New outbreaks in Tianjin and Shanghai, a few facts and a few unanswered

2020-11-11 14:16 Reporter: Dai Xuan Editor: Pei Jianfei Fan Yijing

Wu Zunyou said that there are already many clues that point to seafood imported by cold chain transportation, which may be the source of the epidemic.

Since November 8, confirmed cases and asymptomatic infections have occurred in Tianjin, Shanghai, and Anhui, including several cold storage movers, cold chain transport drivers, and airport movers.

Knowing Jun noticed that according to the current official notification information, the three newly confirmed patients in Tianjin this time are all related to imported cold chains; a porter at Shanghai Pudong Airport has confirmed the diagnosis and has not been exposed to frozen food recently; Anhui has also confirmed one He is a colleague with the confirmed patient in Shanghai.

Last night, in an interview with CCTV's "News 1+1" column, Liang Chunzao, deputy commander and deputy head of the Prevention and Control Headquarters of Tianjin Binhai New Area, said that after tracing the source, the evidence so far points to the current round of the epidemic. From things to people".

Wu Zunyou, chief expert of epidemiology at the Chinese Center for Disease Control and Prevention, also said in an interview with the media that there are already many clues pointing to seafood imported by cold chain transportation, which may be the source of the epidemic. "More and more evidence shows that frozen seafood or meat products may introduce viruses from epidemic countries to our country." Wu Zunyou said.

Several facts about new outbreaks in three places

    1. Three newly confirmed and asymptomatic cases in Tianjin are all related to the imported cold chain

At 2 o'clock in the morning on November 8, the Tianjin Prevention and Control Headquarters announced that after receiving the notification from the two places, it immediately sealed and controlled the relevant cold storage in Binhai New Area and Dongli District.

On the 8th, the Tianjin Centers for Disease Control and Prevention released information that a worker in the Hailian Cold Storage in the Sino-Singapore Eco-city in Binhai New Area was diagnosed with the 138th local confirmed case of new coronary pneumonia in Tianjin. The case is a loading and unloading worker of Tianjin Hailian Frozen Food Co., Ltd. The patient participated in the company's imported frozen food handling work on November 4. The city expert group confirmed the 138th local case in Tianjin.

At 14:00 on November 9th, news from the Tianjin Prevention and Control Headquarters showed that one new asymptomatic patient in Tianjin, a truck driver, had been to the Binhai New Area, where the 138th newly confirmed case was working. Chenghailian Refrigerator pulls goods. Through epidemiological investigations, it was found that although the driver was not a cold storage worker, he was connected to the scene involved and belonged to the second circle of investigation by the disease control department. (According to the latest notification from Tianjin on November 11, the patient has been converted to a confirmed case)

According to a report from the Tianjin Center for Disease Control and Prevention, at 10 am on November 10, during the "two full coverage" investigations of cold chain practitioners and frozen goods nucleic acid testing in the city, it was found that a cold chain practitioner in Dongjiang Port of Binhai New Area swallowed The swab sample tested positive for the new coronavirus nucleic acid. The Municipal Center for Disease Control and Prevention immediately reviewed and the result was still positive. At 17:00, the city-level expert group judged that he was asymptomatic and has been transferred to a designated hospital.

    2. A porter at Shanghai Pudong Airport has confirmed that he has not been exposed to frozen food recently

On November 9th, according to a press conference on the prevention and control of the new crown pneumonia in Shanghai, one new confirmed case was reported in Shanghai. The case was in Pudong Airport and was engaged in porting work.

At 13:45 on November 8th, due to fever, nasal congestion, fatigue and other symptoms, the patient rode an electric bicycle to the fever clinic of Pudong New Area People's Hospital alone. According to the patient's statement, he has been carrying out porterage work at the Cargo Terminal of the West Airline Division of Pudong Airport since April 8, and has never left Shanghai. He said that he had no history of living in high-risk areas in the 14 days before the onset of illness, no history of contact with people infected with the new crown virus, and no history of visits to other medical institutions in the past 3 months.

According to the official report, according to the current investigation, the case himself has not been exposed to frozen food recently, and his work and his trajectory before diagnosis are not related to the third CIIE.

    3. Anhui confirmed a case of Shanghai-associated new coronary pneumonia

On November 10, according to the website of Anhui Fuyang Health and Health Commission, a news was published on the website of Yingshang County, Fuyang City. The patient Lan was a close contact of Wang XX who was a confirmed case of new coronary pneumonia notified by Shanghai on November 9. After county-level nucleic acid testing and provincial and municipal review, Lan Mou was diagnosed as a case of new coronary pneumonia at 8:17 on November 10.

The case worked in a company in Shanghai for a long time. Before returning to Yingshang, he and Wang were engaged in airport cargo handling work at Shanghai Pudong Airport. He always wore a mask at the workplace and took his temperature every time he entered the workplace.

Epidemiological investigations showed that Lan returned to Yingshang on the Wan K58699 bus from Chuansha, Shanghai at 16:00 on November 5, and then took the Wan K5098J van back to his home in the community. On November 7, Lan and his daughter-in-law took their grandson to Yingshang County People's Hospital for treatment. During the whole process, they wore masks and did not go out at other times. Prevention and control measures have been implemented in the place of residence of the case and the places visited.

There are still several problems to be solved in this epidemic

After knowing you, I found that since July 3, at least 10 places across the country, including Xiamen in Fujian, Dalian in Liaoning, Yantai in Shandong, and Wuhu in Anhui, have tested positive for imported cold chains. The goods mainly involve frozen vannamei shrimp produced in Ecuador and frozen chicken wings imported from Brazil. Will cold chain product transportation infect the new crown? How long will the virus survive? How to deal with the risk of transmission? These problems have yet to be resolved.

    1. Is the source of the epidemic overseas?

Wu Zunyou, chief epidemiologist of the Chinese Center for Disease Control and Prevention, said in an interview with the media that the virus will not freeze to death in a low temperature environment. Generally speaking, the lower the temperature, the longer the virus will survive. If the virus is placed in an environment of minus 180 degrees Celsius, such as liquid nitrogen, it may survive for decades or even hundreds of years. The low temperature environment created by the cold chain provides a good space for the virus to survive.

The temperature of the cold chain is different, the time the virus survives is also different. Generally speaking, in an environment of minus 20 degrees Celsius, the virus can survive for several months. Even with ordinary cold chain transportation, the virus can survive for several weeks. If the contaminated items, including food or outer packaging, are transported through the cold chain in an area with a high incidence of the new crown epidemic, the virus can be brought to the non-epidemic area, causing contact transmission and detonating a new new crown epidemic.

Wu Zunyou also said that looking back at the early epidemic in Wuhan at the beginning of the year, patients in the South China Seafood Market were also mainly concentrated in frozen seafood areas. These clues all point to seafood imported by cold chain transportation, which may be the source of the epidemic. Following this line of tracing, it was indeed found that seafood imported from many countries was contaminated with the new coronavirus. Subsequently, customs in many cities across the country tested imported frozen seafood or meat and found that the new coronavirus nucleic acid test was positive. More and more evidence shows that frozen seafood or meat products may introduce viruses from affected countries into my country.

In addition, according to the official blog issued by Tianjin, on November 8, the Tianjin CDC smeared samples of the 138th local confirmed case of the respiratory tract and related cold storage environment in Tianjin, and conducted high-throughput sequencing and sequencing of the new coronavirus whole genome analysis. Sequencing analysis results on the 9th showed that it was highly similar to the strains circulating in North America from March to June, and belonged to the L-genotype European family branch II (North American branch).

    2. How are these patients infected?

So, if the virus does enter the market through the cold chain, how can it be infected?

According to a disease control expert who has experienced multiple rounds of epidemics, the prerequisite for infection is that the virus is a live virus, not just a positive nucleic acid test.

Jiang Rongmeng, director of the Office of the National Center for Infectious Diseases Medical Quality Control, said that when the environment is contaminated by viruses, people are in it and lack protection, the environment is highly enclosed and the viral load is high, the virus may be infected through aerosols; such as humans. Contact with the virus and then touch the human mucous membrane may cause contact transmission.

Liang Chunzao said that the confirmed cases notified on the 8th had not been to the place where the epidemic occurred before they were diagnosed, nor had they been in contact with other sick cases or patients with fever. The only possible route of infection was "from object to person." The asymptomatic infection announced on the 9th, he did not have direct contact with the confirmed case, but had been to the same cold storage, his infection route may also be "from object to person."

    3. How to deal with the risk of cold chain transmission?

On November 9, the Comprehensive Prevention and Control Mechanism of the State Council issued the "Work Plan for Preventive and Comprehensive Disinfection of Imported Cold Chain Food." This plan points out that, without changing the existing overall prevention and control arrangements in various regions, according to the logistics characteristics of imported cold chain foods, after completing the new crown virus detection and sampling work as required, the inspections, transportation, and boxing will be carried out at ports. In links such as warehouses, wholesale and retail, preventive and comprehensive disinfection of imported cold-chain foods will be implemented before they come into contact with people in China for the first time.

During the Tianjin epidemic, imported foods with weakly positive outer packaging detected from Tianjin to Shandong were transported to Hailian cold storage on November 4, unpacked and unpacked, and were loaded and transported to Shandong on November 5. The two infected people reported in the local area recently worked in cold storage during this period.

After the outbreak in Tianjin, other regions also reiterated the prevention and control of cold chain transportation of food. On November 10, at the 176th news conference on the prevention and control of the new crown pneumonia epidemic in Beijing, Beijing Municipal Government spokesperson Xu Hejian pointed out that Beijing must strictly implement cold chain food virus detection and disinfection work, and organize the implementation of preventive comprehensive disinfection work , To realize the traceability of the whole process closed-loop control, and minimize the risk of the new crown virus importing through imported cold chain food.

Wu Zunyou pointed out that to do a good job in the prevention and control of epidemics in the cold chain, three inspections must be done. One is to conduct sampling tests on imported products and outer packaging to detect contaminated food in time; the other is to work on the cold chain business industry. The environment, including transportation vehicles and cold storages for storage, etc., are regularly sampled to detect pollution; third, for cold chain practitioners, nucleic acid screening should be carried out no less than once a week to detect early infections in time. Find out the epidemic in time and put it out in time. In terms of management, on the one hand, it is necessary to train all managers and practitioners of the cold chain industry to strengthen the implementation of normalized prevention and control measures; on the other hand, it is necessary to regularly supervise and inspect the implementation of normalized prevention and control measures for the new crown epidemic in the cold chain industry , Find weak links and block the risk points in time.

Tianjin and Shanghai have been listed as medium-risk areas

From November 8th to 9th, Tianjin and Shanghai successively reported locally confirmed cases of new coronary pneumonia. Hangu Street, Binhai New District, Tianjin, Zone A and B of Zhongxin Fishing Port Cold Chain Logistics Zone, and Zhuqiao Town, Pudong New District, Shanghai, have been listed as medium-risk areas.

Knowing Jun discovered that at present, Changchun, Wuhan, Xiamen, Fuzhou, Shenyang, Harbin, Langfang and other places have issued announcements on the return and return of medium-risk areas in Tianjin and Shanghai, and respectively proposed "Report to the community" and "Provide within 7 days. Prevention and control measures such as “nucleic acid test negative certificate”, “centralized isolation medical observation for 14 days”.

The situation of imported cold chain epidemic prevention is complicated and must not be taken lightly

      2020-11-14 23:02 Editor: Wang Yanhu

The society cannot underestimate the complexity, uncertainty, and unknownness of the spread of the epidemic. It must be prepared to fight a protracted battle in the normalization of prevention and control, and put an end to luck.

The Jinan Municipal Health Commission recently announced that at 5 pm on November 13, the Jinan Center for Disease Control and Prevention tested positive for the new coronavirus nucleic acid in imported frozen food and packaging samples stored in the cold storage of the Gasgoo Logistics Park. At about the same time, the Chengguan District of Lanzhou City carried out regular sampling and sampling at the Jiaojiawan Refrigeration Plant on November 13, and found a positive nucleic acid test for a shrimp package sample from Tianjin Port cold chain shrimp. Just a few days ago, Tianjin successively detected that cold chain practitioners were infected with new crown pneumonia, and the outer packaging of cold chain food was also detected with new crown virus.

Moving forward, since June of this year, more than 10 provinces including Liaoning, Anhui, and Fujian have all tested positive for the new coronavirus nucleic acid on imported cold chain food or packaging. These repeated risks indicate that the epidemic prevention and control situation in the cold chain food import link may be more complicated than many people think, and all parties need to go all out and must not be taken lightly.

Among the many imported foods, a considerable number of categories, including fresh food, are highly dependent on cold chain logistics. Compared with ordinary logistics, cold chain logistics has higher requirements for storage temperature, which also provides convenience for the parasitic of the virus.

Wu Zunyou, chief epidemiologist at the Chinese Center for Disease Control and Prevention, said that even with ordinary cold chain transportation, the virus can survive for several weeks. In addition, cold chain food imports are from a wide range of sources, and uncontrollable factors may be associated with them from the place of production to the transportation process. Under such a situation, strengthening the epidemic prevention and control of imported cold chain foods in light of reality can be said to have become a key point for "foreign defense import".

Not long ago, the Joint Prevention and Control Mechanism of the State Council issued the "Work Plan for Preventive and Comprehensive Disinfection of Imported Cold Chain Food", which clearly requires all localities to implement closed-loop control over imported cold chain food. The division of labor among customs, transportation, health, market supervision, local governments, and related production and business units has also been clarified one by one. The implementation of these measures and division of labor should be the same as the previous "internal non-proliferation" to ensure that every link is foolproof.

It is worth mentioning that the above-mentioned plan clearly states that "anything that cannot be provided with a disinfection certificate cannot be sold on the market." This is particularly critical. Up to now, no cold-chain foods with viruses have been found on the market. This situation must continue.

At the same time, in view of the large amount of work and multiple links in cold chain food disinfection and testing, it is also necessary to improve the current testing efficiency and prevention and control safety from the perspective of upgrading technology. For example, some terminal managers and workers suggested that relevant departments should organize scientific and technological personnel to jointly tackle key problems, upgrade cold chain food handling equipment, and minimize personnel exposure risks; at the same time, promote the use of ultraviolet lamps and irradiate frozen products without unpacking. Maximize the use of modern technology to improve the level of epidemic prevention and control. These suggestions from the front line of prevention and control may be worthy of reference.

What needs to be clarified is that, in combination with the current probability of cold chain food epidemic risks and the reality that the global epidemic situation is still unclear, the epidemic prevention and control of imported cold chain food can no longer be regarded as a “temporary task”, but must Bring it into the normalized prevention and control system without compromise, and have a longer-term and comprehensive consideration of specific prevention and control measures and even resource input.

For example, to limit the risk spread of imported cold chain foods to the greatest extent, we cannot stop at the import link disinfection and testing. Follow-up traceability work is equally important. In this regard, many places have actively responded. For example, starting this month, the Beijing cold chain food traceability platform will be officially used, and imported frozen meat and aquatic products must upload traceability data. In addition, Shanghai, Zhejiang, Sichuan and other places have also launched corresponding cold chain food traceability platforms. In the next step, this traceability system should become a standard configuration in various places, and all links such as transportation and market operations should be included in the traceability system.

Compared with “person prevention”, the prevention and control of “things” including imported cold-chain food may cause a relaxation of consciousness, and it belongs to prevention and control within a certain system. It is also in terms of transparency and social perception. Often less than some conventional epidemic prevention measures. This will inevitably require more targeted supervision and follow-up in the implementation of corresponding epidemic prevention measures.

Currently, disciplinary inspection and supervision agencies in many places have carried out corresponding supervision actions, "strengthening supervision with strong supervision, urging relevant departments to check at all levels, and compacting supervision responsibilities", which has set a positive example. All localities should strengthen relevant supervision and guarantee to ensure that the implementation of prevention and control measures will not drop the chain.

From the initial “prevention of people” to the current “prevention of people”, it is necessary to “prevent things”, and once again remind the whole society not to underestimate the complexity, uncertainty, and unknownness of the spread of the epidemic. It must be normalized in prevention and control. You must be prepared for a protracted battle, put an end to fluke, and don't miss any risk points in any link. At the same time, in response to repeated exposure to the risk of cold chain food epidemics, it is necessary to strengthen the corresponding social prevention and control science to avoid overreaction by ordinary consumers and market entities, and to minimize negligence and loopholes. In short, imported cold chain epidemic prevention must be vigorous and truly solidify the barriers to epidemic prevention and control.
