      • Mubende locals, leaders protest fresh lockdown

        A section of Mubende District leaders and residents have protested the government’s three-week extension of the Ebola-induced lockdown....

        ..The Resident District Commissioner and chairperson of the Ebola taskforce, Ms Rosemary Byabashaija, called for calm.

        “It is good that most of our people appreciate that the past lockdown is the reason for the reduction in the number of Ebola positive cases. As the district taskforce, we shall have to forward the many challenges raised to the high authorities,” she said.

        Ms Byabashaija said the district has registered only three positive cases in the last seven days.
        Government declared the first lockdown in Mubende and Kassanda on October 15 as part of the mitigation measures to contain the spread of the Ebola disease. Districts that are high risk areas are Mubende, Kassanda, Kampala, Wakiso, Kyegegwa, Kagadi, and Bunyangabu.

        Ms Phoebe Namulindwa, the Kassanda Resident District Commissioner, welcomed the new lockdown. “Our past record in handling the Ebola pandemic was not very good. Some of our leaders and a section of the residents failed to follow the guidelines,” she said.

        Recently, the Ministry of Health raised a red flag over Kassanda after it registered a surge in Ebola cases. Some of the cases were attributed to an incident where unidentified people exhumed remains of one of the Ebola victims at Kitongo Village in Kalwana Sub-county.

            Now, to the Ebola outbreak in Uganda.

            135 confirmed and 21 probable cases of Ebola have now been reported from eight different districts, with 53 confirmed and 21 probable deaths. So far, 62 patients have recovered.

            The government’s efforts to respond to the outbreak in the district of Mubende, where the outbreak began, appear to be paying off.

            However, in the past two weeks, the majority of cases have been reported from the capital Kampala, and the district of Kassanda.

            One case has also been reported from the district of Massaka, in the south of the country.

            The detection of cases in several different districts is clearly concerning.

            Our primary focus now is to strengthen contact tracing, community engagement, and infection prevention and control measures.

            • Ebola kills 8 children as infections rise in schools

              The Ministry of Education and Sports has revealed that eight children across the country have succumbed to the Ebola disease.

              Addressing journalists at the Uganda Media Centre yesterday, the State Minister of Primary Education, Ms Joyce Kaducu, said a total of 23 Ebola cases have since been confirmed, of which, eight died, five fully recovered while 16 are still under isolation.

              Of these cases, 11 are children from five schools in Kampala, Wakiso and Mubende, with one Primary Six learner who succumbed to the disease last month.

              Ms Kaducu said one child in baby class was in stable condition and recovered, while three children in Primary Two, Primary Four and Primary Seven are in stable condition and recovering.

              In Mubende District, one child, a Primary Five learner recovered and was discharged, while a Primary Four pupil is still hospitalised.

              The whereabouts of the five school -going children were not mentioned, but sources said they are still battling with the virus.

              Ebola forces govt to close schools on November 25

              The Ministry of Education and Sports has directed all pre-primary, primary and secondary schools across the country to close early for third term holidays.

              The Ebola outbreak has since claimed the lives of eight learners, forcing government to direct schools to close on November 25 instead of December 6.

              Addressing a press conference yesterday, the State Minister for Primary Education, Ms Joyce Kaducu, said: “We decided to shorten the term by two weeks to curb the spread of the virus among learners and staff.”

              • Description of the outbreak

                From when the outbreak was declared on 20 September until 7 November, a total of 136 confirmed cases and 53 confirmed deaths (CFR 38.9%) from Ebola disease caused by the Sudan ebolavirus (SUDV) have been reported, representing an increase of 18% and 66% respectively since the last DON published on 28 October 2022. In addition, 21 probable deaths have been also reported since the beginning of the outbreak, with the last probable death notified on 29 September. Three additional cases and three additional deaths have been reported among healthcare workers since 28 October, resulting in a total of 18 cases and seven deaths among these workers.

                Since the last DON, one newly affected district has been reported (Masaka), leading to a total of eight districts reporting cases. The most affected district remains Mubende with 63 (46%) confirmed cases and 29 (55%) confirmed deaths, followed by Kassanda with 46 (34%) confirmed cases and 19 (36%) confirmed deaths. Two districts, Bunyangabu and Kagadi, have not reported cases for more than 40 days.

                As of 7 November, a total of 1386 contacts in seven districts are currently under surveillance, with a follow-up rate of 92%. In the week commencing 31 October, an average of 1586 contacts were followed up daily, a decrease of 16% from the previous week (week commencing 24 October) when a daily average of 1896 contacts were followed up. A total of 34 contacts developed symptoms during the past week. Since the beginning of the outbreak, 3867 contacts have been registered, of which 2237 (68%) have completed the 21-day follow-up period.

                As of 7 November, at least 2835 alerts have been received at a daily average of 71 alerts. Approximately 94% (n=2671) of all alerts received were investigated within 24 hours, of which 1120 were validated as suspected cases. The proportion of alerts investigated within 24 hours has been steadily increasing, and in the week commencing 31 October, nearly all alerts (657/659) were investigated within 24 hours, of which 31% (n=203) were validated as suspected cases.

                Since the beginning of the outbreak, a total of 2139 samples were collected (suspects, repeat samples, swabs), of which 419 in the week commencing on 31 October (+11% increase as compared to the previous week when 377 samples were collected and tested). ...

                  Ebola disease caused by Sudan virus in Uganda

                  EVENT DESCRIPTION

                  In the past week, a new district Masaka, located in Central
                  Uganda registered a positive case of Ebola disease bringing the
                  total number of affected districts to seven. The confirmed cases
                  (deceased) was a 23-year-old female who earlier admitted at
                  Masaka Regional Referral Hospital where she was identified as
                  a suspect case and isolated. She succumbed to the deadly virus
                  hours after being evacuated to Entebbe treatment centre.
                  As of 6 November 2022, a total of 135 laboratory-confirmed
                  and 21 probable cases have been reported. Seventy-four (74)
                  fatalities have been registered including 53 confirmed cases,
                  resulting to an overall case fatality ratio of 47.4%, and 39.3%
                  among confirmed cases.

                  Cumulatively, 66 patients have been discharged, giving a recovery
                  rate of 48.9%. In total, 18 healthcare worker infections have been
                  recorded including two probable cases, of whom seven (39.0%)
                  have died.

                  Between weeks 43 (24-30 October) and 44 (31 October-6
                  November), the number of new confirmed cases has decreased
                  sharply, by 84.6%, from 39 to six new confirmed cases, while
                  the number of new recoveries has increased by 35.3%, from 17
                  to 23. However, the number of new deaths has kept increasing
                  over the past three weeks, from five deaths in week 42 (17-23
                  October), nine deaths in week 43, to 16 deaths in week 44.
                  No new affected Sub-County has been reported over the past
                  seven days. Hence, the number of affected Sub-Counties
                  remains at 26, distributed in seven Districts of which Mubende
                  has reported majority of cases (65 confirmed and 19 probable;
                  53.8%), followed by Kassanda (43 confirmed and two probable;
                  28.8%) and Kampala (18 confirmed; 11.5%). Kassanda remains
                  the District of highest concern, reporting two-thirds (66.7%) of
                  the new confirmed cases in the past week. No new case has been
                  reported from Mubende over the past week.

                  From the beginning of the outbreak to 5 November 2022, a total
                  of 3 867 contacts have been listed in 14 Districts, of whom 2
                  237 have completed 21 days of follow-up including all those
                  from Bunyangabu, Fort Portal City, Kagadi, Kakumiro, Luweero,
                  Nakasongola, and Ntungamo. On 5 November 2022, 1 396
                  contacts were still under active follow-up in seven
                  Districts including Kampala, Kassanda, Kyegegwa, Masaka,
                  Mitooma, Mityana and Mubende, 91.9% of whom (n=1 274) were
                  seen over the past 24hrs. An average of 1 464 contacts have been
                  followed-up on a daily basis during week 44, which represents a
                  14% decrease from week 43 during which 1 703 contacts were
                  followed-up daily. Since the beginning of week 44, 34 contacts
                  have become suspected cases of SVD.

                  Alert systems have been set-up in Mubende, Kassanda, Greater
                  Kampala Metropolitan with call centre, alert desk and linkage to
                  Emergency Medical Services (EMS) dispatch system. As of 5
                  November, a total of 2 798 alerts have been received, giving an
                  average of 71 alerts per day. Of these, 2 636 (94.2%) alerts were
                  investigated within 24hrs, of which 1 120 (42.5%) alerts were
                  validated as suspected cases of SVD. The proportion of alerts
                  investigated within 24hrs has consistently increased; in week 44,
                  100% (n=622) of alerts were investigated within 24hrs, of which
                  32.6% (n=203) were validated as suspected cases of SVD.

                  Of note, findings from the population mobility mapping exercise
                  conducted by the International Organization for Migration and
                  WHO, prioritized four mobility corridors based on the high
                  volume of population movement and connectivity to and from the
                  epicentre to major towns: Mubende – Kassanda Road, Kassanda
                  – Kampala Road, Kassanda – Mubende – Fort portal – Kasese
                  road and Mubende – Kygegwa – Kazo – Ibanda – Mbarara road.
                  In addition, four key factors were identified to be the main drivers
                  of population mobility to, within and from the five initially affected
                  Districts: gold mining, refugee settlements, socioeconomic
                  factors including pastoral movements, and traditional healers/
                  shrines.

                  PUBLIC HEALTH ACTIONS

                  Coordination

                  Daily District Task Force meetings, partners’ meetings and
                  pillar meetings continue to be held in all affected Districts.

                  National Task Force meetings are held thrice a week.

                  Surveillance and Laboratory

                  Epidemiological investigations, contact tracing, and active
                  case finding are ongoing in all affected Districts. On 5
                  November 2022, 1 386 contacts were still under active
                  follow-up, among whom 1 274 (91.9%) were seen over the
                  past 24hrs.

                  On 5 November, 126 alerts were received nationwide, all of
                  whom were verified; 34 (27.0%) of them were validated as
                  new suspected cases and 9 (26.5%) were evacuated to Ebola
                  Treatment Units (ETUs).

                  On 5 November, 81 samples were processed at Uganda Viral
                  Research Institute and Mubende mobile laboratory, with no
                  new confirmed case.

                  Case management and psychosocial support

                  Four ETUs, one each in Entebbe, Madudu, Mubende, and
                  Mulago are fully operational. The total bed capacities are 182
                  beds for the isolation unit, and 103 beds for the treatment
                  unit. Cumulatively, 66 (48.9%) recoveries have so far been
                  recorded.

                  The training in case management has been completed for
                  cohort 3 participants.

                  On 5 November 2022, Emergency Medical Services teams
                  performed 21 evacuations in Mubende, 13 in Kassanda and
                  one in Kampala/Wakiso.

                  Psychosocial support continues to be offered to suspected
                  and confirmed cases in ETUs, as well as to their family
                  members. Pre-test (and post-test) counselling are also being
                  offered to all patients at ETUs, and to contacts. In addition,
                  the resettling of the discharged persons and negative
                  suspected cases into their community continues.

                  Psychosocial teams are also providing supportive counselling
                  to all staff working at ETUs.

                  Infection, prevention and control (IPC)

                  Daily monitoring of IPC measures is ongoing in all ETUs.

                  IPC and WASH teams continue to participate in the hotspot
                  integrated community engagement activities ongoing in
                  hotspot villages, and plan to include traditional medicine
                  practitioners.

                  Safe and dignified burial teams continue to undertake safe
                  and dignified burials in all affected Districts. Ten safe and
                  dignified burials were performed on 5 November.
                  Risk communication and community engagement

                  RCCE interventions are ongoing in all affected Districts and
                  beyond.

                  At least 120 radio spots messages continue to be aired daily
                  through six radio stations in Mubende, Kassanda, Kagadi,
                  Kakumiro, Buyangabu and Kyegegwa.

                  At total of 350 information, education and communication
                  (IEC) materials have been disseminated in Kagadi and
                  Bunyangabu.

                  House to house sensitization is ongoing in Mubende,
                  Bunyangabu, Kagadi, Kyegegwa and Kassanda Districts, by
                  community volunteers and Village Health Teams, where 898
                  households representing 8 480 people including 4 411 males
                  and 4 069 females have been reached with SVD prevention
                  messages.

                  At least 26 head teachers/deputies from schools in Makindye
                  Division, Kampala have been trained on SVD prevention and
                  response.
                  Logistics

                  The distribution of supplies in healthcare facilities within all
                  affected districts is ongoing, including personal protective
                  equipment, spray pumps, wooden pallets, tarpaulin,
                  essential medicines, discharge packages, fuel, vehicles,
                  stationery, digital thermometers, infra-red thermometers
                  from MoH and partners.

                  SITUATION INTERPRETATION

                  The ongoing response interventions have yielded positive and
                  promising results, with a significant reduction in the number of
                  new confirmed cases over the past week. Of note, no new case
                  has been reported from Mubende over the past seven days,
                  inferring that transmission chains are being controlled locally.
                  However, over the two preceding weeks, majority of cases have
                  been reported from Kassanda where there is still need to scaleup
                  response interventions. The number of deaths has however
                  kept increasing, highlighting some challenges around case
                  management, considering that the chance of survival is dependent
                  of early detection of, and rapid and adequate care to confirmed
                  cases. Moreover, IPC is another area of improvement. A recent
                  evaluation in Kampala has revealed an overall IPC capacity of
                  77% in health care facilities, much lower in private facilities.

                  • Uganda’s capital races to curb spread of Ebola

                    Kampala – Uganda declared an outbreak of the Sudan ebolavirus on 20 September 2022 in the country’s central Mubende district, which is about a two-hour drive away from the capital, Kampala. Some Ebola patients travelled from the outbreak epicentre to Kampala in search of care and by 7 November, there were 17 confirmed cases in the city. Stemming the spread of the virus in a densely populated urban area poses difficulties, as the population is highly mobile. Close-proximity living, and, in some areas, inadequate access to proper sanitation, increases the risk of infection.

                    The health authorities have acted swiftly to mount a robust contact tracing system that has helped to quickly isolate and monitor contacts, as well as provide care to those in need. Public awareness efforts as well as preventive measures are also being reinforced in the city.


                    Every morning, Grace Asiimwe, a health worker in Rubaga division, Kampala, takes a purple folder in which she keeps the forms that help record people’s vital signs and pays a visit to the leaders of the local council.

                    Some of the 17 confirmed Ebola cases reported in Kampala come from this densely populated division.

                    “Going to the local council chairperson of the villages where the contacts are located enables us to have his support and ensures that we are not seen as strangers in the localities, since he is aware of our presence in the area,” says Asiimwe.

                    Every day, Asiimwe and her team monitor nine contacts – people who have come into direct or indirect contact with a person with Ebola. The team checks their temperature and asks a list of questions designed to monitor the onset of Ebola symptoms. Asiimwe then verifies if each contact has been checking their temperature properly and takes note of any change.

                    On her route today, Asiimwe drops by the home of Emmanuel Oreba, a teacher in a school where a child tested positive for Ebola.

                    “This is my 14th day, and my body temperature is normal. In fact, I’m feeling fine,” says Oreba. “At home, I have my own utensils and bedding which I don’t share with the rest of the family until after the mandatory 21 days monitoring is done,” he explains. His biggest challenge right now, he says, is to resist the temptation to escape boredom by taking a stroll outside his house.

                    With four previous outbreaks, Uganda has a long history in dealing with Ebola. This has led to a comprehensive approach to prevention measures in outlying areas.

                    “When a disease like Ebola hits such a big city, communities have a pivotal role to play to curb the spread of the virus,” explains Dr Charles Katureebe, the WHO Coordinator of the Ebola response in Rubaga division of Kampala. “It’s essential to educate and engage the community so that the disease is understood by all… especially the prevention measures, its signs and symptoms so that all suspected cases are reported early and taken care of,” he says.

                    With WHO support, around 50 000 posters and 20 000 leaflets containing information about Ebola, its symptoms, and options for care have been circulated by contact tracers in Kampala.

                    At total of 276 people have been identified as contacts of a confirmed case in Kampala as of 10 November 2022.

                    On 26 October, six children from three different schools in Kampala tested positive for Ebola. They were all contacts of the confirmed case. Their classmates and teachers were quarantined and monitored. The authorities reached out to all the teachers and parents in the school to sensitize them on the need to comply with preventive measures.

                    Other schools in the city were instructed to ensure that all children regularly wash their hands and have their temperatures taken when they arrive in the morning. The thermometers used in the process are provided by the United Nations Children’s Fund.

                    These preventative measures are being rolled out in other institutions across the city.

                    “We work closely with the Kampala district Ebola Taskforce,” says Dr Grace Nanyondo, Medical Director at the Lubaga Hospital in Rubaga division.

                    “At the hospital gates, all people are required to wash their hands before being let in,” she says. “At the triage, we take note of the body temperatures of all patients. If suspicious cases are identified, they are moved to the fully-fledged isolation unit we set up to handle suspected cases.”

                    The hospital will report any patient with Ebola-like symptoms to the Ministry of Health and samples are taken for testing at the Uganda Virus Research Institute. Out of the nine suspected cases recorded so far at the hospital, none have tested positive.

                    Dr Umaru Mutekoba, Head of the Ebola Taskforce at Lubaga Hospital, has just finished checking up on a suspected Ebola patient. After he removes all his personal protective equipment, it will be disposed to avoid any risk of transmission.

                    “The general awareness levels in the country have increased,” he says. “That is why we are seeing more and more suspected cases coming up voluntarily. This is making it easier for contact tracing and management.”

                        A confirmed fatal Ebola case has been reported in Kayalwe B village, Buyengo sub-county, Jinja district. This was a 45-year-old male who is linked to a probable case, his brother.

                        The brother, who is linked to the Rubaga cluster travelled to Jinja where he was ill for 10 days and passed on the 3rd November 2022. The confirmed fatal case also died at his home on 10th November and was buried on 12th November.

                        A sample was obtained on the 10th from the dead body, by healthworkers from St Catherine Medical center, Luzinya Buwenge, a private clinic where he had sought treatment before passing on. Contact tracing and epidemiological investigations have been activated.

                          First Ebola death confirmed in Jinja

                          The 45-year-old...a farmer and resident of Kayalwe B village, in the Buyengo Town Council, Jinja District was admitted at Buwenge Health Center IV following his referral from a private clinic in Buwenge town council early this week.

                          He had earlier on been admitted to the clinic on suspicion that he had typhoid. He presented with fever, diarrhoea, abdominal pain, muscle pain, cough, difficulty in breathing and swallowing, and sore throat.

                          A medical worker at Buwenge Health Center IV, says that when the patient failed to improve, they decided to pick his samples to test for Ebola since he had informed them that he was in close contact with another case with similar signs in Kampala City.

                          The samples were extracted from the patient on November 11, 2022, a few hours before he passed on. His body was handed over to his relatives for burial. However, a report issued by the Uganda Virus Research Institute-UVRI on Saturday night shows that Waiswa succumbed to Ebola.

                            300 medical staff at Rambam Medical Center are being trained on how to treat Ebola patients.
