Uganda's ministry of Health confirms Ebola outbreak in Mubende

            September 20, 2022Written by URN
            ...
Ministry of Health permanent secretary, Dr Diana Atwine told journalists at a press conference at the ministry today that they have dispatched a rapid response team to Madudu and Kiruma villages in Mubende to do a verbal post-mortem on the six reported victims who died between September 11 and 15, 2022.

The team is also expected to gather the contacts of the confirmed case for quarantining. According to Atwine, three adults and three children who died were living in the same family with the confirmed case, which has now been isolated in the mortuary awaiting burial by the medical team.

"We're right now gathering more information on the possible source of infection of the confirmed cases. We also want to let you know that on suspicion of this patient, the district rapid response team, initiated investigations as they'd also been informed of the community deaths. This report was specifically talking about strange illness and sudden deaths in the villages of Madudu and Kiruma sub-counties in Mubende district. The district went on the ground and verified the deaths as follows; a total of 6 deaths had occurred, 3 were adults and 3 were children," said Atwine.

While this case has been confirmed, the exact source of infection is still unclear as epidemiologists both at the ministry and the World Health Organization (WHO), say no country has reported the circulation of the Sudan variant. The neighbouring DRC has over the last three years been registering outbreaks of the Zaire variant as Dr Henry Kyobe explains.

Six of the deceased were residents of different villages in the sub-counties of Madudu and Kiruma in Mubende district.

According to a health report from Mubende regional referral hospital, among the deceased is a 10-month-old baby from Kiwanyi village in Kiruuma sub-county, a two-year-old from Kakeezi in Madudu sub-county, a 32-year-old nurse at St Florence medical clinic (also from Madudu), and a 43-year-old female adult from Bulega among others.

Dr. Emmanuel Batibwa, the director of Mubende hospital says, on September 15 during the Mubende hospital morning assembly, it was noted that there was an increase in the number of hospital deaths; some attributed to fevers and bleeding. Five of the six people had initially been diagnosed with malaria, while others reportedly stormed health facilities with fever and bleeding.

On Saturday, the regional response team sat and decided to isolate the patient in the MDR-TB ward. Samples were collected from the patient and sent to UVRI for laboratory confirmation. At the same time, a team was dispatched off to Madudu to investigate the cause of the increasing deaths.

He says the laboratory findings from UVRI indicate that the admitted patient that actually passed on Monday morning tested positive for Ebola Sudan variant.

Rare Ebola outbreak declared in Uganda

              © WHO
For the first time in more than a decade, Uganda has recorded an outbreak of the Sudan strain of Ebola virus. 20 September 2022

              Health

An outbreak of Ebola virus has been declared in Uganda after a case was confirmed in Mubende district, in the centre of the country.

The UN World Health Organization (WHO) said on Tuesday that a sample taken from a 24-year-old man was identified as the relatively rare Sudan strain.

It is the first time in more than a decade that the Sudan strain has been found in Uganda, which also saw an outbreak of the Zaire strain of Ebola virus in 2019.

              Suspicious deaths

The latest outbreak follows six suspicious deaths in Mubende district so far this month. There are also eight suspected cases who are receiving care in a health facility.

Dr Matshidiso Moeti, World Health Organization Regional Director for Africa, said that the UN agency was working closely with Ugandan authorities to investigate the source, and support efforts to control it.

“Uganda is no stranger to effective Ebola control”, she said. “Thanks to its expertise, action has been taken to quickly to detect the virus and we can bank on this knowledge to halt the spread of infections.”

              No effective vaccine

Existing vaccines against Ebola have proved effective against the Zaire strain but it is not clear if they will be as successful against the Sudan strain, WHO said in a statement.

Ebola is a severe, often fatal illness affecting humans and other primates. It has six different strains, three of which - Bundibugyo, Sudan and Zaire - have previously caused large outbreaks.

Case fatality rates of the Sudan strain have varied from 41 per cent to 100 per cent in past outbreaks. Early roll-out of supportive treatment has been shown to significantly reduce deaths from Ebola, WHO said.

              Sending supplies

The agency has dispatched supplies to support the care of patients and is sending a specialized tent that will be used to isolate patients.

While ring vaccination of high-risk people with Ervebo (rVSV-ZEBOV) vaccine has been highly effective in controlling the spread of Ebola in recent outbreaks in DRC and elsewhere, said WHO, this vaccine has only been approved to protect against the Zaire strain.

Another vaccine produced by pharmaceutical company Johnson and Johnson may be effective but has yet to be specifically tested against the Sudan strain.

An outbreak of Ebola virus has been declared in Uganda after a case was confirmed in Mubende district, in the centre of the country.

WHO bolsters Ebola disease outbreak response in Uganda

                      22 September 2022

Brazzaville – In an accelerated effort to scale up response to the Ebola disease outbreak in Uganda, World Health Organization (WHO) is delivering medical supplies, providing logistics and deploying staff to support the national authorities in halting the spread of the virus.

So far, seven cases, including one death, have been confirmed to have contracted the Sudan ebolavirus – one of the six species of the Ebolavirus genus. Forty-three contacts have been identified and 10 people suspected to have caught the virus are receiving treatment at the regional referral hospital in Mubende, the district where the disease was confirmed this week, making it the first time Uganda has detected the Sudan ebolavirus since 2012.

Mubende is in the central region of Uganda and is about a two-hour drive from the capital Kampala and sits along a busy road leading to the Democratic Republic of the Congo. There are gold mines in the area, which attract people from different parts of Uganda, as well as other countries. The mobile nature of the population in Mubende increases the risk of a possible spread of the virus.

WHO has deployed a technical team to Mubende district to support surveillance, infection prevention and control and the management of cases. The Organization is also assisting in the activation of surveillance structures in neighbouring districts and is repurposing its country-based staff to bolster the response. In addition, five international experts will be deployed, with numbers increasing if needed.

“We are acting quickly and decisively to draw the reins on this outbreak. Our experts are already on the ground working with Uganda’s experienced Ebola control teams to reinforce surveillance, diagnosis, treatment and preventive measures,” said Dr Abdou Salam Gueye, Regional Emergency Director with the WHO Regional Office for Africa. “Africa’s stronger homegrown emergency readiness is proving ever more crucial in tackling outbreaks such as Ebola.”

Due to Uganda’s previous outbreaks and the threat of importation of cases from neighbouring Democratic Republic of the Congo that has fought several outbreaks, WHO and the Ministry of Health have collaborated on many preparedness activities, the last such exercise was in August 2022, where nine Ugandan clinicians were trained on how to manage viral haemorrhagic fevers and are now working in the response.

WHO already has six viral haemorrhagic fever kits in Uganda, and one has been delivered to Mubende. While there are no therapeutics that specifically treat Sudan ebolavirus species, early identification of cases and treatment of symptoms greatly increases the chances of survival.

Current evidence shows that ERVEBO vaccine, which is highly effective against the Zaire ebolavirus, doesn’t provide cross protection against the Sudan ebolavirus.

There are at least six candidate vaccines against Sudan ebolavirus which are in different stages of development. Three of them have Phase1 data (safety and immunogenicity data in humans) and the remaining are in the preclinical evaluation phase.

The WHO Research and Development Blueprint team is in contact with all developers and is leading a collaborative effort involving international experts to determine which vaccine(s) may be suitable for additional evaluation during this outbreak (and if doses with required standards are available) should more cases be confirmed. A CORE protocol for their evaluation exists and WHO will discuss the proposed next steps with the Ugandan authorities to seek their approval.

WHO held a press conference today led by Dr Patrick Otim, Health Emergency Officer, Acute Events Management Unit, WHO Regional Office for Africa. He was joined by Dr Kyobe Henry Bbosa, Ebola Incident Commander, Ministry of Health of Uganda; Dr Ana Maria Henao-Restrepo, Co-Lead Research and Development Blueprint for epidemics, WHO Health Emergencies Programme; and Dr William A. Fischer II, Director of Emerging Pathogens at Institute for Global Health and Infectious Diseases and Assistant Professor of Medicine in the Division of Pulmonary and Critical Care Medicine, University of North Carolina.

In an accelerated effort to scale up response to the Ebola disease outbreak in Uganda, World Health Organization (WHO) is delivering medical supplies, providing logistics and deploying staff to support the national authorities in halting the spread of the virus.

Mubende Ebola task force has suspended the operation of all entertainment places and congested public places in the district. The task force has also restricted access to public offices that are considered high-risk facilities in a bid to curb the spread of the Ebola virus disease.

According to the Mubende RDC, Rosemary Byabashaija who also doubles as the head of the task force, a meeting held on Tuesday evening also resolved to suspend cinemas, markets, and events such as weddings.

She says due to limited manpower to enforce this directive, the task force is currently focused on villages in Madudu sub-county where an isolation camp has been set up but it applies to the entire district.

The latest Ebola outbreak has so far claimed 2 lives with several other suspected cases still under medical investigation.

Health officials and civic leaders have suspended operations of entertainment centres and banned crowded burials and wedding parties in Mubende District in a race to break the chain of Ebola Virus Disease (EVD) spread in the area.

This followed the confirmation on Tuesday that a 24-year-old man who passed on at Mubende Regional Referral Hospital on Monday had Ebola of Sudan strain.

Responders yesterday isolated 11, raising the number of confirmed and probable Ebola fatalities and infections to 18, according to a Health ministry update.

Dr Paul Batiibwe, the director of Mubende Regional Referral Hospital, confirmed that a woman, a resident of Ngabano in Madudu Sub-county, succumbed to EVD at the facility yesterday.

In Kampala, Ministry of Health Spokesman Emmanuel Ainebyoona said he had not received the report, but instead confirmed the death of a child linked to probable Ebola.

After eight Ebola deaths, Uganda is gearing up to stem the spread

                            RFI

At least eight people are believed to have died of Ebola in Uganda, an official from the Ministry of Health confirmed on Thursday (September 22). Authorities in the East African country announced the Ebola outbreak on Tuesday after a youth died in Mubende in the central region. The origin is not yet known, but it is a " relatively rare " strain, nicknamed " Sudanese ". It is less fatal than that detected in the DRC, but vigilance is still required.

For the moment, no vaccine exists against the “ Sudanese ” strain of Ebola. The Ugandan authorities are therefore relying on responsiveness to detect and treat suspected cases as soon as possible. But the mobility of the population of Mubende, the center of the epidemic , increases the risk of a possible spread of the virus.

While for the moment we do not know what could have caused the outbreak of this highly contagious disease, the response is being organized to stop the spread of the virus.

Henry Kyobe Bbosa, in charge of the Ebola response at the Ugandan Ministry of Health, spoke Thursday, September 22 during an online press briefing from the World Health Organization (WHO): " The epicenter is along a highway that comes from South Sudan, passes through Kampala and goes to the DRC. We have therefore expanded our response to 11 districts neighboring the focus of the epidemic. »

Our priority is to find people who have had contact with patients, treat the sick, protect health personnel, and raise awareness in order to stop the spread. There are several hypotheses about the origin of this strain. It could be related to the mango season, because at the end of this season, many children start eating everything they can find. They could have contaminated themselves at that time. But for now, these are just assumptions. The epicenter is in the district of Mubende. And a case has been detected in a neighboring district. With regard to outbreaks of Ebola, a radius of 20 to 30 kilometers is monitored. Even if of course, our field of action extends to the whole country.

Ugandan officials are trying to trace around 40 people who had contact with the deceased patients. For its part, WHO has deployed a technical team to Mubende district to support the response. The organization also calls on neighboring countries to remain vigilant:

“ The countries of the region, explains Patrick Otim, head of health emergencies at the Regional Office for Africa , in particular the six countries which are South Sudan, Rwanda, Burundi, the DRC, Kenya and Tanzania, must strengthen their preparedness in the event of an epidemic and improve their resilience mechanisms in order to detect any suspected case and be able to react effectively. »

Uganda has put in place border control measures aimed at limiting the spread of the virus.
