Infection with avian influenza was confirmed Feb. 4. His mother reported a history of close contact with a dead poultry. The child was in a good general condition. The MOHP reported this was the 54rd case of highly pathogenic avian influenza in Egypt. 23 of the cases have been fatal.

Infection with the H5N1 avian influenza virus was confirmed by the Egyptian Central Public Health Laboratory, according to the World Health organization. The MOHP said the girl was in good and stable condition Jan. 26. Her mother reported a history of close contact with sick poultry. The WHO reported this was the 53rd case of highly pathogenic avian influenza in Egypt; 23 of the cases have been fatal.

      • District: Kerdasa (urban area located in western Giza; population 460,000)
      • Village: Kafr Abu Hegazi (Saad Zaghloul Street near Abu Hegazi mosque)
      • Event summary: Female infant, 21 months old, began experiencing symptoms January 9 evening. The family kept unvaccinated poultry that had become sick and died in the previous week. Infant was taken to hospital January 10. AI was suspected on Jan. 11; patient isolated; tests taken. Results on January 12 showed AI; patient was given Tamiflu and transferred. Specimens from mother and other contacts tested negative. The child was reported in good, stable condition Jan. 14.
        • This was Egypt's 52st human infection.
        • For Sixth of October Governorate, it was the fourth suspected human infection but the first to be confirmed positive. For Kerdasa district, it was the first suspected human case.

      • Medical response: The infant around 8 p.m. January 9 began experiencing symptoms of fever, high temperature (41 C), cough, nasal discharge, vomiting, rapid breathing. Family sought medical care at Imbaba Fever Hospital on January 10.
        • On January 11, case suspected as AI; patient isolated; specimen taken; Tamiflu administered at 1 p.m. Lab test of specimen returned positive for H5N1 virus on January 12; results by Central Public Health Laboratory (CPHL) and NAMRU-III. Case was referred to Manshiyet el Bakry Hospital, Cairo.
        • Specimens (blood samples and throat swabs) were taken at Imbaba Fever Hospital from mother and others who had contact with poultry; contacts were kept under supervision at home by Kerdasa Hospital health team. Negative results were found from specimen tests by CPHL.

      • Condition of the home: The family had raised poultry at home that were unvaccinated. Last quantity was 150 chickens, 22 ducks, and 8 geese.
        • The mother said some poultry began dying about a week before the child became sick. During the week, the mother and her family slaughtered the remaining poultry that were sick: 70 chicken and 6 geese. ESU reported they kept some live ducks and sold some ducks in the local market; and discarded many of the dead poultry in the garbage and around an irrigation canal.
        • ESU said samples from poultry and from slaughtered birds stored in the house refrigerators were tested by MALR's National Laboratory for Veterinary Quality Control on Poultry Production (NLQP); all were found positive for H5N1 virus. MALR/GOVS said NLQP found only live ducks, only frozen. MALR/GOVS said samples were also taken from a neighbor's house located across a narrow street and were found positive. See related report.

      • Findings of the Ministry of Health and Population's Epidemiology and Disease Surveillance (ESU): Mother's awareness and notification of dead poultry helped in early diagnosis and administering of Tamiflu (around 36 hours after the onset of the disease).<!-- This is the first confirmed AI infection in Sixth of October since the appearance of AI in Egypt.       • Tests came back negative on a suspected human case of AI in a 4-year-old boy in Menoufiya Governorate, according to the MOHP/ESU. The suspected case was reported in the news media.

      • Event summary: Female, 16 years old, began experiencing symptoms December 8. Admitted in Assiut University Hospital December 13 under ventilator in critical condition. Died December 15. This was Egypt's 51st human infection.
      • Findings of the General Organization for Veterinary Services (GOVS) Epidemiology Unit:
      • Among homes neighboring the girl's home, there had been a high mortality and flu-like symptoms for several weeks, with dead birds discarded in a drainage ditch that passes by the girl's home. The girl's family said it had recently purchased, slaughtered, and eaten two ducks; according to neighbors, one of the ducks died and the family quickly slaughtered the second. When the girl began to have trouble breathing, she was mistakenly diagnosed at a local hospital as having normal influenza.
      • Medical response: Family members said the deceased began to suffer a headache and fever on December 7, 2008. When she developed signs of respiratory distress, she was taken to a hospital Dec. 8, 2008. Only one doctor was present to respond, diagnosing the case as normal flu and giving the patient some medications. The family went to another hospital on Dec. 11, 2008 after severe complications, bloody sputum.
      • Condition of the home: The deceased’s home is one of a small number of scattered houses at a far side of the village surrounded mostly by agricultural land. An agricultural drainage canal passes in front of the house.
        • There were conflicting accounts on whether the deceased's family raised poultry. <!-- (The deceased&rsquo;s family told GOVS that poultry had not been kept at the home for six months. Neighbors confirmed this, and local veterinary officers said the house had no signs of keeping poultry. However, a report by the Ministry of Health and Population stated that the deceased's family raised chicken and ducks.) --> Family members told GOVS that the girl had not left the home or visited anyone in the recent period.
        • Family members said they bought two adult ducks from a market two months earlier, slaughtered and ate them directly. Neighbors said the family bought two ducks for rearing; that one of the ducks died, and the other was quickly slaughtered, producing blackish blood.

      • Condition of neighboring homes: Nearby neighbors keep poultry. Sampling was conducted by MALR/GOVS, MALR Central Lab, local veterinary preventive medicine specialists and a local lab team. Samples taken were nine tracheal swabs and nine cloacal swabs from ducks only.
        • In general, most of houses said that in the last month and a half to two months, there was a high mortality approaching 100 percent in the kept poultry with signs of cyanosis of the head, whitish diarrhea and nasal discharge.
        • As a general practice, dead birds are disposed of by being thrown in the drainage canal.
        • Most of sampled houses were vaccinated in the last vaccination campaign.

      • Local markets: There are two markets, a small market on Thursdays 3 km from the deceased's house, and a big market most weekdays in the district seat. A number of peddlers collect birds from the houses to sell at the markets – one or two peddlers pass every day. Some peddlers sell two-week-old chicks to the houses to raise.
      • Actions taken by the local veterinary directorate: Massive vaccinations in the surrounding houses; disinfection for the surrounding houses; culling of 41 birds in district market.
