    New malaria cases continued to be reported during the past week, including from Boloso Bombe and Boloso Sore woredas of Wolayita zone (SNNPR), Ankasha woreda of Awi zone and Mecha woreda of West Gojam zone (Amhara), and South West Shewa, Illua Aba Bora and Jimma zones (Oromia). However, the number of cases reported showed a decreasing trend in all three regions for the first time since malaria cases started to be reported four weeks ago. The joint Ministry of Health and health partners team, tasked to start preparing a malaria mitigation and preparedness plan and identifying high risk woredas in the country, has identified 151 high risk woredas in Amhara, Beneshangul-Gumuz, Oromia, SNNPR and Tigray. Of the 151 woredas, 63 were classified as hyper hotspot woredas and given highest priority for mitigation and prevention efforts. Preventive and control measures continue, including house-to-house case search and treatment, focal spraying, distribution of insecticide treated nets (ITNs) and mass mobilization for environmental management.       Following intensive malaria prevention and control measures underaken by woreda health offices, the number of new malaria cases reported in the past week has declined in some areas, including in Boloso Bombe and Boloso Sore woredas of Wolayita zone (SNNPR), and Ankasha woreda of Awi zone and Mecha woreda of West Gojam zone (Amhara). However, increasing incidence of malaria has been reported in North Gonder zone (Amhara), Wondo Genet town, Dalla, Aleta Chuko and Dara woredas of Sidama zone (SNNPR) and from seven malaria hotspot zones in Oromia Region, with the highest number of reports coming from East Shewa and Jimma zones.
      New measles cases were reported during the past week from Gurage and Gedeo zones of SNNPR. In Oromia, the Regional Health Bureau, with support from WHO, has started preparations to conduct a measles vaccination campaign beginning on 2 August and targeting children in 36 woredas of seven zones in the region.
      No new cases of meningitis were reported during the past month from areas of SNNPR affected by the recent outbreak. As a result, WHO has deployed a technical team to conduct a post-epidemic evaluation in the region and confirm the end of the outbreak. The team will also conduct a meningitis outbreak risk assessment in selected hotspot woredas in Amhara, Beneshangul Gumuz and Oromia Regions. For more information, contact: who-wro@et.efro.who.int

        Localized outbreaks of malaria have been reported in parts of Wolayita and Sidamo zones of SNNPR in the past week, while increasing malaria caseloads have been observed in Southwest, West Shewa and Wellega zone of Oromia. Prevention and control measures are on-going in priority areas, including distribution of ITNs, health education on ITN utilization, environment management (through community mobilization), permanent and temporary mosquito breeding site identification, data analysis, early detection and referral of malaria cases to health centres. In SNNPR, 60 woredas have been identified as hot spots and weekly case monitoring has been initiated after development and communication of plans on vector control to the Regional Health Bureau and woredas health offices. In Oromia, training of health workers on management of malaria outbreaks is planned to begin next week in collaboration with WHO, while training of staff in the 63 hyper hot spot woredas in six regions is also planned to strengthen disease surveillance. Amhara region is conducting training on outbreak management in East Gojjam zone, having distributed indoor residual spraying chemicals to all 55 hot spot woredas. Overall, interventions by woreda and regional health officials and partners have resulted in declining cases of malaria in most parts of the country. Also during the past week, 27 confirmed and 5 suspected cases of measles were reported in Ditta and Tocha woredas of Gamo Gofa zone, SNNPR.           Reports confirm 40,229 positive cases of malaria (including 1 death at Case Fatality Rate/CFR of 0.0025 per cent) at the
          national level in the previous week. Overall, a total of 1,236,778 malaria positive cases and 160 deaths (CFR=0.013) were
          reported across all regions of Ethiopia since January 2012. Three regions made up the 88.7 per cent of the national case
          load, including Amhara (29.2 per cent), Oromia (12.6 per cent) and SNNP (46.9 per cent). A peak case load was reported
          in June but the trend declined in consecutive months. The most affected areas in the three regions are West Gojjam, South
          and North Gondar, Awi (Amhara); West and South West Shewa, Jimma, east and west Wollega (Oromia) and Wolaita,
          Gamogofa, Sidama, Kembata Tembaro and Hadia (SNNPR). To supplement ongoing prevention and control measures,
          three teams of experts from the Public Health Emergency Management/ Ethiopian Health and Nutrition Research Institute
          (PHEM/EHNRI) and the Federal Ministry of Health (FMOH) are in the Amhara, SNNP, and Oromia regions. The teams
          reactivated disease surveillance and monitor data at woreda level. Weekly situational monitoring meetings continue at the
          Federal Ministry of Health.
          The EHNRI laboratory received a total of 312 blood samples suspected to be carrying the measles virus from all regions of
          Ethiopia in the past four weeks. Amhara and Oromia region suspected cases amount to 64 per cent of the national case

            At the national level, a total of 53,550 cases of malaria were reported along with 10 confirmed deaths (Case Fatality Rate of 0.01 per cent) during the previous week. Some 95 per cent of the cases occurred in three regions: SNNPR (47 per cent), Amhara (26 per cent); and Oromia (22 per cent), although certain localized areas of the Oromia Region such as West Shoa, Arsi and Horoguduru zone continue to report increasing case loads compared to previous weeks.

            The SNNP Regional Health Bureau/Public Health Emergency Management (RHB/PHEM), Sidama WHO conducted training (27-31 August 2012) on early warning, surveillance and management of communicable disease outbreak focusing on Acute Watery Diarrhoea (AWD) and malaria as part of preventive and control measures implemented in the region. The training targeted the Sidama Zone Integrated Disease Surveillance and Response (IDSR/PHEM) focal persons at Yirgalem Town collaboration with WHO. A total of 131 focal persons from all the woreda health offices and all health institutions (hospitals and health centers) in Sidama Zone took part in the training.

            Since July 2012, the Somali Region has reported cases of diarrhoea with peaks recorded in the first week of August 2012. Numbers of new cases have declined in the last three weeks. The outbreak centered on Dollo Ado woreda, Liben Zone. The 292 cases reported since the onset of the outbreak; children under the age of five constitute the majority. According to UNHCR, during the first two weeks of August, there was an increase in diarrhoea related cases in Melkadida and Buramino camps. The combined WASH and health sector response by carried out by UNHCR, the Administration for Refugee and Returnee Affairs (ARRA) and partners centered on investigation, coordination, improved water provision, sanitation and hygiene promotion, as well as coordinated sharing of available supplies. No new health center admissions occurred and Melkadida Camp particularly has registered remarkable progress. Overall, diarrhea surveillance remains a priority in all camps.

            The Oromia Regional Health Bureau, Zonal Health Department (ZHD) and WHO have conducted an assessment in Arero and Moyale woredas (Oromia Region) where Internally Displaced People (IDP) are located in the first week of September. The team used the findings to prepare a proposal for the rapid initiation of health service provision in three health posts in a location where functioning health posts do not exist but people continue to return.

            Epidemic preparedness activities have begun with the establishment of a temporary clinic and a Voluntary Counseling and Testing (VCT) center in Borena Zone, Oromia Region where thousands of people are expected to gather for a ceremony. Deployed health workers will work in the clinic; supervise hygiene conditions (bars and restaurants); provide information on TB, HIV/AIDS, malaria, diarrhoea and other communicable diseases and also distribute condoms. The clinic is equipped (drugs and medical supplies) and is giving routine services to the community. Assigned community health informants will detect and report outbreaks in advance. Surveillance is strengthened and the Oromia Health Bureau allocated budgets (transferred from WHO) in addition to distributing water purifying chemicals.               During the reporting week, 55,143 new cases of malaria were reported countrywide, with 43 per cent (23,746 cases) reported from SNNPR, 40 per cent (22,189 cases) from Amhara, and nearly 17 per cent (9,130 cases) from Oromia. No new cases were reported from Afar, Gambella or Tigray Regions. The national figure continues to show a slight decline in weekly cases. The SNNP Regional Health Bureau and WHO continue to support malaria prevention and control activities in 60 hot spot woredas, as this is the peak season for malaria transmission. Meanwhile, an outbreak of diarrhoea in Gode woreda, Shebelle zone (Somali Region) was confirmed as normal diarrhoea, not acute watery diarrhoea (AWD).                 An increasing number of malaria cases - 81,683 in the reporting week ending 4 October ? were reported nationally, with the majority from SNNPR (39 per cent, Amhara (37.5 per cent) and Oromia (18.3 per cent). The most affected zones include Sidama, Wolayita, Halaba and Gamo Gofa (SNNPR); west Gojjam, North Gonder, Awi and South Gonder (Amhara); and Jimma, East Wellega, West Shewa and Ilu Aba Bora (Oromia). The Ministry of Health (MOH), Regional Health Bureaus and partners are conducting active surveillance in hot-spot woredas and monitoring disease trends. In Oromia, a new monitoring assessment conducted by the RHB and WHO in West Shewa zone revealed ongoing preparedness activities, including: distribution of malaria guidelines to woredas and health facilities; completion of indoor residual spraying in 75 per cent of highly malaria-endemic woredas; and distribution of 17,000 insecticide-treated bed-nets.
                There were 36 new suspected measles cases during the week in Oromia, Amhara and SNNPR, and 18 new cases of meningitis in Amhara, Dire Dawa, SNNPR and Tigray. Given the upcoming meningitis transmission season, WHO has provided funding to initiate enhanced disease surveillance and training on case management and outbreak investigation for health workers in Amhara, Beneshangul Gumuz, Gambella, Oromia, SNNPR and Tigray.                   WHO reported a continued increase in the number of malaria cases in Amhara, Oromia, SNNPR and Somali regions during the week ending 12 October. In Oromia, 26 woredas across Arsi, West Arsi, Borena, East, West and South-West Shewa, Horo Gudru, Jimma, and West Wellega are among the areas most affected. With the major malaria transmission season normally occurring between September and December, the Regional Health Bureaus (RHBs) in all areas are implementing malaria outbreak prevention activities and monitoring gaps to enhance control measures. A total of 55, 60 and 64 hyper hotspot woredas have been identified for enhanced surveillance and prevention activities in Amhara, SNNP and Oromia regions respectively. UNICEF has imported 1.4 million doses of anti-malaria medicine (Coartem) for distribution to the regions as required. WHO has provided emergency health kits to support communicable disease case management.

                  Also during the reporting week, Amhara, Oromia and Somali regions reported new suspected measles cases, which are under investigation. Meanwhile, WHO will support two sub-national immunization campaigns against polio planned for 19 to 22 October and 16 to 19 November and targeting 2.4 million children under age 5 in 22 zones around the periphery of the country. Zones to be covered include Metekal and North Gondar (Amhara); Asossa, Kemashi and Tonga (Beneshangul Gumuz); Agnewak, Gambella, Mejenger and Nuer (Gambella); Bench Maji and South Omo (SNNPR); Bale, Borena and East Hararge (Oromia); and Afder, Doolo, Fafan, Liben, Jarar, Korahe, Nogob, Shabelle and Shinile (Somali).                     Increasing cases of malaria have been reported from Oromia, SNNPR, Amhara and Tigray. The record of compiled surveillance data for the first week of October showed a total of 82,398 cases with 6 deaths from the four regions, compared to 81,683 cases during the last week of September. SNNP and Amhara regions reported close to 68 per cent of the cases (each reported close to 34 per cent of the cases), while Oromia reported 17 per cent of the cases, and Tigray 8 per cent of the cases.
                    Surveillance reports showed that the malaria case load peaked in June (with over 400,000 cases) and declined markedly in the month of July (with close to 220,000 cases), but has been increasing continuously since, with 260,271 cases in August and 394,650 in September.
                    Preventive and control activities including case management, ITN distribution and IRS are still being implemented by the Ministry of Health (MoH) and Regional Health Bureaus (RHB) in collaboration with partners. Accordingly, WHO supported field level investigations in Dawro zone of Tocha woreda (SNNPR) in addition to malaria epidemic preparedness and response activity in East and West Wellega, Kellem Wellega and Jimma zones of Oromia region. Following the field investigations, health workers obtained support for orientations on malaria case management protocol as per the new guidelines and malaria situation monitoring using the monitoring chart support.
                    During the first week of October a total of 44 suspected measles cases and one death were reported from Amhara, Oromia and SNNPR with majority of cases reported from Amhara, while during the previous week SNNP reported 38 cases from Bench Maji, Meint Shsha, Shako and Gedeo zones.                         160 hyper hot-spot woredas previously identified. Increased attention is also being given to areas in Beneshangul
                        Gumuz and Tigray Regions, where the majority of malaria cases are reported among day labourers. Preventive and
                        control activities including case management, distribution of insecticide-treated nets and indoor residual spraying
                        continue to be implemented by the MOH and Regional Health Bureaus, with support from health partners. WHO is
                        supporting resource mobilization efforts and providing technical assistance for assessments in parts of Amhara and
                        Oromia Regions, which are facing critical gaps in available supplies and funding.

                        In the past week, 43 measles cases were reported from Bench Maji zone of SNNPR?blood samples have been sent
                        for confirmation. The zone is enhancing disease surveillance while providing routine immunization and case
                        management. Gedeo zone authorities have also conducted a measles vaccination and Vitamin A supplementation
                        campaign in Gedeb Town, reaching 173 and 185 school children respectively. The zone also plans to conduct mass

                            Some 149 new cases of measles were reported from six regions and Addis Ababa city during the week ending 16 December 2012, of which 70 per cent were from Oromia Region (Arsi, Bale, East Harerge and East Shewa zones), while 18 cases of meningococcal meningitis were reported from three zones/regions (North Gonder zone in Amhara, Kembata Tembaro in SNNPR and Jijiga town in Somali). Close monitoring and case management is ongoing: as part of prevention and control measures, WHO supported training for health workers in Amhara, Oromia and SNNPR on outbreak surveillance and case management. Also during the same week, 94,601 new malaria cases were reported from Amhara, Oromia, SNNP and Tigray. The data indicates that while cases in Amhara, SNNP and Oromia ? areas that have consistently reported the majority of malaria cases since January 2012 - have been gradually decreasing over the past month, an increasing number of case have been reported in Tigray. A new malaria outbreak was confirmed in Abobo woreda (Agnuwak zone) of Gambella, with 738 cases reported.
