    شددت وزارة الصحة إجراءات الحجر الصحي علي منفذ السلوم البري بعد وفاة شخصين وإصابة 24 آخرين بمرض الطاعون بمنطقة الطرشة - 40 كيلو جنوب مدينة طبرق الليبية علي الحدود المصرية الليبية وأكد عادل أبوزيد- وكيل وزارة الصحة بمطروح- أنه قد تم رفع حالة الطوارئ بالحجر الصحي بمنفذ السلوم لتوقيع الكشف الطبي علي جميع العائدين من ليبيا بعد ظهور مرض الطاعون بها، وقال أبوزيد لـ ? الدستور? إنه متواجد حاليا بالسلوم لمتابعة الإجراءات الوقائية التي بدأت وزارة الصحة في اتخاذها للمسافرين عبر منفذ السلوم خاصة العائدين من ليبيا.

    ويسبب بكتيريا يعرف باسم (يرسينيا بيستس) الطاعون الدملي الذي يؤدي إلي تضخم الغدد الليمفاوية ويعد الطاعون الرئوي أكثر خطورة حيث ينتشر من شخص لآخر من خلال السعال أو العطس.

    كشف وزير الصحة ?حاتم الجبلي? في بيان ألقاه أمس عن أنه قد تلقي بلاغات عن انتشار حالات مصابة بمرض الطاعون علي بعد 250 كيلو متراً من الحدود المصرية مع ليبيا، وقال الوزير رداً علي بيان للنائب ?محمد الصحفي? إن الطاعون يعالج بالمضادات الحيوية والنوع الذي ظهر علي الحدود هو أقل الأنواع ضرراً، وكشف الوزير عن أنه تحدث هاتفياً مع وزير الصحة الليبي الذي أخبره بأن المرض ظهر في إحدي القري وسجل 11 حالة التهاب رئوي وقد عولجت 7 حالات وخرجوا من المستشفي، وتم استدعاء إحدي الشركات المتخصصة التي قامت برش المنطقة بالكامل للقضاء علي الفئران والبراغيث، وهذه المنطقة بالذات كان قد ظهر فيها المرض من قبل عام 1977.

      Another nice find. Bubonic plague is certainly strongly suspected, but not necessarily confirmed. These sources do have a lot of inconsistencies, including the numbers of deaths and cases, whether the plague is bubonic or pneumonic, and even the distance to the Egyptian border.

      The source that RSOE is quoting mentions a suspect case in Egypt, something that no other source mentions. And the first post in this thread mentions a family with one death and the others critically ill, that it considers undiagnosed, something else that I have not seen in any other source. It almost seems like there might be two separate outbreaks here - one about 50-100 km from the border with 13 cases inlcuding one death (of which 10 might be excluded as plague), and a second outbreak 250 km from the border with 11 cases, one death and 7 discharges (which sounds like pneumonia, not bubonic plague).

      That would add up to the 24 cases quoted. I believe so far we have read of the death of an 18yr old man (June 10?), and a middle-aged woman.

      Information here is a little difficult to determine.

            Ouch. Leakage of biological weapons? That's a new one, and that would be bad. That would also explain why there might be more than one illness at work here, and would potentially be very alarming.

            It is important though, not to jump to conclusions about that. Plague naturally was reported from that area twice in the past 35 years.

            IMO, the WHO can't get to this one fast enough!!!

                Sources familiar with the newspaper (today's Libya) said that two deaths have been recorded due to the illness of the plague that has spread in the Trcp (40 kilometers south of the city of Tobruk) among 13 cases booked for treatment at the hospital.

                Al-libya day management of Hospital TOBRUK that the government is exerting great effort to contain the disease, since vaccination began in the population of the city of Tobruk, and areas around it against the disease that he had passed through some insects, as the prohibition of entry and exit from the Trcp.

                  The only "plague" in Libya is A/(novel)H1N1.

                  Plague, due to Yersinia Pestis is endemic worldwide but is easily treatable by controlling the rat population which carries fleas that transmit the disease to humans. In the event the rats can not be controlled, then a host of cheap generic antibiotics are very effective treatment for this disease. These include the tetracycline compounds and Cipro derivatives.

                  These news reports are propaganda pure and simple. There is no chance that the plague could become epidemic in any country today. Of course, if there were a breakdown in civilization caused by a severe influenza pandemic, yes we could once again see epidemic plague but not today; it is too early for that. This is why these reports are not credible.

                    Several things seem to be occurring here. I agree with you there is more fear in that area than would seem to be warranted, and some of the countermeasures seem extreme. However, a couple things are clear.

                    First, there is some kind of outbreak in that area, as there have been a couple dozen cases within two weeks. It is possible that this is due to rats and fleas, but it has been 25 years since plague has been reported in that area. It is also possible that there is an outbreak of pneumonic plague due to the accidental release of a biological weapon (see above post). The 11 cases of "pneumonia" mentioned would be unusual for a natural bubonic plague outbreak as only a tiny minority of bubonic plague cases develop pneumonic plague.

                    The other thing that is not clear is exactly the etiological agent involved. It might be yersinia pestis, but it could just as easily be anthrax in the livestock (with the pneumonia being inhalation anthrax), or some hemorrhagic fever, or even H5N1 could be mistaken for pneumonic plague. It is also possible that more than one agent is involved. Laboratory confirmation of the agent(s) involved would explain a lot.

                      Libyan authorities have reported an outbreak of bubonic plague in the
                      Mediterranean coastal town of Tubruq, and the World Health
                      Organization (WHO) was sending a team to investigate, a WHO official
                      said on Tuesday [16 Jun 2009].

                      The cases, approximately 16 to 18 have been reported, would be the
                      1st in more than 2 decades in Libya of the disease known in medieval
                      times as the Black Death, according to John Jabbour, a Cairo-based
                      emerging diseases specialist at WHO.

                      "It is reported as bubonic plague," Jabbour said, adding WHO still
                      didn't have "a full picture" of the situation. "It is officially
                      reported by Libya... Tomorrow [17 Jun 2009], the WHO is deploying a
                      mission to Libya to investigate the whole situation, to see how many
                      of the cases are confirmed, or not confirmed."

                      He said preliminary information from Libyan authorities showed 16 to
                      18 reported cases including one death, and that Tripoli had asked for
                      assistance from the global health body.

                      Tubruq, where the new cases were reported, is approximately 125 km
                      [78 miles] from the Egyptian border and was the scene of previous
                      plague cases decades ago, Jabbour said. Egypt, already fighting to
                      contain an outbreak of the H1N1 flu virus, said it had no reported
                      cases of plague.

                      Globally the WHO reports about 1000 to 3000 plague cases each year,
                      with most in the last 5 years occurring in Madagascar, Tanzania,
                      Mozambique, Malawi, Uganda and the Democratic Republic of Congo. The
                      USA sees about 10 to 20 cases each year.

                      Consumption of the meat of an infected camel has been reported to
                      cause plague in Libya in 1976 (1). The abstract of the paper is:
                      "In 1976, in a small, remote Libyan village, an apparently sick camel
                      was slaughtered and skinned, and the camel meat was distributed for
                      human comsumption. A few days later, 15 villagers suffered a severe
                      febrile illness. Of the 5 individuals who had participated in the
                      killing and dispensation of the camel, all were dead within 4 days.
                      When samples of serum from 9 of the remaining patients were examined,
                      7 were found to be positive for plague as determined by the passive
                      hemagglutination test. Another 6 persons became ill after killing 2
                      goats, and the serum of one goat contained antibodies to _Yersinia
                      pestis_. Because all of the remaining patients except one were
                      treated early enough, they recovered. These incidents confirm
                      previous reports that the camel and the goat are susceptible to
                      naturally occurring plague infection and have a significant role in
                      the dissemination of human plague."

                      Two more recent outbreaks in the Arab world have been reported in
                      2005. In Saudi Arabia (2), a cluster of 5 plague cases were reported
                      including 4 with severe pharyngitis and submandibular lymphadenitis.
                      These 4 case-patients had eaten raw camel liver. _Yersinia pestis_
                      was isolated from bone marrow of the camel and from jirds (_Meriones
                      libycus_) and fleas (_Xenopsylla cheopis_) captured at the camel
                      corral. In Jordon (3), a 1997 outbreak of plague was reported,
                      associated with cervical lymphadenopathy and fever, occurred in the
                      Jordanian village of Azraq ad-Druze, 50 km [31 miles] west of the
                      border with Saudi Arabia. The 12 cases who presented at hospital were
                      initially assumed to have tularaemia, and all were successfully
                      treated with gentamicin. But all 12 were found to have anti-_Y.
                      pestis_ IgM. 11 of the 12 patients reported that, 2-4 days before
                      their symptoms appeared, they had eaten the meat cut from the carcass
                      of the same camel, either raw (10 cases) or cooked (one case). All 12
                      patients were diagnosed as cases of pharyngeal plague (the 1st cases
                      of plague reported in Jordan for more than 80 years), caused by _Y.
                      pestis_ that most had acquired when they ate raw meat from a camel
                      that was infected with the pathogen.

                      The source of the cases of bubonic plague in northeastern Libya is
                      not reported in this posting but may be related to camels and camel
                      fleas.

                        It suggests that they really do have a plague outbreak.

                        Despite what some might think of Libyan health care, their doctors are very well trained and are not dummies. They know the difference between bubonic plague and pneumonia from pandemic flu. So, IMO, if doctors in Lybia have made these diagnosis, then I think we can rely on them being accurate.

                        Given this new information, I think my previous opinion may well be incorrect.

                            Your welcome. I know for many FT members, what we track is pandemic flu, but FT does track other outbreaks. I think this probably is yersinia pestis plague, but it could be anthrax, hantavirus, hemorrhagic fever, or some other similar agent. I think H1N1 and H5N1 are fairly unlikely here.

                            There is some kind of outbreak here, even if not flu related.

                              The Egyptian Foreign Ministry is following up on a problem whereby 6000 Egyptians are stranded on the borders with Libya because the Libyan authorities of the Mossaed crossing have requested them to pay 500 dinars before allowing them to leave.

                              Foreign Ministry Spokesman Hossam Zaki said Foreign Minister Ahmed Abul-Gheit gave instructions to immediately solve the problem of the Egyptians that are stranded on the borders with Libya and give them their full rights, as the Libyan government is collecting fines from the foreigners who have violated the Libyan residence laws.

                              Zaki has affirmed that this issue gets the attention of the competent members of the government and at the highest level, as it is keen on safeguarding the interests of the Egyptians abroad.

                              He said the Egyptian authorities are making intensive contacts with the Libyan side to reach a quick and appropriate solution.

                              Libya requires the foreigners to pay 500 dinars before allowing them to leave.
