regular coordination meetings are conducted to make
        strategic decisions regarding the outbreak

Response teams have been deployed to conduct
        investigations in affected wilayas

Surveillance activities have been reinforced across affected
        areas

Ongoing investigations in all areas where cases live including
the listing of contacts for cases

Infection, prevention and control measures have been
        reinforced in all health facilities

Community sensitization in areas at risk is ongoing as it is an
        endemic disease in Mauritania

Management of cases is being done in referral hospitals at
        the national and regional levels.

        SITUATION INTERPRETATION

Four cases of CCHF have been confirmed in a relatively short
period of time from different parts of Mauritania. This indicates
potential widespread transmission of CCHF in an already
endemic region. Continued monitoring and interventions need to
be strategically focused for a disease that involves a One Health
        approach.

        PROPOSED ACTIONS

Enhance surveillance monitoring with a main focus on One
Health initiatives since CCHF can be acquired by the exposure
of infected ticks or through contact with infected animals.

Nosocomial infections are also a possibility with CCHF and
precautions should be taken at all health facilities in order to
prevent infections among health care workers.

Surveillance should be reinforced at points of entry and
there should be good cross-border bilateral communication,
especially with some of the cases coming from near the
        border with Mali.

Risk communication should also be intensified since the risk
groups are wide, ranging from agricultural, pastoral, health
        care workers, etc.

                      EVENT DESCRIPTION

A Crimean-Congo hemorrhagic fever (CCHF) outbreak has been
ongoing in Mauritania since 5 February 2022 after confirmation of
the first case. The Nouakchott National Hospital Centre notified this
case during epidemiological week 4 (ending 30 January 2022) and
four additional confirmed cases were subsequently notified: two by
the same hospital, one by Kiffa Regional Hospital Centre and another
                      by Cheik Zayed Hospital Centre.

The sixth case was notified on 11 March 2022 by Hamed Hospital
of Boutilimit. This is a 35-year-old breastfeeding female residing in
Bir Barka village in Nebakhiya community, near Boutilimit, 157kms
from the capital city Nouakchott. She has no history of contact
with the previous confirmed cases. However, her husband lives in
Nouakchott where a confirmed case was identified and occasionally
comes to visit her. She reported history of travel to Boutilimit where
a confirmed case had been reported, with her last travel date being
28 February 2022. Her family possesses a herd of goats.

On 6 March 2022, she reported onset of fever, diarrhea, body
weakness, gingival and vaginal bleeding, prompting her to visit Bir El
Barka health post two days later. As no significant improvement was
observed, she consulted Hamed Hospital of Boutilimit on 11 March
2022 where she was immediately hospitalized, isolated and a sample
was collected. Laboratory results turned positive for CCHF on 12
March 2022. An investigation was conducted on 13 March and 10
contacts were identified and listed around the case.

As of 14 March 2022, 58 suspected cases have been tested for
CCHF and a cumulative number of six confirmed cases and two
deaths (CFR 33.3%) have been registered. Three cases have fully
recovered, while the sixth case is currently hospitalized and receiving
                      appropriate care.

The ages of the confirmed cases range from 25 to 80 years, with a
male predominance (66.7%; male/female sex ratio: 2:1). Three of the
four men are shepherds and the other is a butcher. The two females
                      are housewives.

The epidemic has affected six Moughataas so far, located in three
Waliyas: Kobenni and Tintane Moughataas in Hodh El Gharbi Waliya,
Ouad Naga, R’Kiz and Boutilimit Moughataas in Trarza Waliya, and
                      Arafat Moughataas in South-Nouakchott Waliya.

                      PUBLIC HEALTH ACTIONS

A coordination meeting was held on 15 February 2022, involving
the MoH and WHO. In addition, the One-Health Committee held
four meetings, While WHO held a 3-level teleconference on 15
February 2022. These various meetings purposed to organize
                      and support the response in-country.

A national response plan was developed and is being
                      implemented.

Teams already deployed in the field for polio surveillance were
                      briefed on CCHF.

Investigations have been conducted around confirmed cases
and contact tracing is ongoing. Ten contacts were listed around
the last confirmed case and are under follow-up.

A sero-prevalence study of CCHF among domestic animals
belonging to confirmed cases was carried-out to investigate the
                      potential source of infection.

continues to receive samples and is using RT-PCR for laboratory
                      confirmation.

National and regional hospitals have been capacitated to properly
isolate and manage suspected and confirmed cases.

Educational messages are being broadcast to sensitize the
population on preventive measures against the outbreak and
early referral to appropriate care in health care facilities.

                      SITUATION INTERPRETATION

The CCHF outbreak has affected three regions and six districts, with
no clear history of contact between the confirmed cases. Although the
primary source of infection is not yet clearly identified, the outbreak
seems under control with only six cases reported over the past six
weeks. However, quick activation of the One-Health Committee has
permitted a rapid implementation of a comprehensive response,
with joined interventions by human and animal health authorities.
Response teams on the ground to rapidly identify the various
                      hotspots.

                      PROPOSED ACTIONS

The national surveillance system should be reinforced to enable
early detection of CCHF cases and other public health threats.
Multidisciplinary rapid response teams should be trained and
ready to be deployed, both at national, regional and district
                      levels.

Considering that two of the affected Moughataas share
borders with Mali, there is need to put in place a cross-border
surveillance and collaboration between Mauritania and Mali
                      local health authorities.
