Headline News/Nusantara/Monday, 23 February 2009.

Metrotvnews.com, Tangerang: A boy resident of Gruduk Mekar Jaya, Sepatan, Tangerang Banten suspected to be infected by bird flu virus. It has been over a week that Muhammad Heru (2) is developing high fever, coughing and breathing difficulty.

Heru, son of Rusdi and Siti Isnia, is now treated at Tangerang regional hospital after received treatment at Sepatan public health center before. His condition hasn’t showed any improvement.

According to victim’s neighbor, dead chickens were founded around Heru’s house lately. People worried if bird flu H5N1 virus has spread in their neighborhood.

Since the beginning of the year 2009, Indonesia has reported several bird flu outbreaks in chickens and human suspected cases.

The first recorded and officially confirmed human H5N1 infected victims were 29-year-old woman from Tangerang and 6-year-old girl from Bekasi.

Since then several human suspect cases appeared and one of them, St (F/28) from Surabaya had been confirmed positive H5N1 infection. However, the former case hasn’t been officially announced as positive case by Indonesia government.

Other suspect cases from Demak, Bali, Cirebon and Maros had been reported negative by media news according to medical examination, meanwhile some others remain uncleared.

In Bogor, last February 2009, two victims had possibly killed by bird flu virus followed by one more suspected ill victim from the same area. Those three victims were also known as siblings. Eventhough medical examination showed highly possibility of bird flu infection, but no confirmation has been released until now.

Somehow, the fatal victims from Bogor were also buried strictly as they might have infectious disease. Most recent cases, involving 7 human suspected victims from Bekasi are also still under investigation.

Most all cases have contact history with dead chickens, or dead chickens findings around the neighborhood or visiting wet market. Two cases, NA (F/43) from Cengkareng and St (F/28) from Surabaya reported had no any contact with birds nor did visit wet market.

Avian influenza news whether related to chicken outbreaks or human suspected cases is not popular topics in Indonesia lately. This may be due to other domestic hot news such as upcoming general election event and national incidents as earthquakes, transportation accident and political issues.

Surabaya Avian Influenza Human Case

On January 24, 2009 there was a new human case of Avian Influenza infection. The victim, S, 21 years, was a housemaid. She was living in Darmo Permai.

On January 31, 2009 we went to the house of S and investigated the factor of infection. After we arrived there, many questions occured in our mind. How can she get infected? in what way? why there were many questions in our mind because we saw that the living are were clean and hygiene. No pets, no bird and no chicken in her house.

According to her friend who lived there too, she had not contact with chicken before she got sick and she have had fever 3 days before finally the family took her to Soetomo Hospital.

After we investigated deeply, we found the fact that everyday she went to wet market, which was about 1 km from her house. And in the wet market, we found 2 chicken seller who pooled chicken from many area in East Java. Until now we are still tracking the origin of the chicken and do the surveillance in that wet market.

And on February 2, 2009 we went to Muji Rahayu hospital. It was the first hospital which S came to the medical doctor and hospitalized before she finally was brought to Soetomo Hospital. In this hospital there were 32 health worker who involved directly to treat the victim.

Until now, the investigation is still going on.

FLASH NEWS : we got information that 5 staff of Soetomo Hospital are quarantine because suspected bird flu. One of the suspect is medical doctor.

Two suspected human cases of avian influenza in Tangerang, one fatal

A child (aged 2 years 9 months) residing in the Serpong Subdistrict
of Tangerang City South [Banten province] is suspected to have died
as a result of avian influenza virus infection.

Another Tangerang resident (a 15-year-old boy) is suspected also to
have contracted avian influenza virus infection and is currently
being treated in hospital.

The child's father stated that his son developed a high fever on 14
Feb 2009 and he immediately took the child to the local community
health centre and contacted his doctor. After 3 days the clinic
transferred the child to hospital on suspicion that he was suffering
from avian influenza virus infection. However despite intensive care
the condition of the child continued to deteriorate and he died on 17
Feb 2009. In a statement to correspondents the father admitted he did
not know the precise cause of his son's death, and so far he had been
given no information by the hospital. The child has now been buried
in TPU Kramat Tajug.

An [agricultural service official] stated that he has received
information from Litbangkes (Laboratory Dep of Health) that the child
had tested positive for avian H5N1 influenza virus infection. [This
official] had been responsible for collecting samples from poultry in
the area, but had not yet received results of the tests.

This is related with the flash news from AI lab unair, posted 4th of February. A doctor (anesthesiologist) and 4 paramedics of Dr Sutomo hospital Surabaya were quarantined due to bird flu-like symptoms on early February.

This is right after one Surabaya bird flu victim (St/18) was treated in that hospital. Further test revealed that the doctor and one of paramedic tested negative bird flu infection.

Up to last Sunday (22/2), three other paramedics are still in critical condition.

Ilness onset of each health worker is vary.

They had developed bird flu-like symptoms.

This protocol contributes to the high case fatality rate, which has been close to 80% since 2005 and suggests a high level of unreported milder cases.

The recent fixing of Tamiflu resistance, H274Y in H1N1 seasonal flu has increased concerns that co-infections involving season flu with H274Y and H5N1 in hosts being treated with Tamiflu will selected Tamiflu resistant H5N1.

Indonesia has imposed a media blackout, and reports confirmed cases on a delayed basis. The delay is reflected in WHO updates, and continues to violate International Health Regulations. The latest confirmed cases from Indonesia were also from the Jakarta area. The January 22 WHO update described a Tangerang housekeeper (29F) who developed symptoms December 11 and died December 16. The second confirmed case (5F) was in the Jakarta suburb of Bekaski who developed symptoms December 23 and died January 22.. These reporting delays, as well as these more recent cases in Bogor and Tangerang continue to raise concerns about Indonesian reporting and WHO enforcement of these reporting delays.

The most recent cases are linked to confirmed or suspect clusters, which also increase concerns.

Timely reporting of these lab confirmed cases would be useful.

The above health care workers are associated with the same facility where an H5N1 confirmed patient died in late January (see updated map). Although this is not an official case, hospital personal indicate H5N1 was confirmed, even though the patient did not have a history of contact with poultry.

If the patient and health care workers are H5N1 infected, they would represent one of the largest H5N1 clusters in Indonesia reported to date. Health care workers are rarely infected with H5H1 because they are aware of hazards H5N1 infected patients prevent. Moreover, they are quickly treated, so they rarely progress to a critical state. The report of three health workers in critical condition is alarming.

More information on the status of these patients and test results would be useful.
