    U.N. scraps clinic contract as Ebola exposes Mali readiness gaps

    The U.N. mission in Mali, whose peacekeepers are helping to protect the nation against Islamist rebels, reversed on Saturday a decision taken earlier in the week to renew the contract with the Pasteur Clinic in Bamako to care for sick or injured troops.

    A U.N spokesman said the decision was taken "due to prevailing circumstances" but gave no further details.

    It followed the death in the clinic in late October of an elderly imam, or Muslim religious leader, from Guinea. The sick man was never tested, but his case directly led to a chain of confirmed deaths from Ebola, including a 25-year-old nurse who treated him and a woman who washed his dead body.

    The clinic, one of Bamako's best known and used by expatriates and the country's elite, denies any wrongdoing. It says it followed all its procedures for treating Ebola and that the imam never showed any signs of the fever.

    Ibrahima Fall, World Health Organisation representative in Mali, said the Pasteur Clinic - which has no connection to the Paris-based Institut Pasteur - made "a terrible mistake" by not alerting authorities to the case of the sick Guinean imam, who would have been showing Ebola-like symptoms.

    He said it was WHO officials tracing reported deaths in Guinea who had discovered this case in Mali.

        Ebola: more than 570 people under medical supervision in Mali

        Mali, last hit by Ebola, with four deaths in five cases registered in the country less than a month, said Monday it has put under health surveillance more than 570 people who have had contact with the virus.
        All these people are linked to a Guinean imam died October 25 in Bamako, the capital, and not the first case, a two year old girl also came from Guinea and died October 24 in Kayes (west), which has resulted in any contamination, said the Ministry of Health.

          ..Over the weekend volunteers and medical staff searched for more than 500 people, thought to have had contact with the Imam or the nurse. The number of contacts is likely to rise in the coming days, according to Sow.

          ?The first day we had less than 100 contacts," he said. "Then there were 180. It rose to 442 up to now when we have identified 552 contacts. I told my team this morning to not rest until they find all of them. By the end of this week I expect we?ll have identified over 1,000 people."

          The rising number of possible contacts is good news, said the doctor.
          ?The key to contain and fight Ebola is to find as many contacts as possible and then make sure they?re isolated and monitored,? said Sow..

            French Official: Key Week for Ebola Fight in Mali

            The French president's point man on Ebola says this is a "decisive week" for Mali in its efforts to block the spread of the virus.

            Jean-Francois Delfraissy says a handful of recent cases in the landlocked African country "could be the start of a new epidemic" even though the World Health Organization hasn't yet officially confirmed one in Mali.

              1. Mali said Monday it had placed nearly 600 people who may have been exposed to EVD under daily surveillance as the country battles to contain the spread of the virus. After the deaths of a Guinean imam and the Malian nurse who treated him in the capital Bamako, a friend who had visited the imam in the Pasteur clinic also died of probable EVD, while a doctor at the clinic who contracted the disease is undergoing treatment.               5. The EVD related death of an imam in Kerouane, Kankan region, Guinea, was followed by contamination of several of his family members through unsafe burial practices. This is especially worrying considering that there is no ETC or transit center in Kankan. 76 contacts are being followed up in the region by seven health workers equipped with motorbikes provided by UNICEF.

                Ebola-Mali: the affected person under "treatment"

                As of November 18, 2014, the situation is as follows in Mali: the number of contact persons followed by health services is 413. They are all under observation for health control, contact persons related to the case of Kayes are no longer under observation, because outbreak is over after a period of more than 21 days.                     A total of 6 cases (5 confirmed and 1 probable), including 5 deaths (4 confirmed, 1 probable), have now been reported in Mali (figure 1). The most recent cases are in the Malian capital Bamako, and are not related to the country?s first EVD case, who died in Kayes on 24 October. All identified contacts connected with the initial case have all now completed 21 day follow-up. As of 16 November 2014, a total of 384 contacts have been identified and are under follow-up; 96 of the 384 contacts are HCWs.

                      Mali: Details of the additional cases of Ebola virus disease

                      Ebola situation assessment - 20 November 2014


                      As of today, Mali has officially reported a cumulative total of 6 cases of Ebola virus disease, with 5 deaths. Of the 6 cases, 5 are laboratory confirmed and one remains probable as no samples were available for testing.

                      These numbers include the 2-year-old girl who initially imported the virus into Mali and died of the disease on 24 October.

                      Intensive tracing and monitoring of the child?s numerous contacts, including many who were monitored in hospital, failed to detect any additional cases. All 118 contacts, including family members, have now passed through the 21-day incubation period without developing symptoms.

                      The virus was almost certainly re-introduced into Mali by a 70-year-old Grand Imam from Guinea, who was admitted to Bamako?s Pasteur Clinic on 25 October and died on 27 October. He has been reclassified as a Guinea case, as he developed symptoms in that country. No samples were available for testing.

                      Pasteur Clinic: direct and indirect links


                      All 5 cases in this new outbreak are linked, 3 directly and 2 indirectly, to the patient in the Pasteur Clinic.

                      The first was a 25-year-old male nurse who worked at the clinic and was assigned to care for the Imam. He was hospitalized on 8 November. His case was laboratory-confirmed on 11 November and he died the same day.

                      The second case was confirmed in a doctor who worked at the clinic and treated the Imam. He developed symptoms on 5 November and was hospitalized on 8 November. Laboratory confirmation was received on 12 November. He is currently undergoing treatment.

                      The third case was a 51-year-old friend of the Imam who visited him at the Pasteur Clinic. He developed symptoms on 7 November and died on 10 November from an undiagnosed cause. He is the country?s single probable case.

                      The friend?s infection led to an additional 2 confirmed and fatal cases. The first was a 57-year-old woman who had direct contact with the friend. She developed symptoms on 29 October. She was admitted to another clinic on 11 November and then transferred to a hospital on 12 November. She died that same day.

                      The second case was the woman?s son. He visited yet another clinic on 5 November and died at home on 14 November. Ebola infection was laboratory-confirmed in both cases.

                      Stepped up contact tracing


                      A massive effort is currently under way to identify all potential chains of transmission, monitor contacts, and prevent the outbreak from growing larger. At present 338 contacts have been identified and 303 (90%) of these have been placed under daily surveillance.

                      This rigorous ?detective? work shows that the deceased nurse from the Pasteur Clinic had the largest number of contacts, at 98, including 75 family members.

                      As the successful experiences in Senegal and Nigeria show, aggressive contact tracing, which seeks to find and break every chain of transmission immediately after an imported case, can hold the number of additional cases to very small numbers and support a rapid end to the outbreak.

                      The Ministry of Health, with assistance from the WHO country office, has augmented the number of staff engaged in contact tracing by drawing on polio surveillance teams and using local medical students with training in epidemiology.

                      In addition, WHO has deployed 10 epidemiologists through its Global Outbreak Alert and Response Network, or GOARN.
                      The country is also ramping up its capacity to perform exit screening at the Bamako airport.

                      Guinea: Many mourners attended the funeral


                      The Imam was buried in his native village of Kour?mal?, Guinea, on 28 October. That event has now been investigated. Thousands of mourners may have attended the funeral. Some of them touched the body as part of the traditional funeral ceremony. About 300 contacts are being traced.

                      Experts in Mali and at WHO agree that Mali will remain at risk of further imported cases as long as transmission across the border is ongoing.

                          A doctor in Mali, who treated the patient that sparked a second wave of Ebola in the West African nation, has died, Samba Sow, who head's Mali's Ebola response, said on Thursday, making him the eighth person to die of the disease there.

Before Thursday's announcement, the World Health Organization was already reporting five Ebola deaths in Mali, while medical charity Medecins Sans Frontieres (MSF) put the figure at seven.

                            MSF said later on Thursday that is was evacuating home a Spanish member of its staff in Bamako after the worker was pricked by a needle that had been used on a confirmed Ebola patient earlier in the day.

                            "The member of the team is not showing symptoms of the disease and the repatriation is part of the protocols established by the medical organisation for security and preventive measures," MSF said in a statement.
