      • Wakaba-ku 1, Chiba city, Chiba:
        • On 12 March 2011, poultry was tested positive for influenza A with rapid test at a farm in Wakaba-ku.
        • Virus was identified as H5 with RT-PCR on 13 March and as H5N1 on 16 March.
        • Mandatory movement restriction has been enforced within 10 km around the affected farm since 12 March 2011.
        • All of the poultry at the farm was destroyed by 15 March.
      • Wakaba-ku 2, Chiba city, Chiba:
        • On 16 March 2011, poultry was tested positive for influenza A (H5) with RT-PCR.
        • Virus was identified as H5N1 on 19 March.
        • Mandatory movement restriction has been enforced within 10 km around the affected farm since 16 March 2011.
        • All of the poultry at the farm was destroyed by 21 March.
      • Mandatory movement restrictions were lifted on 10 March in Hiyoshi (follow-up report No. 9), on 11 March in Kitaura-machi (follow-up report No. 9), on 14 March in Kishigawa-cho (follow-up report No. 9) and on 15 March in Kiho-cho (follow-up report No. 9).
