    machinetranslated

It is not clear what we are talking about, could be a seasonal strain or may be swine-origin triple reassortant strain? Seems something to watch.

In Mongolia recorded unknown influenza virus

New, not experts studied the flu virus, found in Mongolia. For current flu symptoms similar to the so-called "avian" and "pig" influenza viruses. The virus is classified as H3N2.

In Ulan Bator have registered 17 cases of the disease. The main danger of young children suffer.

As the channel "Vesti" , head of public relations, National Research Center for Infectious Diseases Mongolia Chuluunbat Urtnasan explained that doctors currently do not know how to protect yourself from the virus H3N2.

He said that primary attention be given to protecting the body from cold. It weakened to a cold body prone to a new viral infection.

Singular Urtnasan recommended for the very first symptoms of infection immediately contact a doctor for help. He recommends at work and at home more often ventilated room and do a wet cleaning.

Re: Mongolia recorded "unknown" influenza virus

I'm very confused. H3N2 is likely the seasonal strain (Brisbane, Perth, or whatever its current drift variant is named). Why are they referring to it as an unknown flu?

I don't think the lab facilities are there in Mongolia to detect the triple reassortment strain that was found in two US cases (and the article indicates that 17 people have tested positive, which is quite a lot), so this H3N2 is almost certainly seasonal.

Re: Mongolia recorded "unknown" influenza virus

They have a national research centre for infectious diseases - which is not unreasonable, when you consider that a biolevel 3 or more lab would want to be housed in an area of low population in an ideal world. Where better than Mongolia?

Sounds like a swine/human reassortant H3N2 to me - however, whether it is sufficiently transmissible and competitve to become established and a wider problem to humanity is another matter.

I have moved this thread to our trH3N2 forum so that we can watch this situation closely. If we determine that it is a human flu, or other novel flu situation, I will move this thread to the appropriate forum.

          Thank you everyone.

Re: Mongolia recorded "unknown" influenza virus

At this stage, as showed in the web site listed above (http://www.flutrackers.com/forum/sho...96&postcount=2 ), nothing is pointing toward a novel influenza A strain.

Instead, the seasonal human A/H3N2 strain is starting the winter circulation in Mongolia as happening in other part of norther hemisphere regions such as Europe and norther America.

The reason for the alarm may be the increased ILI incidence among children and the possible low or absent seasonal trivalent vaccine coverage among these individuals.

                Sure.

This is a large forum. Placing this thread here gives this subject more visibility. We can always move it after we track the situation for a few days.

We are not making a definite statement whether this event is seasonal or novel.

Novel influenza viruses are what the site is about and we will following this subject intensely.

Russian doctors will travel to Mongolia to explore a new kind of flu

                    December 3, 2010,

Russian doctors interested in finding in Mongolia novel influenza virus, which, according to some, like swine and avian influenza.

"No panic. But we're interested in, because it is close to our borders "- said the head of Rospotrebnadzor, chief state sanitary doctor Gennady Onishchenko.

"Our Mongolian colleagues say they have discovered the flu virus H3N2. Through the Foreign Ministry, we have requested access to clinical material, which is in Mongolia ", - said Onishchenko.

According to him, the specialists of the Russian NGO "Vector" - the regional center for the study of influenza - are going to explore a new flu, which is found in Mongolia.

"They will go to Mongolia, if we agree, and if there will be something to investigate. It is unclear what kind of flu. The clinic, he is small, unlike normal influenza. More research is needed "- said the head of Rospotrebnadzor.

In Mongolia, we found a new, unexplored by experts virus, the symptoms similar to the so-called avian and swine influenza.

"It was classified as H3N2. In Ulan Bator today registered 17 cases. It is also known that the main risk exposed young children "- delivered on Thursday channel" Stork-TV, citing the National Research Center for Infectious Diseases in Mongolia.

"Now we do not know the specific methods of protection from the virus H3N2. We believe that the main thing - it's to protect themselves from cold. It is manifested through her new infection "- said the representative of the Mongolian Center, reports "Interfax"

Российские медики заинтересовались обнаруженным в Монголии новым вирусом гриппа, который, по некоторым данным, похож на свиной и птичий грипп.

Re: Mongolia recorded "unknown" influenza virus

This story continues to be puzzling.

As posted above, WHO FluNet received notification of nine isolation of H3N2 human seasonal influenza viruses from Mongolia in week 45.

Latest WHO influenza bulletin stated as:

''Influenza A(H3N2): Algeria, Australia, Bolivia, Brazil, Cambodia, Cameroon, Canada, Chile, China, China Hong Kong SAR, Costa Rica, Cuba, Fiji, French Guiana Ghana, Ireland, Japan, Kenya, Madagascar, Mexico, Mongolia, Norway, Republic of Korea, Singapore, South Africa, United Kingdom and United States of America.''

                      (11/22/10, http://www.who.int/csr/disease/influ.../en/index.html )

The continuing indication of a 'novel H3N2 virus' represents an intriguing development and it is not clear if the reason is only for public health concern.

If a trH3N2 virus of swine origin, isolated in USA - Wisconsin and Pennsylvania in three, unconnected instances - would have already traveled to Mongolia... when the world would be well ahead in the global diffusion of this novel pathogen...

A new influenza strain would have circled around the world in six weeks - as seen for H1N1 (2009) - and this putative pathogen would have been already detected elsewhere in the world, ie Europe, Australia, Canada, Brazil.

Until further details will be known, it is prudent to call for more information.

                        Non-dangerous influenza

Moscow. December 3. INTERFAX.RU - Mongolia has discovered the flu virus, detected almost a month ago in the U.S.. H3N2 strain of symptoms similar to the so-called "bird" and "swine flu, but not yet sufficiently well understood by experts.

"In Ulan Bator today registered 17 cases. We also know that the main hazard prone young children," - delivered on Thursday afternoon television Stork-TV Ch words Urtnasana, Head of Public Relations, National Research Center for Infectious Diseases Mongolia. "We do not know the specific methods of protection from the virus H3N2. We believe that the main thing - it's to protect themselves from cold. It was through her shows new infection," - said Urtnasan.

Head of Rospotrebnadzor Gennady Onishchenko, urging people not to panic. "No panic. However, we are interested, because it is close to our borders," - he told Interfax. "Through the MFA, we have requested access to clinical material, which is in Mongolia," - said Onishchenko. According to him, the specialists of the Russian NGO "Vector" - the regional center for the study of influenza - are going to explore a new flu, which is found in Mongolia.

"They will go to Mongolia, if we agree, and if there will be something to investigate. It is unclear what kind of flu. At the clinic, he is little different from ordinary flu. Looking for more research," - said the head of Rospotrebnadzor.

In early November in the U.S., the virus was detected in two people - a 7-mesyanogo child in Illinois and 46-year-old man in Pennsylvania. Then head Rospotrebnadzor reported that this virus is a triple-reassortant - is at once the three viruses that have previously circulated. "It consists of three pieces of the former viruses. But it is different in genetic structure. We are studying this information," - Onishchenko said, adding that Russian doctors asked the American specialists about the new strain.

Their study showed that the new virus should not lead to a global epidemic. "The scientific outlook for today: is not expected any of epidemics and pandemics," - said to Interfax on November the chief physician of Russia Alexander Chuchalin. "To date, no evidence that we will meet with an unusual new respiratory tract infections, there is no" - he said.

"From the standpoint of genetics is the drift of the virus. There is a mixture of virus genomes, but the science noted that in this case, it happens in the human body. There is no evidence that this new reassortsiya lead to the emergence of highly virulent strains," - said Chuchalin.

Onishchenko reported earlier that season of influenza and acute respiratory disease has already begun: the excess of the epidemic threshold incidence of influenza detected in four regions of Russia - Kostroma (in excess of 13%), Ryazan (11%), Vologda (25%) and Sverdlovsk (27%) regions as well as in Ivanov, Novgorod, Irkutsk, Yakutsk, Magadan and Blagoveshchensk.

According to him, in the Komi Republic, Chelyabinsk, Amur and Murmansk regions the incidence of influenza approached the level of epidporoga, but in Moscow and St. Petersburg there is no epidemic. Head of Rospotrebnadzor said that Russia circulating seasonal influenza viruses, swine flu cases - single.

On Thursday, December 2 Rospotrebnadzor reported that a week from 22 to 28 November, the incidence of influenza and SARS in Moscow grew by 10% compared with the previous week. "Only sick 58,4 thousand people, of whom nearly two thirds - the children," - told Interfax a source in the department. According to him, with a diagnosis of SARS "and" flu "last week 994 people were hospitalized, and the total proportion of influenza in the structure of ARI was 0.02%, the incidence of SARS and influenza in the lower than estimated the epidemic threshold of 25,4%.

Meanwhile, on Friday Onishchenko told Interfax that up to New Year's epidemic of any kind of influenza in Russia is unlikely. "While the reasons for such forecasts do not" - he said.

On the situation with the flu in Russia, including Moscow and St. Petersburg, he said that "it is doepidemicheskom level. On the whole, Russia, the situation is satisfactory."

This fall in Russia seasonal flu shots made almost 32 million people. Most vaccines made from the federal budget, especially the so-called vulnerable groups - children, pensioners and social workers. About 3 million vaccinations done at the expense of corporate and regional budgets. "It remains to be made less than a million doses. We may assume that vaccination is completed. Planned threshold is reached," - said Onishchenko.

Previous flu epidemic, including swine, took place in Russia in autumn 2009. The incidence rate of influenza has declined at the end of December 2009. According to the head of Rospotrebnadzor, "swine flu" in a mild form of chickenpox in 2009, about 6 million Russians.

At the end of March 2010 in the Security Council of Russia, "Interfax reported that in October-December 2009 during an epidemic of trouble" swine flu "ill with about 4,6% of the population of Russia.

Re: Mongolia recorded "unknown" influenza virus

                          Re trH3N2

Firstly it would be very strange if at least some of the early media reporting was not just plain wrong, that is just the norm with this kind of story.

What we do seem to know is that there have been three documented cases this year, in the U.S, described in the CDC’s MMWRs as ‘novel H3N2’. They have been spread out geographically and temporally and – as far as I know – the sequences have not been released into the public domain. This is perfectly normal and to be expected. We are seeing more cases because we are looking harder and have better and more widely distributed tools so to do.

The ‘triple reassortment’ description is not in the MMWR but starts appearing in the Russian press from a WHO circulated US report. Week 44 fluview gives a little more data

                          Novel Influenza A Virus:

Two cases of human infection with a novel influenza A virus were reported: one case by the Wisconsin Department of Health Services and one case by the Pennsylvania Department of Health. Both patients were infected with a swine origin influenza A (H3N2) virus similar to the three other swine origin influenza A (H3N2) viruses previously identified in 2009 and 2010. The Wisconsin case reported contact with pigs in the week preceding symptom onset on September 8, 2010 and required hospitalization. No contact with pigs has been identified in the Pennsylvania case in the week before symptom onset on October 24, 2010; however the case lives in an area close to pig farms. Both patients have fully recovered from their illness. The cases are not related and the viruses from these two cases have some genetic differences, indicating that they did not come from the same source.
Although both investigations are ongoing, there is no evidence of human-to-human transmission with this virus in either case. Early identification and investigation of human infections with novel influenza A viruses is critical to evaluate the extent of the outbreak and possible human-to-human transmission. Surveillance for human infections with novel influenza A viruses continues year round.
For additional information on these cases and swine influenza, see http://www.cdc.gov/media/subtopic/heard.htm#h3n2.
But it is the link at the end to the information for the press in 'have you heard' that we get the first official reference to triple reassortment

Test samples from two patients submitted by Wisconsin and Pennsylvania have been confirmed at CDC as positive for swine origin triple-reassortant (tr) H3N2 influenza viruses—viruses that normally infect pigs.
So it would appear the ‘tr’ relates to the triple reassortment that led to the currently circulating swine H3N2 and is not a recent event that has caused a new pandemic candidate (in humans).

To update the story there is another novel H3N2 reported in MMWR week 46 (but none this week) and then there are the Mongolian news reports.

I think the above history makes it fairly clear that the US cases are not seeding a Mongolian pandemic and that the two stories are basically unrelated. I doubt there is anything very interesting happening in Mongolia but that will become clear once we have sequence data. Why these sequences are not just dumped in the public domain immediately is an ongoing scandal and would quash most wild rumours dead in their tracks.

All the above is gleaned from links provided by other posters in the linked threads here at FluTrackers. I am sorry not to thank you all individually but what goes on here is a team effort, you know who you are and please don’t think freeloaders like me fail to appreciate the time and effort.

It remains at least disconcerting the fact that two of the main Russian media outlets (Interfax, a semi-official press agency - and Kommerzant, a quasi-independent daily news) did spread information about a triple reassortant swine origin H3N2 virus originated in US.

This could be part of a disinformatjia activity or real factual accounts. To readers the quest for an answer.

Came from Mongolia is not very pleasant news: the country extends a new strain of influenza A/H3N2. Cases are still few - only 17 people, but experts are worried: influenza unknown, and who knows what he threatens. In the neighboring country to study contagion, which has no name, ready to fly out experts from the NGO "Vector". This step is necessary because the new flu threatens Russia from the East - above all, he can get through the Irkutsk region, Trans-Baikal and Buryatia.

                              - When detected a new strain of a flu virus, experts always take off on a place to learn it - says a spokesperson for Rospotrebnadzor Russia Lyubov Voropayeva. - Fear is not worth it, but to pay attention to their health need. In Irkutsk celebrated the growth of SARS, record and flu in a few cases, however, this situation is normal for the seasonal rise in incidence. The more so because of the monitoring data is known: dominated by parainfluenza viruses (11.85% of the total number of surveyed patients), adenovirus (8.7%). Seasonal viruses, as well as a pandemic virus fix infrequently - to 3,9% of cases.

Last week's Irkutsk doctors counted at kids 5 cases of the disease. Establish exactly how the virus infected children has yet to be, but doctors believe - probably the most common.

                              - In Siberia and the Far East began to spread, according to our data, the virus H3N2, - says the scientific secretary of the St. Petersburg Institute of Influenza Tamara Lobova. - However, this virus is unchanged. Cases of influenza B detected on the Volga River and the north-west. Throughout the country, the situation in non-epidemic influenza. There is growing only by SARS.

In early December, came down Ekaterinburg: there epidporog disease was exceeded once at 40%. Now the Urals is still ill, and Irkutsk only pulled to the threshold of the epidemic.

                              - Over the past week with signs of SARS in health clinics Irkutsk asked 5,590 people - the press-secretary of the Office of Rospotrebnadzor Irkutsk region Svetlana Kaurova. - Epidporog exceeded by 18%, primarily due to illness among the adult population.
