        Sacramento County Department of health confirm first case of coronavirus

        SACRAMENTO, Calif. — Sacramento has its first confirmed case of coronavirus [COVID-19], according to Sacramento County Public Health officials.

        The Sacramento County resident recently traveled from China, the epicenter of the outbreak, and returned on Feb. 2. Upon arrival, the individual self-quarantined as a precautionary measure. The person began exhibiting mild symptoms during their quarantine period and after consulting with the individual's doctor, the Sacramento County Public Health coordinated testing with the Centers for Disease Control and Prevention [CDC] and the California Department of Public Health [CDPH]. A clinical specimen was collected from the individual and sent to CDC where laboratory testing confirmed the infection, according to Sacramento County Public Health...

            BREAKING: The Centers for Disease Control and Prevention has been informed of the first case of the coronavirus in a person who did not recently return from a foreign country or have contact with a confirmed case, according to a person briefed on the case. Officials have begun tracing the contacts of the resident to find out how the person may have been infected and who else might have been exposed..

              Per NBC - KCRA - Sacramento:

              There is a new case of novel coronavirus confirmed in Northern California, a Centers for Disease Control and Prevention spokesperson said Wednesday.
              The case was confirmed through the public health system in Northern California, the CDC said.
              Officials said the case is not connected to the Diamond Princess cruise ship.
              It's unclear how the patient contracted the virus.
              No other details about the new case of the coronavirus were released.

                CDC Confirms Possible Instance of Community Spread of COVID-19 in U.S.


                The Centers for Disease Control and Prevention (CDC) has confirmed an infection with the virus that causes COVID-19 in California in a person who reportedly did not have relevant travel history or exposure to another known patient with COVID-19.

                At this time, the patient’s exposure is unknown. It’s possible this could be an instance of community spread of COVID-19, which would be the first time this has happened in the United States. Community spread means spread of an illness for which the source of infection is unknown. It’s also possible, however, that the patient may have been exposed to a returned traveler who was infected.

                This case was detected through the U.S. public health system — picked up by astute clinicians. This brings the total number of COVID-19 cases in the United States to 15.

                The federal government has been working closely with state, local, tribal, and territorial partners, as well as public health partners, to respond to this public health threat. Unprecedented, aggressive efforts have been taken to contain the spread and mitigate the impact of this virus.

                  CDC Confirms Possible First Instance of COVID-19 Community Transmission in California

                  SACRAMENTO – The U.S. Centers for Disease Control and Prevention today confirmed a possible first case of person-to-person transmission of COVID-19 in California in the general public. The individual is a resident of Solano County and is receiving medical care in Sacramento County. The individual had no known exposure to the virus through travel or close contact with a known infected individual.

                  California has a strong health care system and public health infrastructure. California has prepared for the potential spread of diseases, such as H1N1, in the past and is prepared and actively responding to the potential community spread of COVID-19. Contact tracing in this case has already begun.

                  The health risk from novel coronavirus to the general public remains low at this time. While COVID-19 has a high transmission rate, it has a low mortality rate. From the international data we have, of those who have tested positive for COVID-19, approximately 80 percent do not exhibit symptoms that would require hospitalization. There have been no confirmed deaths related to COVID-19 in the United States to date. California is carefully assessing the situation as it evolves.

                  "Keeping Californians safe and healthy is our number one priority," said Dr. Sonia Angell, Director of the California Department of Public Health and State Public Health Officer. "This has been an evolving situation, which California has been monitoring and responding to since COVID-19 cases first emerged in China last year. This is a new virus, and while we are still learning about it, there is a lot we already know. We have been anticipating the potential for such a case in the U.S., and given our close familial, social and business relationships with China, it is not unexpected that the first case in the U.S. would be in California. That’s why California has been working closely with federal and local partners, including health care providers and hospitals, since the outbreak was first reported in China -- and we are already responding."

                  As in any public health emergency, the Department of Public Health's Emergency Operations Center has been actively coordinating response efforts across the state and preparing for possible community transmission. California continues to prepare and respond in coordination with federal and local partners.

                  This would be the first known instance of person-to-person transmission in the general public in the United States. Previously known instances of person-to-person transmission in the United States include one instance in Chicago, Illinois, and one in San Benito County, California. Both cases were after close, prolonged interaction with a family member who returned from Wuhan, China and had tested positive for COVID-19, the disease caused by novel coronavirus. As of today, including this case, California has had 7 travel-related cases, one close contact case, and now one community transmission.

                  As with any virus, especially during the flu season, the Health Department reminds you there are a number of steps you can take to protect your health and those around you:

                  Washing hands with soap and water.

                  Avoiding touching eyes, nose or mouth with unwashed hands.

                  Avoiding close contact with people who are sick are all ways to reduce the risk of infection with a number of different viruses.

                  Staying away from work, school or other people if you become sick with respiratory symptoms like fever and cough.

                  The California Department of Public Health will not be providing additional information about this patient due to patient confidentiality. For more information about novel coronavirus, please visit the CDPH website.

                    -No details on why patient was tested but they believe the result. No travel history or known contact to travel or PUI. But when they called about doing testing it was approved.
                    -No current change in testing recommendations based on this at this time however they state likely to occur soon. If concern about a patient that you think may be appropriate for testing then reach out to Public Health for review.
                    -Identifying and Testing HCW as a priority for the case.
                    -Concern about turn around time for results and use of PPE while waiting.

                      Nevertheless, a small number of medical center employees have been asked to stay home and monitor their temperatures.

                      Lubarsky and Simmons said the patient was transferred to UC Davis from “another Northern California hospital” on Wednesday, Feb 19.

                      “When the patient arrived, the patient had already been intubated, was on a ventilator, and given droplet protection orders because of an undiagnosed and suspected viral condition,” the email said...

                        Health officials are working to identify health care workers who came into contact with a coronavirus patient from Solano County now being treated at UC Davis Medical Center.

                        Travis Air Force Base in Solano County accepted travelers from China and the Japanese cruise ship for quarantine, but this is the first case of community transmission.

                          There are indications that other hospitals could be involved in the case. Kris Concepcion, fire chief and acting public information officer in Vacaville, Calif., said county officials had issued a directive not to transport any new patients to two local hospitals — NorthBay VacaValley Hospital in Vacaville and NorthBay Medical Center in nearby Fairfield. Concepcion declined to say why those orders had been given.
                          ... Steve Huddleston, vice president of public affairs at NorthBay Healthcare, declined to comment. He said the hospital was directing all inquiries to the California Department of Public Health.
                          The two hospitals are in Solano County, home to Travis Air Force Base, where hundreds of Americans repatriated from China and others brought home from the Diamond Princess cruise ship have been kept in quarantine. Many of them have been released.

                            David Lubarsky, vice chancellor of Human Health Sciences and CEO of UC Davis Health, and Brad Simmons, interim CEO and chief operating officer for UC Davis Medical Center, sent the following letter to UC Davis Health employees on Feb. 26:

                            Today we learned a patient we are treating here at UC Davis Medical Center for the novel coronavirus (COVID-19) is being investigated by the Centers for Disease Control and Prevention (CDC) as possibly the first patient to have received the infection from exposure in the community.

                            This patient was transferred to us from another Northern California hospital. Since the patient arrived with a suspected viral infection, our care teams have been taking the proper infection prevention (contact droplet) precautions during the patient’s stay.

                            Upon admission, our team asked public health officials if this case could be COVID-19. We requested COVID-19 testing by the CDC, since neither Sacramento County nor the California Department of Public Health is doing testing for coronavirus at this time. Since the patient did not fit the existing CDC criteria for COVID-19, a test was not immediately administered. UC Davis Health does not control the testing process.

                            Sunday (Feb. 23), the CDC ordered COVID-19 testing of the patient, and the patient was put on airborne precautions and strict contact precautions, because of our concerns about the patient’s condition. Today the CDC confirmed the patient’s test was positive.

                            This is not the first COVID-19 patient we have treated, and because of the precautions we have had in place since this patient’s arrival, we believe there has been minimal potential for exposure here at UC Davis Medical Center.

                            We are proud of our health care workers who have been working to care for this patient and are committed to saving this patient’s life. Just as when a health care worker has a small chance of exposure to other illnesses, such as TB or pertussis, we are following standard CDC protocols for determination of exposure and surveillance. So, out of an abundance of caution, in order to assure the health and safety of our employees, we are asking a small number of employees to stay home and monitor their temperature. We are handling this in the same way we manage other diseases that require airborne precautions and monitoring. We are in constant communication with the state health department and the CDC and Sacramento County Public Health about the optimal management of this patient and possible employee exposures.

                            As we regularly handle patients with infectious diseases, we have robust infection control protocols in place to handle this patient and others with more frequently seen infectious diseases. In this case, we are dedicated to providing the best care possible for this patient and continuing to protect the health of our employees who care for them.
