  • Seven babies die in Annaba in several months after being infected by a virus

Hat-tip Pathfinder. This looks really bad. The translator is a bit rough, but I believe this article says that a baby from Tebessa was brought to the hospital in Annaba, and died of a flu-like illness, and now in the past 48 hours, seven babies have died of this virus, and a couple more seem infected.

My only question here is: Is anyone else (besides the babies) such as HCW or the mothers ill? A contagious deadly virus (like H5N1) would affect everyone, not only the babies. Hospital acquired bacteremia outbreaks can result in lots of dead babies quickly, but those don't typically invovle respiratory or flu-like symptoms like this one does.

Sept bébés décèdent à Annaba après avoir été infectés par une grippe virale contagieuse Seven babies die in Annaba after being infected by a flu virus contagious
    Samir Rahim Samir Rahim
Seven babies died in the last forty-eight hours after being infected by an influenza virus to a hospital specializing in pediatric El Bouni in the wilaya of Annaba.
AI is that practitioners have diagnosed as severe bronchiolitis, was detected after the hospitalization of a baby from in Tébessa. Mainly concentrated in the pediatric EHS El Bouni covering the wilaya of Annaba, Guelma, Tarf, Souk Ahras and Tebessa, this contagious disease has reached all babies hospitalized and those reported by their mothers for ambulatory care.

The information, which spread rapidly, has upset the people of Annaba wilaya and neighbors. When contacted, M Mestouri Monji, Director of Health, Population and Hospital Reform in Annaba, while avoiding mention of an outbreak, confirmed that several babies were actually contaminated with acute bronchiolitis and 7 of have died.
« Toutes les dispositions ont été prises pour éviter que cette maladie contagieuse ne se propage à l'extérieur de l'enceinte du EHS pédiatrique infecté par le virus », at-il précisé. "All steps have been taken to prevent contagious disease from spreading outside the walls of EHS Pediatric infected by the virus," he said. Tuesday early evening, the health status of two other babies infected by the same virus aroused strong concern. Specialized teams in disinfection are hard at work in the EHS.

The article explicitly (in French) calls this a flu virus. This obviously isn't H1N1, as H1N1 doesn't have this high of a fatality rate. It is not clear if they are calling this flu because of the symptoms, or if someone has tested positive for flu.

    This is VERY scary

      Previous hospital-acquired bacteremia outbreaks:

      http://www.flutrackers.com/forum/sho...d.php?p=319797 - India

      http://www.flutrackers.com/forum/sho...d.php?p=303083 - Mexico

http://www.flutrackers.com/forum/sho...d.php?p=329025 - Egypt (the fist couple posts)

      http://www.flutrackers.com/forum/sho...d.php?t=139771 - Mongolia

It is possible that some respiratory symptoms could be shown in such an outbreak (the one in Mongolia was due to Klebsiella pneumoniae, which does infect the lungs), and such an outbreak would develop almost simultaneously as this has, but I wonder whether doctors could mistake such an illness for flu. In such a case, the imported baby from Tebessa would be a red herring. OTOH, I doubt if flu or some other contagious virus would only affect the babies. Also, keep in mind the timing - this all happened within two days, according to the article. Puzzling.

H1N1 could be possible, especially if algerian hospitals are limited in their facilities and availability of IV antivirals - this is an area where H1N1 has not hit before, and newborn's could be especially susceptible - especially if there are secondary bacterial infections with some nasty bacterial pathogens in the hospital.

It is in places like Africa that we are more likely to see how severe H1N1 might be in the basence of hi tech interventions - but then again, these hosptials could be better equipped that I give them credit for. anyone have any data on this?

Nasty bacteria in the hospital. That is exactly what would cause a hospital acquired bacteremia outbreak (and you wouldn't even need any H1N1 at all). I posted the links to several such outbreaks around the world in post #3 of this thread. While it is not clear exactly what causes such outbreaks (an FT member in Mexico commented on the Mexico thread that he believes it is due to contamination of IV fluids in the hospital), they do happen worldwide. To me, that might be the better diagnosis here, in which case the description of "contagious flu" would just be wrong. I hope...

If anyone at FT is good at sending articles to ProMED (I am not), this would be a good one to send.

Going through the ministry of health H1N1 updates (which stopped on Jan 7), there have been cases of H1N1 in the district with a fatality in Tebessa on Dec 11.

Suspicion of an outbreak of acute bronchiolitis in El Bouni

                    The Health Director explained

The Health Board of Annaba held to deny the maddening noise of the deaths between Sunday and Tuesday from 7 infants infected with bronchiolitis in the pediatric ward of a hospital specializing in El Bouni (wilaya of Annaba). The Director of Health wilaya of Annaba responded yesterday to reports that seven infants had died of acute bronchiolitis in 48 hours in a hospital specializing in pediatrics at El Bouni.

According to the official we contacted yesterday, deaths of infants and young children who acquired what had proved a form of acute bronchiolitis were actually registered at the hospital in question, but not not in the last forty eight hours, as reported.

Also according to the director of health, the poor record of infant deaths due to this type of viral infection has been established on the basis of statistics for a period of eight months, ie between 1 July 2009 and February 28, 2010. Do not hide his concern over this statement he described as "disappointing and regrettable", but precludes Mestouri Moundji the existence of an epidemic of flu or another and said that the deaths are caused by delayed treatment , young patients have been presented for admission at the pediatric department of EHS El Bouni critically ill.

"This facility serves five wilayas extreme eastern Algeria and sometimes babies from Souk Ahras, Guelma, Tebessa or El Tarf for outpatient treatment appear to be suffering from viral diseases and therefore eligible emergency. "He added:" Although properly treated immediately after their treatment by medical service, some of these patients, who generally come from disadvantaged backgrounds, unfortunately does not survive the disease because nothing could be done for them. But I repeat, there was no spread of bronchiolitis in El Bouni, there have been only isolated cases. "

Oh, that's very different. I didn't expect that. Seven deaths in eight months is a whole different story. Now H1N1 is possible, even likely (although it could just as easily be RSV or another seasonal pathogen).

There's really no reason to think it is all the same pathogen at that point.

Deaths of seven babies to EHS El Bouni (Annaba): "This is a seasonal flu, there is no need to worry," says DSP

                        17 March 2010 10:00:00 Souhila H.

In forty-eight hours, the hospital specializes mother-child (EHS) El Bouni, wilaya of Annaba, recorded the deaths of seven babies. Infection with seasonal influenza, that practitioners have diagnosed as bronchiolitis, has been fatal.

When contacted, the Director of Health, Population and Hospital Reform of Annaba, Monji Mestouri, confirmed the tragedy. "Many babies were actually contaminated by severe seasonal flu which was fatal for seven of them," confirms the DSP while ensuring that this is not a "pandemic". An accurate size because the EHS pediatric El Bouni covers the needs of populations whose boundary wilayas 9 Guelma, Tarf, Souk Ahras and Tebessa. "It's a bird known as the winter period," adds Monji Mestouri saying that all measures have been taken to prevent contagious disease from spreading in the establishment of a capacity of 140 beds. Thus, an operation of disinfection of the premises has been initiated. This is "systematically conducted every two to three months may be less depending on the epidemiological situation as such disinfection is also a fight against nosocomial infections," says Mr. Mestouri. Regarding reports of two infants with acute bronchiolitis by the Director of Health Annaba asserts that "there are no other children infected with the disease whose treatment is very simple." Regarding the availability of nursing staff to cope with this situation, "ordinary" for Mestouri, especially practitioners and specialist practitioners are on strike, the DSP said that no movement has affected the course of taking burden of hospitalized children. The drama of the EHS El Bouni recalls what happened to the pediatric ward of the hospital Nefissa-Hamoud (former Parnet) Algiers where seven infants died in one night in June of 2006, following a nosocomial infection.

The sentence in the translated text "It's a bird known as the winter period" is not well translated. If I translate it from the original text, this sentence means "it is the same kind of flu that we are seing in winter."

This obviously didn't happen in 48 hours. This article now puts it in a 15-day period, but notably includes hundreds of "milder" cases. Could be seasonal flu or H1N1. What is very odd is that the babies are all under 6 months old. I still wonder about a nosocomial bacteremia outbreak (maybe on top of an H1N1 outbreak) that would explain why only the extremely young are dying - H1N1 kills across all ages.
