the Deputy Chief for Crisis Management, Oishi informed the governor at past noon on 6th Nov

The infected persons were not isolated well
There's an irregularity in prevention's manual
the beginning of the infection spread
27 Oct., the first infected(1 inpatient and 3 staffs)
1 inpatient isolated, 3 staffs cured at home
other 8 persons were in the same room
but the mental hospital did not examine this 8 persons thoroughly
8 persons moved to other rooms, 6 of 8 persons finally infected
the infection control of Takanosu
doctor in hospital: 1 doctor/daytime 1 doctor/nighttime(info: 133 inpatients)
a room used for dining, a room used for group therapy: these use was not limited
complete entry limitation: the 1st of November
explanation of infection to inpatient's family: only when asked by family
Akita prefecture directed the improvement for prevention
