    That Promed posting has been bugging me for the past few months, even before this outbreak. No other source has indicated a longer incubation period for plague than about 3-4 days, but that moderator says up to 14 days.

    It is of note that he is considering an outbreak of a different pathogen that he believes has an incubation period of 2-4 weeks and is trying to use the incubation period of plague to argue that the other disease is not plague. Maybe he exaggerated to prove a point...

    And again, mention that the dog ate the marmot. If the dog ate the marmot, it likely got pneumonic plague itself. The man didn't need to be bitten by the dog's fleas - he was exposed to the dog's cough.

          Taxi drivers have been earning a small fortune by sneaking residents around roadblocks and out of a remote Chinese town cordoned off after an outbreak of pneumonic plague.

          Local officials have denied that any residents of Ziketan, a mainly ethnically Tibetan region of the western Qinghai province, have tried to escape the area by evading police checkpoints set up on roads in a 17-mile radius of the mainly nomadic farming community of about 10,000 people.

          Three people have died of the rare pneumonic plague, one of world's deadliest diseases. Another nine are undergoing treatment in isolation wards of a local hospital and one was yesterday reported to be in a critical condition.

          One resident escaped to the neighbouring province of Gansu where the individual was picked up by police and placed in isolation in a hospital.


          The individual escaped with a taxi driver who knew how to navigate his way around the police checkpoints. Hundreds of others are reported to have left by the same means while some walked out and others drove motorcycles across the plateau.

          The going rate for a taxi out of Ziketan was 2,000 yuan (?200) per car. The person who escaped to Gansu was accompanied by six other people, incuding the driver. That rate is effectively a fortune in China where a single ride in most small towns costs 5 yuan (50p) and car hire for a day in rural areas is about 600 yuan (?60).

          The occupants of the car ? all Tibetans ? were picked up by the police, most in the next town of Heka.

          Tibetans have not been the only people trying to get out of town ? even though there have been no reports of new cases and the World Health Organisation has said the outbreak has been contained.

          Han Chinese residents withdrew their children from a local school and demanded a refund of tuition fees because they were all leaving at once.

          Many people remain in the town ? mostly local Tibetans and Han Chinese who have long been residents of Qinghai. More recent arrivals from other parts of China in search of work and business opportunities in the less-developed West of the country have left, local residents said.

          In the absence of reports of new cases since the first victim died on July 30, officials are considering allowing the town to reopen on August 8, local sources said.

          Occasional cases have been reported in western China, usually in Tibetan areas, over the past couple of decades, but almost all have been limited to herders in remote areas who were infected with either pneumonic or bubonic plague after eating wild marmots or being infected by fleas carried by rats. Such outbreaks are much easier to control.

          However, more herders have been moved into towns in recent years as part of a policy by the authorities to provide permanent housing for nomads and establish greater control over Tibetans.

          The first fatality was a herdsman who succumbed to the plague three days after burying his dog, which had died suddenly, apparently after coming into contact with a diseased marmot. Another 11 people became infected after attending the man?s funeral. Pneumonic plague is spread through the air and can be passed from person to person through coughing.

          The latest victim was a 64-year-old man named Danzhi. He was a neighbour of the 32-year-old herdsman and of a 37-year-old man who died at the weekend.

            And the quarantine is lifted. If they managed to save all nine remaining patients, then they likely had some indication this was plague before it was officially announced. You don't save 75% of pneumonic plague patients unless you treat with the proper antibiotics promptly.

            The quarantine blockade was lifted around the pneumonic plague-hit town in the northwestern province of Qinghai as of 8 p.m. Saturday, local authorities said.

            This came after there has been no reports of new infections for more than a week.

            The infectious disease has killed three villagers in the town of Ziketan, in Xinghai county in the Hainan Tibetan Autonomous Prefecture. The outbreak began July 30 and made 12 people in the town ill.

            Nine patients in quarantine are in stable condition and are recovering from the illness, said Dong Fukui, a deputy prefecture governor.

            Earlier one patient was near to death and another was in serious condition.

            Quarantine was also lifted for another 332 people who had close contacts with the patients.

            Dong ordered medical staff to continue disinfecting the region and step up monitoring over plague outbreaks.

            Local authorities have handed out more than 40,000 leaflets and disks on plague prevention along with other efforts to allay residents' fears.

            The World Health Organization (WHO) will soon evaluate the disease prevention and control efforts.

            The town covers 3,000 square kilometers and has a population of 10,000.

            Pneumonic plague is one of the world's deadliest infectious diseases; patients can die 24 hours after infection, says the WHO.

              And now that this outbreak appears contained, I can post this "joke" that appeared in the New York Daily News about the remoteness of the location of the outbreak.



              The story: There's a little panic as Chinese authorities quarantine a small town called Ziketan when three people die of pneumonic plague. "A lot of people ran off when they heard that another person died," says a food seller named Han. "They headed back home with food, mineral water and their donkeys."

              In other words: They would have headed back home with flat screen TVs and Lexus SUVs, but we do not have such things here in Ziketan. The closest we have to a Lexus dealership is "Xuiu's Used Donkey and Yak Dung Emporium." So, it's a bit different.

                    I would expect a lot of plague cases in animals and even possible some sporadic human bubonic plague cases to be reported out of China in the next few weeks. The alarm has been raised, and it really is one of those "Seek and ye shall find".

                          Plague in China
                          11 August 2009 -- On 1 August, the Ministry of Health (MoH), China reported a cluster outbreak of pulmonary plague cases in the remote town of Ziketan, Qinghai province. The first case was a 32 year old male herdsman, who developed fever and hemoptysis on 26 July. He was referred to a hospital but died en route, and was buried the following day. On 30 July, 11 people who had close contact with the case (mainly relatives who attended the funeral) developed fever and cough, and were all hospitalized. On 1 August, specimens taken from all these 12 people, including the 1st case, tested positive for plague.

                          On 2 August both the 64 year old father-in-law of the first case and a 37 year old male neighbour of the first case (who helped to bury the corpse) also died. Of the remaining 9 cases, 1 is in critical condition, 1 had acute symptoms of fever and cough, and 7 are in stable condition.

                          As of August 6, the local health authority has isolated 332 close contacts for further medical observation, and implemented traffic control around affected area. Experts on both disease prevention & control and clinical management have been dispatched to Qinghai province. Protective clothes, X-ray machines and other medical equipment have been sent to the affected area. Prevention guidance pamphlets have also been disseminated.

                          According to the epidemiological investigation, the source of this outbreak was a wild marmot, which had contact with the dog of the index case. Ziketan is in an area of natural plague bacteria circulation amongst animals and at the present time it is the active season for plague transmission amongst animals. No drug resistance of the bacterium has been found so far and the 3 death cases have been attributed largely to delayed treatment.

                          China has established a national surveillance network for plague, and has prepared necessary supplies in high-risk areas. After this outbreak, special funds, supplies and experts were quickly dispatched to the affected area.

                            The report from China of this unusual outbreak has just been
                            published: Hu Wang et al: A Dog-Associated Primary Pneumonic Plague
                            in Qinghai Province, China. Clin Infect Dis. 2011;52: 185-190.

                            This was reported by ProMED in 2009 in a thread (see ProMED archives
                            below) that ended with the report from WHO and a comment by ProMED
                            that no further information regarding the patient, who was in
                            critical condition, was available. This paper reports that the
                            patient survived and that the index patient contracted pneumonic
                            plague after carrying his dead dog on his shoulders around his neck
                            for 400 meters to bury it.

                            It also clarifies statements in the WHO report that could have been
                            misunderstood to mean that plague had been transmitted from the index
                            case's corpse. To quote WHO:

                            "The 1st case was a 32-year-old male herdsman who developed fever and
                            hemoptysis on 26 Jul 2009. He was referred to a hospital but died en
                            route and was buried the following day. On 30 Jul 2009, 11 people who
                            had close contact with the case (mainly relatives who attended the
                            funeral) developed fever and cough and were all hospitalized. On 1
                            Aug 2009, specimens taken from all these 12 people, including the 1st
                            case, tested positive for plague. On 2 Aug 2009, both the 64-year-old
                            father-in-law of the index case and a 37-year-old male neighbor of

                            The Chinese authors defined a close contact as "a person who lived
                            with or had face-to-face contact (within one meter) with the patient,
                            and a direct contact as a person who once was together with a patient
                            in the same room or in the same car with closed windows."

                            Only the 11 close contacts became infected; none of 61 direct
                            contacts became infected, and nobody who attended the funeral but had
                            not previously been a close contact became infected. They also
                            reported that most of the patients (6 of 11, excluding the index
                            case) in this outbreak developed septicemic plague complicated with
                            shock; 2 of 11 developed multiple organ failure, and one of 11
                            developed meningitis in addition to the plague pneumonia.
