    Between epidemiological weeks 1-15 of 2022, 56 case of plague have been reported with 2 deaths (3.6% CFR). The outbreak is currently focused in two sanitation areas Rassia and Lokpa where the latter makes up 95% of cases reported this year. During 2021, a total of 138 suspected cases including 14 deaths (CFR 10.1%) were reported. During 2020, a total of 461 suspected cases including 31 deaths (CFR 6.7%) were reported.

    A. Situation analysis

    Description of the disaster

    On 04 April 2022, the Provincial Health Division (DPS) of the province of Ituri declared a resurgence of the Bubonic Plague epidemic in the health zone of Rethy, in the territory of Djugu. According to the authorities, the history of this disease in the Rethy health zone goes back to more than 9 outbreaks, including this one. The eighth one dates back to January 2022 in the UKETHA health area. On 07/04/2022, the epidemiological situation covering 26 March to 4 April was as follows from DPS data: 31 notified cases of which only 15 have been tested, 2 deaths and nearly 2,000 contacts listed in the absence of local reactivity. The epicenter of the plague disease is in the Health Areas of Lokpa, Rassia, and Uketha, where the 3 confirmed cases are located. These three health areas are in the Rethy health zone, Djugu territory, in ITURI Province.

    Between 4 and 16 April 2022, 27 suspected cases in addition to the above were notified by the Lokpa Health Centre but in the absence of testing, the confirmation of cases remains very low and the level of vulnerability is underestimated. The absence of partners in the Rethy Health Zone apart from the local branch of the DRC Red Cross, MEDAIR, and MALTESER, as well as the lack of materials, greatly limits the response and the establishment of effective disease control and containment system.....

We ask all the population of the Zone to respect the measures to fight against this disease, namely: avoid sleeping on the ground, clean up their house, evacuate garbage, leftover food which can attract infected rats in the house, destroy the places of refuge of rodents "

        It is in this part of Ituri that the plague is commonly reported. These cases of plague were notified in the health areas of Lokpa and Rassia.

        Symptoms of bubonic plague include the appearance of buboes, fever and certain malaises. When there is a mutation towards the pulmonary form, the transmission is human-to-human, when someone touches it, it can also contaminate the person in front of it and it goes quickly, and it is this form which can be complicated with a septicaemic form (blood infection), and then death is imminent.

            A first case of pneumonic plague was recorded Thursday, June 2, in the health zone of Rethy, in the territory of Djugu (Ituri).

            This situation worries the local health authorities, because this case comes on top of the bubonic plague, which is not yet under control. Indeed, this entity is already facing the epidemic of bubonic plague triggered since March with 5 deaths out of the 217 positive cases notified.

            For the moment, the health zone is faced with problems of stock shortages of drugs for the care of patients.

            Apart from this need, explains this doctor, the medical profession lacks protective equipment as well as other needs.

            The head doctor of the Rethy health zone pleads for assistance in medicines and protective equipment, in order to cut the chain of propagation of these diseases.

                On 04 April 2022, the Provincial Health Division (DPS) of the province of Ituri declared a resurgence of the Bubonic Plague epidemic in the health zone of Rethy, in the territory of Djugu.
                From 27 Mar 2022 to 26 Jun 2022, 91 days of the epidemic, the health zone of Rethy, located in Djugu territory, Ituri Province, Democratic Republic of Congo (DRC), continues to report plague cases. A total of 351 suspected cases of plague have been reported, representing an attack rate of 0.14% with 5 deaths (case fatality rate 1.4%).

                So far, the most affected health area is Lokpa with 80.9% of cases, followed by Rassia 7.6% and Uketha 7.4%. The majority of people who have contracted the plague are in the age group >5 years (302 cases, 86%) compared with 13.9% (49 cases) among those <5 years, with an average age of 16 years. The bubonic form is dominant — 99.7% — with 0.3% of cases being the pulmonary form.

                The health zone is faced with problems of stock shortages of drugs for the care of patients.
